Athlete´s shoulder most frequent lesions are anterior shoulder instability and superior labrum anterior and posterior lesion ( SLAP), altering structures and functions of the shoulder joint, leading to impairments in sports activity. The aim of this thesis was to evaluate functional and supraspinatus morphological adaptations of shoulder in athletes with anterior instability and SLAP lesion. Three studies were developed. The first study investigated functional differences regarding clinical complaints and the scoring systems of American Shoulder and Elbow Surgeons Standardized Shoulder Assessment Form (ASES) and the Athletic Shoulder Outcome Rating Scale (ASORS) in 249 athletes (153 with instability and 96 with SLAP lesion). The groups presented functional differences related mainly to sports activitiy and, even with more complaints of shoulder pain, SLAP lesion athlete´s tolerate more training hours compared to instability athletes. The second study evaluated isokinetic strenght and muscular control of shoulder rotators in four groups: instability group (n=10), instability control group (n=10), SLAP group (n=10) and SLAP control group (n=10). The torque steadiness and rotator cuff strength in 90 and 180°/s were evaluated. Athlete´s with anterior stability presented shoulder rotation weakness, while athletes with SLAP lesion showed higher torque fluctuation of shoulder internal rotation. The third study characterized the different fiber types of supraspinatus and shoulder function in those lesions. This study performed muscles biopsies in 10 athletes undergoing to arthroscopy repair of shoulder anterior instability and SLAP lesion. The volunteers were functionally evaluated with ASES and ASORS, and muscle samples were processed with histochemical reaction for myosin adenosine triphosphatase (mATPase). Muscles fibers were then characterized in type I, IIa, IIb and hybrid, and percentual and lesser diameter of each fiber type were calculated. In this study, volunteers presented evident functional limitations in sports activity and hypertrophy of all typees of muscles fibers. Most of the sample also presented a great number of type IIa and IIb in the supraspinatus muscle. In conjuction, those studies proves different functional and supraspinatus morphologic adaptations in athletes with anterior shoulder instability and SLAP lesion, justifying the need of different rehabilitations approaches.
Contributions of knee and trunk isokinetic muscular strength to bone mass acquisition in female soccer players
Esses resultados de correlação positiva entre a força e a quantidade mineral óssea de sítios específicos provêem evidências adicionais de um papel da força muscular na aquisição da massa óssea regional.OBJECTIVE: to analyze the contribution of isokinetic muscular strength of dominant knee and trunk flexors and extensors in the acquisition of bone mass in the correspondent regions that these forces are applied in female soccer players. METHODS: twenty-two female soccer athletes (GAF) were compared with 20 matched-controls (GC). The bone mineral content (BMC) and bone mineral density (BMD) were evaluated in whole body, lumbar spine and dominant hip (femoral neck and total hip) with the paediatric software of Hologic QDR (Discovery model, Bedford, MA, USA). From whole body scan, the variables of body composition and bone mass of dominant leg, trunk and head were also determined. The muscular strength of flexion and extension movements in dominant knee and trunk were evaluated with the Biodex Multi-joint System 3 (Shirley, NY, USA) with 60°/s, and 5 repetitions at concentric/eccentric mode. The concentric variables peak torque, maximal repetition total work and total work of flexors and extensors were considered. Statistical analysis was performed using Student\'s t-test to compare groups and additionally Pearson\'s correlation coefficient and linear regression analysis to GAF. RESULTS: Both groups were similar regarding age, weight, height, race and calcium intake (p>0.05), however GAF presented a higher percentage of lean body mass and a lower percentage of body fat compared to GC (p<0.001). All loaded sites during exercise had significantly higher values of bone mass for GAF, and so for all isokinetic concentric strength variables of dominant knee and trunk (p<0.05 e p<0.001). Bone mass variables of GAF regions presented positive correlations with weight, body mass index, total and trunk lean mass. The BMD of any region presents significant correlations with variables of strength performance. Nevertheless, all strength variables of trunk flexors were positively correlated with BMC of whole body and trunk, so as knee dominant extensors with BMC of hip and dominant leg. CONCLUSION: muscular strength exerted a positive contribution to BMC acquisition at specifics sites. These results of positive correlation between force and bone mineral quantity of specific sites give us further evidences of the muscle strength role in the acquisition of regional bone mass
Introduction: Overhead sports overload the shoulder complex mainly due to the
repetition of throwing movements at high speeds, which may cause some adaptations in
the athlete’s shoulder, and might increase the risk of developing shoulder pain. Moreover,
it has been considered nowadays the influence of alterations throughout the kinetic chain
as risk factor to injuries in the shoulder complex. Objective: to compare alterations in
osseous, muscular and joint system and the function of shoulder complex, core and hip in
overhead athletes with and without shoulder pain. Methods: This is an observational
study, in which 41 handball and volleyball athletes were assessed. The athletes were
assigned to two groups according to their shoulder pain: no pain group (NPG) (n: 20, age:
21,2±3,1 years and BMI: 23,3±3,2 kg/m2
) and pain group (PG) (n: 21; age: 23,6±3,7 years
and BMI: 25,02±3,9 kg/m2
), based on pain and function self-reported through the Penn
Shoulder Score (PSS) and Disabilities of the arm, shoulder, and hand (DASH)
questionnaires. The individuals underwent an assessment of range of motion (ROM) of
shoulder (rotation, horizontal adduction, low flexion, and humeral retroversion), hip
(rotation) and trunk (flexion, extension, and lateral flexion), strength of glenohumeral
(rotators), scapular (lower trapezius and serratus anterior), hip (abductors and extensors)
and trunk (rotators, extensors and lateral flexors) muscles, besides endurance of the trunk
(flexors, extensors and lateral flexors) and functional performance of lower and upper
limbs through the Y balance test. Data were analyzed in a descriptive and inferential
manner through SPSS 20.0, with a 95% confidence interval and a p<0,05 was considered
statistically different. Results: Athletes from the pain group showed smaller external
rotation (ER) (p<0,01) and total rotation (p<0,01) ROM in the dominant shoulder, smaller
horizontal adduction (p<0,01) in non-dominant shoulder, smaller hip internal rotation
(IR) in dominant and non-dominant hips (p<0,01), and less trunk flexion ROM (p<0,01).
