Improving the experience of patients in contact isolation by human-centered design: A proof of concept study using patient journey mapping
No full textBackground: Patient information provision about isolation measures is often not standardized. Moreover, it is unclear what information patients and informal caregivers need to optimize their knowledge and actual experience of contact isolation. We explored opportunities for improving the experience of patients in contact isolation from a human-centered design perspective.Method: We used patient journey mapping and created patient personas based on observations and interviews with patients and health care workers from a hematology, surgery and acute admission ward of a Dutch university hospital.Result: Patients differed in their experience of contact isolation; three personas were identified. The first liked being in contact isolation, the second had to get used to the measures, the third experienced contact isolation as overwhelming. Patients experienced a lack in structured and tailored information about contact isolation. Nurses confirmed this lack of structure and reported their need for clear instructions on how to inform patients and their informal caregivers and what information to provide at which point in time. By using a human-centered design approach, we created a first draft of an interactive information path for patients, informal care givers and health care workers.Conclusion: To improve the patient experience in contact isolation, patient information could be structured throughout the patient journey. As this issue quickly follows our last there are only a few news items to share with you.
The Forest of Reading / Festival of Trees 2019 which is a national festival, held its largest event in Toronto from May 14 to 16. Students read a variety of Canadian children’s books throughout the year and then vote on their favourite book. Awards are then granted for books in a variety of different categories. The Canadian Children’s Book Centre has a lovely summary article about the festival that includes a lengthy list of all the winners. You can also see all the nominees for the various awards on the Ontario Library Association website.
The Vancouver Children\u27s Literature Roundtable (VCLR) has announced it’s Information Book Award Shortlist. It includes eight juvenile non-fiction titles. Voting for winners continues into the fall and the winner will be announced in November.
The Canadian Children’s Book Centre’s Annual General Meeting will be taking place on Wednesday, June 5, 2019 at 6:00 pm. CCBC members and the general public are welcome to attend: Room 224, Northern District Library / 40 Orchard View Blvd. / Toronto, Ontario M4R 1B9
On a final note, the Children’s book author Judith Kerr passed away earlier this month. Kerr was known for her book The Tiger that Came to Tea. The New York Times recently published an obituary tribute to Kerr.

SMPDL3b modulates insulin receptor signaling in diabetic kidney disease
Get PDFSphingomyelin phosphodiesterase acid-like 3b (SMPDL3b) is a lipid raft enzyme that regulates plasma membrane (PM) fluidity. Here we report that SMPDL3b excess, as observed in podocytes in diabetic kidney disease (DKD), impairs insulin receptor isoform B-dependent pro-survival insulin signaling by interfering with insulin receptor isoforms binding to caveolin-1 in the PM. SMPDL3b excess affects the production of active sphingolipids resulting in decreased ceramide-1-phosphate (C1P) content as observed in human podocytes in vitro and in kidney cortexes of diabetic db/db mice in vivo. Podocyte-specific Smpdl3b deficiency in db/db mice is sufficient to restore kidney cortex C1P content and to protect from DKD. Exogenous administration of C1P restores IR signaling in vitro and prevents established DKD progression in vivo. Taken together, we identify SMPDL3b as a modulator of insulin signaling and demonstrate that supplementation with exogenous C1P may represent a lipid therapeutic strategy to treat diabetic complications such as DKD. © 2019, The Author(s)
New ophthalmosaurid ichthyosaurs from the European lower cretaceous demonstrate extensive ichthyosaur survival across the Jurassic–Cretaceous boundary

Ichthyosauria is a diverse clade of marine amniotes that spanned most of the Mesozoic. Until recently, most authors interpreted the fossil record as showing that three major extinction events affected this group during its history: one during the latest Triassic, one at the Jurassic–Cretaceous boundary (JCB), and one (resulting in total extinction) at the Cenomanian-Turonian boundary. The JCB was believed to eradicate most of the peculiar morphotypes found in the Late Jurassic, in favor of apparently less specialized forms in the Cretaceous. However, the record of ichthyosaurs from the Berriasian–Barremian interval is extremely limited, and the effects of the end-Jurassic extinction event on ichthyosaurs remains poorly understood.
Methodology/Principal Findings
Based on new material from the Hauterivian of England and Germany and on abundant material from the Cambridge Greensand Formation, we name a new ophthalmosaurid, Acamptonectes densus gen. et sp. nov. This taxon shares numerous features with Ophthalmosaurus, a genus now restricted to the Callovian–Berriasian interval. Our phylogenetic analysis indicates that Ophthalmosauridae diverged early in its history into two markedly distinct clades, Ophthalmosaurinae and Platypterygiinae, both of which cross the JCB and persist to the late Albian at least. To evaluate the effect of the JCB extinction event on ichthyosaurs, we calculated cladogenesis, extinction, and survival rates for each stage of the Oxfordian–Barremian interval, under different scenarios. The extinction rate during the JCB never surpasses the background extinction rate for the Oxfordian–Barremian interval and the JCB records one of the highest survival rates of the interval.
Conclusions/Significance
There is currently no evidence that ichthyosaurs were affected by the JCB extinction event, in contrast to many other marine groups. Ophthalmosaurid ichthyosaurs remained diverse from their rapid radiation in the Middle Jurassic to their total extinction at the beginning of the Late Cretaceous

