    Effectiveness of a rehabilitation program for adults with facial trauma and mandibular mobility restriction

    Objetivo: Verificar a efetividade de um programa de reabilitação fonoaudiológica para pacientes adultos com trauma de face e restrição na mobilidade mandibular por meio de avaliação clínica da motricidade orofacial e mobilidade mandibular, autopercepção do paciente e avaliação eletromiográfica dos músculos mastigatórios. Métodos: Os 65 participantes deste estudo foram divididos em três grupos: GP1- composto por 24 participantes submetidos à avaliação fonoaudiológica em até um mês após a correção da fratura; GP2- composto por 29 participantes submetidos à avaliação fonoaudiológica de um a três meses após a correção da fratura; GP3 - composto por 12 participantes submetidos à avaliação fonoaudiológica de três meses ou mais após a correção da fratura. Todos os participantes foram submetidos à avaliação constituída pela aplicação de um protocolo clínico para a avaliação da motricidade orofacial, da mobilidade mandibular e de autopercepção dos sinais e sintomas de DTM. Os participantes foram submetidos ao Programa Terapêutico Fonoaudiológico para Traumas de Face com Restrição em Amplitude Mandibular, com duração de 6 semanas e após a finalização do programa, os pacientes foram reavaliados. Como avaliação complementar, foi realizada a avaliação da musculatura mastigatória pela eletromiografia de superfície em 38% dos participantes dos grupos de pesquisa e em 25 participantes do grupo controle. Resultados: Os resultados indicaram que independentemente do tempo transcorrido entre a correção das fraturas e a intervenção fonoaudiológica, todos os grupos de pesquisa apresentaram melhora significativa em: postura, mobilidade e funções miofuncionais orofaciais, mobilidade mandibular e nos sinais e sintomas de DTM. Em relação à análise eletromiográfica, verificou-se melhora da fisiologia e do desempenho muscular de indivíduos com traumas de face após reabilitação fonoaudiológica. Conclusão: O programa de reabilitação adotado neste estudo mostrou-se eficaz em sua proposta. As diferenças observadas entre os grupos nas avaliações pré-tratamento foram minimizadas após aplicação do programa terapêutico fonoaudiológicoPurpose: to verify the effectiveness of a speech-language rehabilitation program for adult patients with facial trauma and mandibular mobility restriction. For this purpose we inclued a clinical assessment of orofacial motricity and mandibular mobility, a patient self-perception protocol and an eletromyographic evaluation of the masticatory muscles. Methods: Patients included in the study presented midface and/or lower face fractures and were divided in groups according to the time between fracture reduction and the oral motor clinical and self-perception assessments: 0-1 months (Group 1 - 24 participants), 1-3 months (Group 2 - 29 participants) and >3 months (Group 3 - 12 participants). All participants underwent an evaluation that consisted of a clinical protocol for the evaluation of orofacial motricity, mandibular range of movements, and a self-perception protocol of temporomandibular disorders signs. The adopted oral motor therapy program for facial trauma and restricted mandibular range of movements consisted of six weekly 40 minutes sessions. After the treatment program ended, patients were reassessed using the same protocols. As a complementary evaluation, the masticatory muscles were evaluated using surface electromyography (sEMG). Results: The results indicated that regardless of the time elapsed between the facial fracture reduction and the oral motor treatment program, all groups presented a significant improvement in the posture and mobility of the orofacial organs, in the orofacial myofunctional functions, in the mandibular range of movements and a reduction of the signs related to temporomandibular disorders. In addition, we observed an improvement in the physiology and muscle performance of patients. Conclusion: The adopted treatment program demonstrated to be effective in its purpose. The differences observed between the groups prior to treatment were minimized after the program was conclude

    Correlation between the pressure and electromyographic activity of the tongue during swallowing in individuals with different orofacial myofunctional disorders