PG also showed less strength of trunk extension (p=0,01), short endurance time for trunk
extensors (p=0,02) and flexors (p<0,01), decreased reach distance for both lower limbs
in the anterior direction (p<0,05), decreased reach distance in the posteromedial
direction(p=0,04) for the dominant lower limb, as well as smaller composite score for
both limbs (p<0,05). Moreover, for limbs comparison, the dominant limb of the PG
showed smaller ROMs of shoulder IR(p<0,01) and total ROM (p<0,01), of horizontal
adduction (p=0,04) and low flexion(p=0,01), besides decreased reach distance in the
superolateral direction in the upper quarter Y balance test. As for the NPG the dominant limb showed higher shoulder ER ROM (p=0,03), and smaller values for shoulder IR ROM (p=0,02), horizontal adduction (p=0,01) and low flexion (p<0,01). Conclusion: Overhead athletes with shoulder pain show changes in ROM throughout the kinetic chain, besides lower core endurance, and decreased functional performance in the lower limbs.
Shoulder Score (PSS) e Disabilities of the arm, shoulder, and hand (DASH).
(rotações, adução horizontal, low flexion e retroversão umeral), quadril (rotações) e do
Resultados: Os atletas do
The throwing movement imposes an excessive stress on the glenohumeral joint,
since during these movements a very high angular velocity is generated, causing this
joint to suffer with great traction forces. Repeated performance of such movements
generates some bone, joint capsule and muscle adaptations that culminate in the
development of the posterior stiffness of the shoulder and the Glenoumeral Internal
Rotation Deficit, known as GIRD. As a consequence of these deficits, there is an
association of a decreased range of motion (ROM), imbalance of muscle strength
between medial and lateral rotators and shoulder proprioception, which increases the
risk of osteomyoarticular lesions. The objective of this study was to analyze the
biomechanical adaptations and the neuromuscular performance of the shoulder
complex of athletes with and without GIRD. It was an observational cross-sectional
study of 61 athletes practicing handball and beach handball (34 males and 27
females, 27.03 ± 5.99 years, BMI 25.58 ± 3.84) and 23 non-athletes (11 men and 12
women, 26.26 ± 4.32 years, BMI 23.61 ± 2.78). There were evaluated: ROM for
medial (MR) and lateral (LR) rotations and horizontal shoulder adduction (HA);
sense of joint position of shoulder during ROM of LR, both using an Accumar®
digital inclinometer; and isometric ROM of LR and MR strength using a Lafayette®
manual dynamometer for both groups (athletes and non-athletes). Based on the ROM
for MR evaluation, athletes with shoulder differences between limbs of less than 10º
were allocated to the group without GIRD (44 individuals) and those with a
difference greater than 10º in the group with GIRD (17 individuals). The statistical
analysis used was one-way ANOVA for all demographic variables, for comparison
between groups with GIRD and without GIRD and difference between the dominant
shoulder (DS) and the non-dominant shoulder (NDS) for the ROMs (MR, LR and
HA). Two-way ANOVA was used for comparison between groups with and without
GIRD and members (DS and NDS), for the variables ROM of MR, LR, total and
HA, sense of joint position, RM and RL strength and LR/MR force ratio. Only for
the isometric strength variables, comparisons were made between the three groups
(with and without GIRD and non-athletes). And, when necessary, the post hoc tests
of Tukey were employed. P <0.05 was considered. As a result, interaction was
verified for the ROM of MR variable (p <0.01), where the DS of the with GIRD
group presented lower ROM of MR in relation to both members (DS and NDS) of
the group without GIRD. For the ROM of LR variable, with the GIRD group had
higher ROM of LR (p = 0.03), and in both groups a lower ROM of HA was observed
in the DS (p = 0.005). For the SPA variable there was no difference between
members nor between groups. In the assessment of strength in the three groups (with
and without GIRD and non-athletes) the MR and LR strength was higher in the group
of athletes compared to non-athletes for both muscle groups (p<0.01 and p=0.01),
and the force ratio in the non-athletes group in relation to the without GIRD group
only (p<0.01). Among the members there was a higher LR strength in the DS in all
groups ((p=0.02). Despite the differences in glenohumeral rotation ROM, GIRD
athletes did not show differences in ROM of HA, sense of joint position and shoulder
rotator strength when compared to without GIRD athletes. However, regardless of
GIRD, the dominant shoulder presents greater posterior stiffness and athletes, in
atletas praticantes de handebol de quadra e/ou praia (34 homens e 27 mulheres;
27,03±5,99 anos; IMC 25,58±3,84) e 23 indivíduos não atletas (11 homens e 12
mulheres; 26,26±4,32 anos; IMC 23,61±2,78).
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Finally, we show by means of an example that our findings have a high practical relevance.information science;Pearson correlation;cosine;similarity measure;author cocitation analysis
Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more
sophisticated methods
