In the process if the movement entered a narrow shipping channel in the river, it takes a special skill and attention more than sailing on the high seas. Even in the IMO's rules Colreg’s give special attention to Rule 9 of the narrow shipping channel. Besides the issue of groove width and depth of the river, busy traffic can also cause danger to the ship, such as the danger of collision and ran aground. Also in manouvering at musi river , we must know everything that must be considered in manouvering at musi river and how the risk assessment process if the manouver at musi river. In writing this essay, the author describes the theory of the implementation of the navigation entry Musi river channel in a research report and as a foundation for solving the existing problems. The method used in this study is a qualitative method that produces descriptive data in the form of written words of the object under study. In this case the approach to collect data in the form of an object through interviews with experienced resources, metode through literature, and the data associated with the entry process if the motion Musi River channel on MT. SULTAN MAHMUD BADARUDDIN II. Based on the results of research by the author during a practice display at MT. SULTAN MAHMUD BADARUDDIN II, there is a risk of the occurrence of such a problem finding aground, collision, and other hazards on the ship motion exercise in Musi river channel. To the authors wanted to find the cause of such a thing happened. To the authors will identify the cause of it happening by using SHEL, this method consists of Software, Hardware, Environment, and Live ware. Software is part of a non-physical systems, including procedures and rules, Hardware is the equipment used includes, Environment is the place where the activity takes place, and Live ware is referring to every human component of the system in a relational aspects, management, control and communication. Having identified the causes of risk when ship motion exercise in Musi River channel, can we then do a safety assessment to look for things that have a risk that cannot be tolerated during the navigation process. With this assessment's salvation, then each crew will be more careful in every action taken and make every decision in motion exercise safely, effectively, and eficient. By knowing the things that cannot be tolerated, it can be done mitagasi risk, which is to reduce or even completely eliminate the risk that may occur during the process if the motion. In the process of entering a narrow or river cruise line, it takes a special skill and more attention than sailing in the open seas. In addition to the problem of the width of the groove and the depth of the river, the busy traffic can also cause danger to vessel, such as the danger of collision and aground. In writing this essay, the authors describe the implementation of the movement of the Musi River flow in which the implementation there are some problems regarding the implementation of the ship's motion and the responsibility of the Master in motion acting. By solving the problem it is expected that the incident will not be repeated and the ship can work the motion safely, effectively, and efficiently.     Giant Lipoma of Unusual Regions: A Case Series of Two Unfortunate Adults

    Lipomas are prevalent, benign, asymptomatic mesenchymal tumors that can develop in various areas where adipose tissue is present. However, when lipomas reach sizes larger than 10 cm in width or exceed 1 kilogram in weight, they are classified as giant lipomas [1-3]. These oversized lipomas can lead to functional restrictions, aesthetic concerns, and compression-related injuries, all of which can negatively impact an individual’s quality of life. They are usually surgically removed to avoid injury to major neurovascular structures; however, they have a predisposition for recurrence and the risk of malignancy following surgery. This case series discusses the clinical findings and surgical treatment of two patients who had enormous, deformable, fatty tumors of buttocks and thigh, respectively

    Outcome of Intraarticular Communicated Fractures Distal Radius Treatment with Orthofix External Fixators

    Objective: Assess the outcome of intraticular comminuted fracture distal radius treated with orthofix external fixator. Methods: Prospective study with inclusion criteria of Comminuted intraarticular fractures distal radius , 20 to 60 years age, close and open fractures and exclusion criteria of patient above 60 years , extraarticular fracture , volar and dorsal barton fractures , more then 10 days old fracture. Results: From Januarry 2012 to sept 2014 , 63 patient with intraarticular comminuted fracture distal radius treated with orthofix external fixator included in this study. Functional outcome was assessed on Gartland and Wesley scoring system. 40 (63%)male and 23(37%) female patients, 35(56%) road traffic accident and 28(44%) have fall. AO C1, were 32(51%), C2 ,19(30%), C3, 12(19%) patients. Additional kwire fixation applied in 54 patients.Fixator removed at 6 weeks. Functional result were excellent in 26 patient(41%) , good 32(51%) , satisfactory in 4 patient(6%) , poor in 1(2%). Complication observed were , minor and major pin track infection 30(65%), tethering of external indicis tendon 3(7%) patient, k wire loosening 10(22%) and sudecks atrophy 3(7%) patients. Conclusion: Orthofix external fixator application in intraarticular comminuted fracture distal radius is recommended because it is minimal invasive technique ,have excellent to good outcome . The main engine has a very important role to support the smoothness of the voyage of the ship, one of the main support in the parent machine is a Bosch pump, which serves to push the fuel into the injector for subsequent injection injector fuel in to the combustion chamber, in this study the authors will discuss about Bosch pump interference to injector performance. The research method used is qualitative, in determining the priority of the problem to be resolved, the research using an approach tool that is SWOT method by giving the value 1-4. Sources of data that the authors use is the primary and secondary data sources, while the method of withdrawal of conclusions that the author uses is by comparing the actual situation with the results of the author's thorough theory. From the results of research conducted problems that occur is the occurrence of interference on the Bosch pump which causes injector performance becomes not maximal and this is due to blister plunger on the bosch pump that resulted in plunger and barrel is not precise. The impact of blister plunger on the bosch pump results in a decrease in plunger plaque on the barrel so that the resulting fuel pressure becomes reduced. Strategies on the problem are the maintenance and repair of the bosch pump, optimizing the fuel system to keep the fuel in a clean and periodic maintenance to the bosch pump and other machinery so that the engine and its components reach at least the economic life Keywords: Bosch pump, Injector, Main Engine