    Introdução: a literatura indica que a língua é um importante órgão do sistema orofacial responsável por tarefas sensoriais e motoras como mastigação, deglutição, sucção, respiração e a articulação de sons da fala. Objetivos: correlacionar os achados de pressão de língua e da eletromiografia de superfície (EMGs) da deglutição de grupos de pacientes com diferentes alterações orofaciais. Método: participaram do estudo 44 pacientes (20 homens e 24 mulheres com idades entre 17 e 63 anos), com diferentes alterações miofuncionais orofaciais. Os participantes do estudo foram avaliados por meio da Avaliação Miofuncional Orofacial com Escores Expandido (AMIOFE-E) e Avaliação da amplitude mandibular. Além disso, foram realizadas avaliações complementares por meio da mensuração da pressão de língua (ponta e dorso) e exame de Eletromiografia de Superfície (EMGs) em região supra hioidea na tarefa de deglutição de saliva em diferentes volumes de água. Resultados: a análise estatística encontrou algumas correlações fracas entre o valor de pressão do dorso de língua e as seguintes variáveis: medida do terço inferior da face; trespasse vertical e horizontal; postura em repouso; funções orofaciais e pressão da ponta da língua. Os resultados sugerem que quanto maior for a medida do terço inferior, menor será a pressão do dorso da língua; quanto maior for a medida dos trespasses (vertical e horizontal) maior será a pressão do dorso da língua; quanto maior for a pontuação da avaliação de postura e funções orofaciais, maior será a pressão do dorso de língua e quanto maior for a pressão do dorso de língua, maior será a pressão da ponta da língua. Conclusão: os resultados do presente estudo sugerem que as alterações miofuncionais orofaciais encontradas nos diferentes grupos de pacientes estão mais relacionadas às discrepâncias maxilomandibulares do que às patologias pesquisadas no presente estudoIntroduction: the literature indicates that the tongue is an important organ of the orofacial system responsible for sensory and motor tasks such as chewing, swallowing, sucking, breathing, and the articulation of speech sounds. Purpose: to correlate findings of tongue pressure and surface electromyography (sEMG) of swallowing from groups of patients with different orofacial myofunctional alterations. Methods: a total of 44 patients (20 male and 24 female, aged 17 to 63 years) with different orofacial myofunctional alterations participated in the study. All participants were clinically assessed using the Myofunctional Orofacial Assessment with Expanded Scores (OMESE) and had their mandibular range of movements verified. In addition, further evaluations were performed by measuring tongue pressure (tip and dorsum) and the electric activity of the muscles in the suprahyoid region, using surface electromyography (sEMG), during the swallowing of saliva and different volumes of water. Results: the statistical analysis found some weak correlations between the tongue dorsum pressure value and the following variables: measurement of the lower third of the face; vertical and horizontal overlap; resting posture; orofacial functions and pressure of the tip of the tongue. The results suggest that the greater the measurement of the lower third of the face, the lower the pressure of the tongue dorsum; the greater the measurement of the overlaps (vertical and horizontal), the higher the pressure of the tongue dorsum; the higher the score obtained on the evaluation of orofacial posture and orofacial functions, the higher the pressure of the tongue dorsum; and the higher the pressure of the tongue dorsum, the higher the pressure of the tongue tip. Conclusion: the results of the present study suggest that the orofacial myofunctional alterations found in the different groups of patients are more related to the observed maxillomandibular discrepancies than to the pathologies investigated in this stud

    Oral-motor and electromyographic caracterization of patients submitted to open and closed reductions for facial fractures