Get PDFTese (doutorado) - Universidade Federal de Santa Catarina, Centro de Filosofia e Ciências Humanas, Programa de Pós-Graduação Interdisciplinar em Ciências Humanas, Florianópolis, 2009Partindo de dados encontrados em pesquisa de campo realizada em quatro Tribunais de Justiça brasileiros, e com o fundamento teórico dos estudos de gênero e antropologia do parentesco, analiso as posições jurídicas e políticas sobre casamento e família, identificadas pela leitura de 185 acórdãos judiciais e interlocução com 25 desembargadores acerca do tema das uniões entre pessoas do mesmo sexo. Considerando as transversalidades entre os discursos jurídicos e as convicções morais e políticas de seus/suas autores/as, a análise aponta, entre outras questões, a forte influência destes fatores sobre a tomada de decisões judiciais relativas ao acesso a direitos sexuais e possíveis implicações na (re) produção de subjetividades marginais ao acesso aos direitos de família. A análise dos discursos de documentos e de entrevistas com desembargadores sobre o enquadramento das conjugalidades homoeróticas ora no conceito de sociedade de fato ora no de união estável demonstra que a tendência jurisprudencial acentua as disputas políticas sobre a interpretação jurisprudencial, concretizadas nas distintas posições assumidas pelos relatores dos tribunais em estudo, e que se materializam basicamente na controvérsia quanto à necessidade de existência de lei específica para tomada de decisão e/ou interpretação da constituição federal como tarefas impostas aos juízes da atualidade para a resolução dos conflitos postos pela demanda de reconhecimento e de redistribuição de direitos. Os resultados apontam ainda para uma tendência que pode ser considerada mais "positiva" do que "negativa" dos tribunais em estudo, se a discussão for relativizada de um ponto de vista da partilha de bens, central nas disputas judiciais. Por outro lado, a maioria dos relatores mantêm o conceito de família num lugar "naturalizado", seja pelas convicções expressas em entrevistas e acórdãos acerca da conversão em matrimônio civil prevista como conseqüência deste enquadramento, seja pela consideração que se faz acerca do valor patrimonial ou não do trabalho doméstico nas relações entre pessoas do mesmo sexo, ou mesmo sobre a posição das crianças junto ao casal de gays ou de lésbicas, gerando conseqüências que por outro lado são reconhecidas como variáveis e instáveis para a vida dos sujeitos que litigam sob o vazio da lei. From data found on field research conducted in four Brazilian Courts of Justice, and the basis of theoretical studies of gender and anthropology of kinship, I analyze the legal positions and policies on marriage and family, identified by the reading of 185 judicial rulings and interlocution with 25 magistrates about the issue of unions between persons of the same sex. Considering the cross between the words legal and moral and political beliefs of his / her author / as the analysis shows, among other issues, the strong influence of these factors on judicial decision-making on access to sexual rights and possible implications for (re) production of subjectivities marginal access rights of the family. The analysis of speeches of documents and interviews with magistrates on the environment of conjugality homoerotic moment in the concept of society in fact now in the stable case law shows that the trend highlights the political disputes on the case-law, implemented in different positions taken by the rapporteurs the courts under study, which is basically materialized in dispute as to the need of specific law in making decisions and / or interpretation of the federal constitution as tasks imposed on judges of today to resolve the conflict posed by the demand for recognition and redistribution of rights. The results also point to a trend that can be considered more "positive" than "negative" study in the courts if the thread is relativised from a sharing of property, central in judicial disputes. Moreover, most of the rapporteurs retains the concept of family in a place "naturalized" is the belief expressed in interviews and judgments about the conversion to civil marriage planned as a result of this framework is the consideration that is about the value or non-domestic work in the relations between persons of the same sex, or even the position of the couple with children of gays and lesbians, creating consequences that the other variables are known to be unstable and subject to the life of that audience in the vacuum of law
Endothelium-derived NO, but not cyclic GMP, is required for hypoxic augmentation in isolated porcine coronary arteries
No full textThe present study investigated the mechanism underlying the transient potentiation of vasoconstriction by hypoxia in isolated porcine coronary arteries. Isometric tension was measured in rings with or without endothelium. Hypoxia (Po(2) <30 mmHg) caused a transient further increase in tension (hypoxic augmentation) in contracted (with U46619) preparations. The hypoxic response was endothelium dependent and abolished by inhibitors of nitric oxide synthase [N(omega)-nitro-L-arginine methyl ester (L-NAME)] or soluble guanylyl cyclase (ODQ and NS2028). The addition of DETA NONOate (nitric oxide donor) in the presence of L-NAME restored the hypoxic augmentation, suggesting the involvement of the nitric oxide pathway. However, the same was not observed after incubation with 8-bromo-cyclic GMP, atrial natriuretic peptide, or isoproterenol. Assay of the cyclic GMP content showed no change upon exposure to hypoxia in preparations with and without endothelium. Incubation with protein kinase G and protein kinase A inhibitors did not inhibit the hypoxic augmentation. Thus the hypoxic augmentation is dependent on nitric oxide and soluble guanylyl cyclase but independent of cyclic GMP. The hypoxic augmentation persisted in calcium-free buffer and in the presence of nifedipine, ruling out a role for extracellular calcium influx. Hypoxia did not alter the intracellular calcium concentration, as measured by confocal fluorescence microscopy. This observation and the findings that hypoxic augmentation is enhanced by thapsigargin (sarco/endoplasmic reticulum calcium ATPase inhibitor) and inhibited by HA1077 or Y27632 (Rho kinase inhibitors) demonstrate the involvement of calcium sensitization in the phenomenon.© 2011 the American Physiological Society.link_to_OA_fulltex
Surgical treatment of acromioclavicular dislocation associated with midshaft fracture of the ipsilateral clavicle
No full textAcromioclavicular dislocation combined with a midshaft clavicle fracture is a rare traumatic shoulder problem. Various treatment options have been described in literature. We describe a new technique using a hook plate and coracoclavicular sling to replace the ruptured coracoclavicular ligaments in combination with plate osteosynthesis of the clavicle fracture. Furthermore, we provide a short overview of the few cases described in literature and their treatment options.