    The rapid development of Science and technology has resulted in electrical energy becoming a very important thing for humans. What would it be like if we lived without electricity, the use of electrical energy that is already very attached and touches many aspects of human life. Field Work Practice (On The Job Training) is carried out at the Airport that has been determined by the Medan Aviation Polytechnic Education Training. On The Job Training (OJT) is one of the curriculums that must be implemented by Cadets as a form of measuring the level of cadets' abilities in direct work practice and adding knowledge that has not been obtained from campus during the OJT period. After implementing On the Job Training (OJT) for approximately 6 months at Sultan Mahmud Badaruddin II Airport, Palembang, several conclusions can be drawn, including: The performance of the Aviation Electrical Unit which carries out its duties and responsibilities well. Can understand and gain experience and how the conditions or situations are in the world of work. Can develop the abilities possessed from the competencies obtained from the Airport.     Proximal Fibular Osteotomy – Simplest and Most Effective Mean to Delay Total Knee Arthroplasty

    Background: Osteoarthritis involves degeneration of articular cartilage seen with increasing age. The knee joint, the most common joint that falls prey to attacks of osteoarthritis, has 30% incidence in population above 60 years. Total knee arthroplasty (TKA) is the main surgical option for orthopaedics. Though it corrects the deformity and relieves pain, yet it is not the treatment of choice in younger population. This study aimed to validate the effects of proximal fibular osteotomy (PFO) as a newer technique in managing medial compartment knee osteoarthritis. Methods: The diagnosed patients for medial compartment knee joint osteoarthritis were selected for study from Dr. Ziauddin University hospital of Karachi. Excluded were aged less than 40 years, or with BMI more than 30, and patients with tri-compartmental arthritis. Medial and lateral joint spaces along with Oxford knee score were measured and recorded pre- and post-operatively. Patients underwent PFO after giving written and informed consent. Results: Total number of patients selected was 30 for this study; 21 (70%) females, and 9 (30%) males (mean age 58.8 years). Mean pre-operative measured medial joint space on standard antero-posterior (AP) radiograph was 0.442± 0.04 cm. Mean recorded pre-operative Oxford knee score was 23.87±3.74 mm. Improvement was observed in mean postoperative medial joint space to 0.572± 0 .066 cm and mean post-operative Oxford knee score to 40.2±5.8mm. Conclusion: This study concludes that PFO significantly improves joint function in patients with medial compartment osteoarthritis knee and may delay the need for total knee arthroplasty, if carried out at an appropriate stage. SMP Pahlawan Nasional adalah merupakan salah satu sekolah swasta di Kota Medan yang juga melaksanakan Program Bantuan Operasional Sekolah beralamat di Jl. Durung No. 205 Medan Kelurahan Sidorejo Kecamatan Medan Tembung Kota Medan. SMP Pahlawan Nasional dalam melaksanakan pengelolaan sekolah didasarkan pada program kerja yang dituangkan dalam rapat kerja tahunan atau musyawarah dengan melibatkan semua komponen sekolah seperti pihak Kepala sekolah, guruguru/ staf, orangtua siswa, komite sekolah dan pihak-pihak lainnya yang terkait seperti pengawas dan Kantor Dinas Pendidikan Kota Medan. Berdasarkan pentingnya pelaksanaan program BOS dalam membantu pelaksanaan program kerja di SMP Pahlawan Nasional tersebut, maka judul yang penulis angkat pada penelitian ini adalah Evaluasi Kebijakan Program Bantuan Operasional Sekolah Tahun 2015 di SMP Pahlawan Nasional di Kota Medan. Durung No. 205 Village Medan Sidorejo Tembung district of Medan Kota Medan. Junior Pahlawan Nasional in managing the school is based on the work program as outlined in a working meeting annual or consultations involving all components of the school as the school principal, teachers / staff, parents, school committee and other parties related to such supervisory and Education office of Medan. Based on the importance of the implementation of the BOS program in assisting the implementation of the work program at the Junior Pahlawan Nasional, the title which the author adopted in this study is the evaluation of the School Operational Assistance Program Policy Year 2015 Junior Pahlawan Nasional in the city of Medan.     Aneurysmal bone cyst of ischiopubic ramus: A rare location with literature review

    Aneurysmal bone cyst (ABC) is a locally destructive lesion of the bone rather than a true neoplasm. The pelvis is not an unusual site for an ABC; approximately 12% of cases occur in this location. We present a case of aneurysmal bone cyst (ABC) in ischiopubic ramus in a 22 years old male who presented with right inguinal swelling and pain. He was evaluated with X-ray and Magnetic Resonance Imaging (MRI) of pelvis. A review of literature regarding this rare site of ABC with radiological features is also described in this case report.ABC of ischiopubic ramus is an uncommon entity hence diagnosis of such a case requires proper clinical, radiological and histopathological evaluation to rule out other differential diagnoses of an expansile, osteolytic lesion. This will help in selecting a proper treatment plan