    Objetivo: realizar a caracterização do sistema miofuncional orofacial de indivíduos adultos com fraturas de face, que foram submetidos à procedimentos cirúrgicos distintos para a correção das fraturas (redução aberta e fechada). Métodos: 57 participantes divididos em três grupos: G1- composto por 19 indivíduos submetidos a redução aberta de pelo menos uma das fraturas faciais; G2- composto por 19 indivíduos submetidos a redução fechada da fratura; GC - 19 indivíduos voluntários saudáveis, sem alterações no sistema miofuncional orofacial. Todos os participantes foram submetidos à avaliação que consistiu na aplicação de um protocolo clínico para a avaliação da motricidade orofacial, a amplitude dos movimentos mandibulares e a avaliação da musculatura mastigatória por meio da eletromiografia de superfície (EMGs). Resultados: os resultados indicaram que para avaliação clínica da motricidade orofacial, ambos os grupos com fratura de face se diferenciaram do grupo controle, apresentando prejuízo na mobilidade dos órgãos fonoarticulatórios e nas funções de mastigação e deglutição. Os grupos com fratura de face não se diferenciaram neste item da avaliação. Quanto às medidas de amplitude mandibular, os grupos com fratura de face também se diferenciaram do grupo controle, apresentando maior restrição da mobilidade mandibular. Nesta etapa da avaliação, o grupo submetido à redução fechada da fratura apresentou maior amplitude de movimentos, se comparado ao grupo submetido à redução aberta da mesma, principalmente para a medida de abertura oral máxima. Na avaliação eletromiográfica dos músculos mastigatórios, o grupo submetido à redução aberta da fratura apresentou menor ativação muscular se comparado aos demais grupos. Ambos os grupos com fraturas de face se diferenciaram significantemente do grupo controle quando considerado o índice de assimetria do músculo masseter, apresentando funcionamento muscular mais assimétrico na tarefa de máxima intercuspidação dentária. Conclusão: Os pacientes com trauma de face apresentam alterações significativas em postura, mobilidade, funções orofaciais, amplitude mandibular e na atividade eletromiográfica. O tipo de tratamento médico não influenciou os resultados da funcionalidade muscular no período de até seis meses após a correção da fraturaPurpose: to characterize the oral-motor sytem of adults with facial factures, comparing the performance of individuals submitted to open and closed surgical treatment. Methods: 57 adults divided into three groups: G1 - 19 individuals submitted to open surgery for the correction of at least one of the facial fractures; G2 - 19 individuals submitted to closed surgery for the correction of facial fractures; GC - 19 voluntary healthy individuals, with no alterations of the orofacial myofunctional system. All participants underwent the same assessment: clinical assessment of the orofacial myofunctional system; evaluation of the mandibular range of movements; and surface electromyography (sEMG) of the masticatory muscles. Results: the results indicated that both groups with facial fractures presented significant differences when compared to the control group, in terms of the mobility of the oral-motor organs, mastication and swallowing. However, the groups with facial fractures did not differ in performance. Regarding the measurements obtained for the mandibular movements, both groups with facial fractures presented significant differences when compared to the control group, indicating greater restrictions in mandibular movements. The group submitted to closed surgery presented better range of movements when compared to the group submitted to open surgery, especially when considering the measurement of maximal mandibular opening. When analyzing the results of the sEMG, the group submitted to open surgery presented lower muscle activation when compared to the other two groups. Both groups with facial fractures differed significantly from the control group when considering the index of asymmetry for the masseter muscle, presenting a more asymmetrical muscle function during the task of maximum voluntary teeth clenching. Conclusion: Patients with facial trauma present significant deficits in posture, mobility and function of the oral motor system. The type of medical treatment did not have an influence on the results of muscle function during the first six months after fracture reductio

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     Characterization of swallowing in patients with myotonic dystrophy of Steinert

