Constrained gradient flows for Willmore-type functionals

    No full text
This cumulative thesis discusses various aspects of constrained gradient flows of higher order. The main focus is the analysis of constrained Willmore-type flows of curves and surfaces and especially their asymptotic behavior. These flows yield quasilinear nonlocal geometric evolution equations of fourth order, making them challenging from the perspective of partial differential equations. Chapter 1 provides a brief review on the relevant geometric and analytic concepts needed for the following chapters 2 to 7, each of which contains one research article, Articles A to F. Chapter 2. Article A: F. Rupp. On the Lojasiewicz-Simon gradient inequality on submanifolds. J. Funct. Anal. 279 (8):108708, 2020. 33 pages. We prove a suitable version of the Lojasiewicz–Simon gradient inequality, a fundamental functional analytic tool to study general gradient flows with constraints, which will be essential for the subsequent asymptotic analysis of concrete geometric evolution problems. Chapter 3. Article B: F. Rupp and A. Spener. Existence and convergence of the length-preserving elastic flow of clamped curves, 2020. Preprint. 49 pages. We discuss the length-preserving elastic flow of open curves with clamped boundary conditions and prove existence, parabolic smoothing and convergence for initial data lying merely in the energy space. Chapter 4. Article C: F. Rupp. The volume-preserving Willmore flow, 2020. Preprint. 46 pages. We consider a constrained version of the Willmore flow of immersed closed surfaces which preserves the enclosed volume. For spherical initial data with nonzero volume and Willmore energy below 8π, we show global existence and convergence. Chapter 5. Article D: F. Rupp. The Willmore flow with prescribed isoperimetric ratio, 2021. Preprint. 39 pages. We study the Willmore flow with a constraint on the isoperimetric ratio. This flow describes a dynamical approach to the Canham–Helfrich model for lipid bilayers with zero spontaneous curvature. Under suitable assumptions on the topology and the initial energy, we can show that the flow exists globally and converges to an equilibrium. Chapter 6. Article E: M. Müller and F. Rupp. A Li–Yau inequality for the 1-dimensional Willmore energy, 2021. To appear in Adv. Calc. Var. 33 pages. We study the relation between self-intersections of planar curves and Euler’s elastic energy and show embeddedness along the elastic flow below a certain energy threshold. Chapter 7. Article F: T. Miura, M. Müller and F. Rupp. Optimal thresholds for preserving embeddedness of elastic flows, 2021. Preprint. 39 pages. We extend the previous result in Article E by finding optimal energy thresholds below which any initially embedded curve will remain embedded under the elastic flow. Artikel A: F. Rupp. On the Lojasiewicz-Simon gradient inequality on submanifolds. J. Funct. Anal. 279 (8):108708, 2020. 33 Seiten. Kapitel 3. Artikel B: F. Rupp and A. Spener. Existence and convergence of the length-preserving elastic flow of clamped curves, 2020. Preprint. 49 Seiten. Kapitel 4. Artikel C: F. Rupp. The volume-preserving Willmore flow, 2020. Preprint. 46 Seiten. Kapitel 5. Artikel D: F. Rupp. The Willmore flow with prescribed isoperimetric ratio, 2021. Preprint. 39 Seiten. Kapitel 6. Artikel E: M. Müller and F. Rupp. A Li–Yau inequality for the 1- dimensional Willmore energy, 2021. Zur Veröffentlichung akzeptiert in Adv. Calc. Kapitel 7. Artikel F: T. Miura, M. Müller and F. Rupp. Optimal thresholds for preserving embeddedness of elastic flows, 2021. Preprint. 39 Seiten.

    No full text

This doctoral thesis will explore the use of different high-resolution mass spectrometry techniques to identify and quantify disease biomarkers and metabolic pathway biomarkers in pediatric patients. Mass spectrometry is an essential tool in translational medicine, enabling precise and detailed analysis of complex biological compounds, such as proteins, lipids, and metabolites, which can serve as disease indicators or key elements in physiological and pathological responses. Specifically, various concrete applications of these techniques will be discussed, focusing on relevant pediatric clinical issues. The first application will involve the study of lung inflammation in children with congenital heart disease undergoing corrective surgery with cardiopulmonary bypass. In this context, mass spectrometry will be employed to identify specific inflammation biomarkers and to track the changes induced by extracorporeal circulation on lung surfactant composition. The second application will focus on the use of mass spectrometry with isotopic enrichment techniques to study the placental transfer of DHA (docosahexaenoic acid). The last application of mass spectrometry will involve the use of untargeted metabolomics for the analysis of plasma and urine from children with congenital heart disease undergoing corrective surgery with extracorporeal circulation. This will allow us to identify altered metabolic pathways in acute brain injury, improve patient management during and after surgery, and prevent long-term complications. The overall goal of this study is to apply high-resolution mass spectrometry techniques to measure and characterize different disease biomarkers in pediatric populations, and to identify different phenotypes within the same pathology. This approach will move towards increasingly personalized medicine, where therapies can be tailored to the specific biological characteristics of each patient, thereby improving treatment effectiveness and reducing the risk of complications

IS IT POSSIBLE TO IMPROVE EMPATHIC SKILLS IN NURSING STUDENTS DURING ACADEMIC TRAINING COURSES? A LONGITUDINAL STUDY

    No full text
Empathy is accepted as a critical component of supportive relationships. Many scholars have argued that empathy provides health professionals with the capacity to improve the health of patients, so it should ideally be taught to make health professionals more responsive to patient needs (Hojat, Vergare, Maxwell et al. 2009; Ward, Schaar, Sullivan et al. 2009). For this reason many authors and professional organizations in the field of healthcare would like empathic skills to taken more into consideration (Hojat, Gonnella & Maxwell, 2009). Our study initiated from two beliefs: a) individual empathy skills are amenable to change and b) it is possible to enhance empathy during education in healthcare professions (Ozcan, Oflaz & Sutcu, 2010). Starting from these assumptions, we conducted a cohort longitudinal study, which aimed to determine whether specific training courses would enhance empathic skills in student nurses during a 3-year degree course (2007-2010). Data were collected using the Italian versions of the Balanced Emotional Empathy Scale (BEES) and the Jefferson Physician Empathy Scale (JPSE adapted for nursing students). Measurements of levels of emotional empathy (BEES) and levels of empathy within the student-patient relationships (JPSE) were taken and repeated at regular intervals during the academic course (at the beginning, T0; during the course, T1 and T2; at the end of the course, T3). 148 participants (77.6% women) completed the study and were divided into 3 groups: 1) "Intervention A" Group (N=62); 2) "Intervention B" Group (N=45); 3) Control Group (no specific empathy training course; N=41). The results showed that the specific training courses were effective, especially for emotional empathy in women (BEES T0=31,59 ; BEES T3=40,22; p < .001). We also found a significant difference between the Control Group and the two other groups (p < .05). On the contrary, levels of empathy in student/patient relationships fluctuated during the three years of assessment (JPSE T0=122,32; JPSE T1=117,78; JPSE T2=123,00; JPSE T3=120,31) but there were no significant differences between groups and between T0 and T3. To sum up, the specific training courses that were added to the standard academic course seem to increase emotional empathy (Cunico, Sartori, Maragnolli et al. 2012) but this does not apply to the student/patient relationship, which may require different training in terms of duration and context. The results will be discussed taking into account the amount and timing of the students’ clinical experience and the role of some important aspects such as cognitive mediation and emotion regulation, which are critical in order to create an empathic and supportive relationship with patients
