Aminaphtone in the treatment of pseudophakic cystoid macular edema: a case report.
Cystoid macular edema (CME) following cataract surgery has been recognized for over 50 years as an important cause of suboptimal post-operative vision. The incidence of CME varies widely, but is likely in the range of 1-2% using modern cataract extraction techniques. We report the case of resolution of post-operative CME after treatment with aminaphtone 75 mg three time a day for one month. A 74-year-old causasian woman presented with reduced vision in the left eye after one month from uneventful cataract phacoemulsification. She underwent a complete ophthalmological examination comprehensive of spectral domain optical coherence tomography (SD-OCT) which showed CME and a central foveal thickness (CFT) of 703 micron. The patient was treated with aminaphtone for one month. CME disappeared, the CFT was within normal limits when aminaphtone was ceased, and best corrected visual acuity was 20/20 at the end of the treatment. In Shakespeare’s Julius Caesar Brutus’ speech appears to be modelled upon the description of Brutus’ oratory style in the Life of Brutus by Plutarch. However, one feature lacks from the theatrical speech: Brutus’ laconic expression, conveyed by the Greek word ἀποφθεγματικός. In this article, I argue that this may be due to the translation by Thomas North, Shakespeare’s source for Plutarch’s Lives, of the word ἀποφθεγματικός in the Life of Brutus
Recovery of corneal sensitivity after collagen crosslinking with and without epithelial debridement in eyes with keratoconus
PURPOSE: To evaluate the changes in corneal sensitivity after corneal collagen crosslinking (CXL) with the epithelium off (epi-off) and with the epithelium on (epi-on) in eyes with keratoconus.
DESIGN: Prospective interventional case series.
SETTING: Eye Clinic, A. Fiorini Hospital, Terracina (Latina), Rome, Italy.
METHODS: On the basis of the corneal pachymetry at the area of topographic steepening, eyes treated with CXL were divided in 2 groups: those with a corneal thickness of 400 pm or more (epi-off) and those with a corneal thickness of less than 400 gm (epi-on). Corneal sensitivity was measured using a contact esthesiometer (Cochet-Bonnet) before treatment and 1 and 7 days and 1, 3, 6, 9, and 12 months after treatment.
RESULTS: The study evaluated 50 CXL-treated eyes in 50 patients (25 epi-on and 25 epi-off) from January 7, 2012, to December 15, 2012. In the epi-off group, corneal sensitivity was statistically significantly reduced for up to 3 months after CXL and gradually returned to normal levels. In the epi-on group, corneal sensitivity was statistically significantly reduced for up to 7 days but was not statistically significantly different from preoperative values at other measurement times. At 7 days, the corneal sensitivity was statistically significantly lower in the epi-off eyes than in the epi-on eyes.
CONCLUSIONS: Epi-off and epi-on CXL both caused hypoesthesia, but corneal sensitivity subsequently recovered completely. The corneal hypoesthesia was more pronounced in eyes in which the epi-off technique was used, and the recovery time was shorter for eyes treated using epi-on CXL
Corneal collagen cross-linking followed by phacoemulsification with IOL implantation for progressive keratoconus associated with high myopia and cataract
PURPOSE:
To evaluate the visual and topographic outcomes of a two-stage approach treatment for progressive keratoconus (KC) associated with high myopia and cataract.
METHODS:
Two patients, a 50-year-old man and a 54-year-old woman with high myopia (spherical equivalent greater than -10D), cataract, and progressive KC with poor spectacle-corrected vision and contact lens intolerance underwent a 2-stage approach treatment: first corneal collagen cross-linking (CXL) followed after at least 6 months by phacoemulsification with intraocular lens (IOL) implantation in both eyes.
RESULTS:
In both patients after a mean follow-up of 3.5 years (from 2 to 4) uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), and corneal topography pattern improved in both eyes, and the patients were satisfied with the visual improvement and happy with tolerable glasses.
CONCLUSION:
This 2-stage approach reported no complications and was effective in improving visual acuity and corneal patterns in patients with high myopia, cataract, and progressive KC
Isotretinoin, vitamin A derivates and Psychopathology: Making the point
Isotretinoin (13-cis retinoic acid, a retinoid receptor agonist) is a synthetic oral retinoid that has great efficacy against severe cystic or recalcitrant acne vulgaris. The growing number of reported cases of depression and suicide associated with isotretinoin use in patients with acne has prompted concern among dermatologists, patients, and their relatives and has triggered new warnings from regulators including depression-related, patient-informed consent forms. Despite this increasing number of reports in medical journals the association between isotretinoin and depression has received little attention in the psychiatric literature. The purpose of this critical review is to highlight this association, examine the possible link and review the clinical implications. A search was made of the MEDLINE, EMBASE, COCHRANE and PsychINFO databases using the search terms 'isotretinoin', 'depression' and 'suicide'. At the present time there is no evidence that use of isotretinoin is associated with an increased risk for depression, suicide, or other psychiatric disorders. Although the current literature does not support a causative association between isotretinoin use and depression, there are important limitations to many of the studies: the available data are insufficient and , at the present time, there is no known pharmacological mechanism that would account for psychiatric symptomatology, however, retinoid receptors are widely distributed in the brain and more research is needed to ascertain whether they have a role in depression. © 2011 by Nova Science Publishers, Inc. Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