    INTRODUÇÃO: A disfagia orofaríngea e os distúrbios de motilidade esofágica são considerados as mais importantes causas de pneumonia aspirativa em pacientes com distrofia miotônica. O objetivo deste estudo foi avaliar as características clínicas da motricidade orofacial e a deglutição de indivíduos com distrofia miotônica (DM1), utilizando um protocolo clínico padronizado e eletromiografia de superfície (EMGs). MÉTODO: Os participantes foram divididos em dois grupos: G1-composto por 20 adultos com DM1; G2-composto por 20 voluntários saudáveis, os participantes foram pareados por idade e gênero com G1 para a análise estatística. Foi realizada a avaliação das estruturas e funções orofaciais, utilizando um protocolo clínico padronizado, e mensurada a atividade mioelétrica da deglutição por meio da EMGs, com eletrodos localizados em quatro grupos musculares: (1) orbicular da boca, (2) masseter, (3) musculatura suprahioidea e (4) extrínseca da laringe. A atividade mioelétrica foi medida durante o repouso muscular e durante a deglutição de saliva e de 16,5ml e 20ml de água. Os traçados da EMGs foram avaliados durante o inicio (onset), pico e o término (offset), dos evento da deglutição. A análise estatística incluiu a ANOVA de duplo fator para intragrupos e intergrupos e o teste de Bonferroni para correções de comparações múltiplas. RESULTADOS: Pacientes com DM1 apresentaram déficits em posição, postura e mobilidade dos órgãos miofuncionais orofaciais, e nas funções de mastigação e deglutição. Além disso, os resultados da EMGs para diferentes tarefas de deglutição indicaram maior atividade muscular do orbicular da boca e maior duração da ativação muscular para os músculos: orbicular da boca, masseter e extrínseco de laringe. Não foi observado aumento significativo na amplitude da EMGs, nos pacientes com DM1, quando comparado aos resultados obtidos no teste de deglutição normal com o teste de estresse. CONCLUSÕES: A maior duração da deglutição na EMGs no grupo DM1, possivelmente está relacionada a miotonia e/ou incoordenação dos músculos envolvidos no processo da deglutição ou pode estar relacionado a uma adaptação fisiológica para uma deglutição segura. A identificação precoce dos distúrbios da deglutição permite reabilitação precoce oral, o que poderia diminuir o risco de pneumonia por aspiração nesta população.INTRODUCTION: Oropharyngeal dysphagia and oesophageal motility disorders were found to be the most important reasons causing aspiration pneumonia in patients with myotonic dystrophy. The purpose of this report was to evaluate clinical characteristics of the oral motor movements and swallowing of individuals with myotonic dystrophy type 1 (DM1), using a standardized clinical protocol and surface electromyography (sEMG). METHOD: Participants were 40 individuals divided in two groups: G1- composed by 20 adults with DM1; G2- composed by 20 healthy volunteers paired by age and gender to individuals in G1. Participants of all groups underwent clinical assessment of the orofacial structures and functions using a standardized clinical protocol. The myoelectric activity of swallowing was measured using sEMG. Four muscle groups were examined: (1) the orbicularis oris superior and inferior; (2) the masseter; (3) the submental muscle group; and (4) the laryngeal strap muscles. Muscle activity was measured during rest, during dry swallows and during the swallowing of 16.5ml and 20ml of water. Surface EMG traces were evaluated for onset, peak and offset of activity during swallow events. The statistical analysis included the one-way ANOVA with two factors for within and between group comparisons and the Bonferroni correction for multiple comparisons. RESULTS: Patients with DM1 presented deficits in posture, position and mobility of the oral motor organs, as well as compromised mastication and deglutition. Moreover, sEMG results for different swallowing tasks indicated higher muscle activity for the orbicularis oris and longer durations of muscle activation for the orbicularis oris, masseter and laryngeal strap muscles. When considering within group comparisons, DM1 patients did not present a significant increase of sEMG amplitude during the stress test in comparison with the normal swallow test. CONCLUSION: Compared to healthy individuals, patients with DM1 presented longer times to pass a bolus from the oral cavity to the esophagus. The larger duration of sEMG in the DM1 group is possibly related to myotonia and/or incoordination of the muscles involved in the swallowing process or could reflect a physiological adaptation for safe swallowing. Early identification of swallowing disorders enables early oral rehabilitation, which in turn could decrease the risk of aspiration pneumonia in this population

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
