Introduzione: Gli ospedali sono ambienti particolarmente a rischio per l’acquisizione dell’infezione da Legionella; in tali contesti, secondo gli ultimi dati ufficiali nazionali, la frequenza di casi nosocomiali notificati è del 5,3% ma la letalità raggiunge tassi assai più elevati (28,6%) rispetto ai casi comunitari (12,6%). Aspetti strutturali e condizioni d’uso, quali tubazioni spesso compromesse, presenza di biofilm e temperatura dell’acqua calda mantenuta a valori di circa 48oC per evitare ustioni ai pazienti, sono alcune delle condizioni favorenti la replicazione del microrganismo.
Nell’ambito delle numerose misure di prevenzione per la riduzione del rischio correlato all’infezione sono comprese varie metodiche di disinfezione che, pur risultando efficaci come interventi puntuali, non garantiscono continuità di azione e, frequentemente, si assiste ad una ricolonizzazione dei circuiti idrici.
Obiettivi: In questo contesto, lo studio si prefigge di verificare l’efficacia di un nuovo metodo di prevenzione del rischio basato sulla riduzione della concentrazione di Legionelle presenti nei circuiti idrici di alcune strutture sanitarie di ricovero.
Metodi: A tal fine è stato adottato un dispositivo (Quantum Hospital Freebioenergy - QHFBE) che, operando con onde elettromagnetiche, è in grado di creare una sfera volumetrica di 60 m di raggio all’interno del quale migliora l’acqua corrente degli impianti idrici aumentandone la superficie interna e diminuendo la tensione superficiale di circa il 20%; lo strumento, attraverso una ionizzazione negativa dell’acqua, è in grado di trasformare fisicamente il calcare (CaCo3) in una forma allotropica che riduce notevolmente le nuove incrostazioni degli impianti termoidraulici ripulendo, nel giro di alcune settimane, anche le vecchie incrostazioni.
Il Servizio di Igiene Ospedaliera dell’U.O.C. di Igiene e Medicina Preventiva ha provveduto a dislocare tale dispositivo in un punto ritenuto significativo della complessa rete idrica dell’Azienda Ospedaliero-Universitaria di Sassari articolata in padiglioni; nell’ambito del raggio di azione del dispositivo sono stati identificati tre punti di prelievo e si è proceduto (attraverso una sequenza di tre campionamenti) ad una caratterizzazione dell’acqua prima dell’applicazione del dispositivo (tempo 0) e, successivamente, dopo l’inizio del trattamento sono stati eseguiti campionamenti con frequenza mensile per circa un anno (tra il 2013 e il 2014).
Le fasi del prelievo, trasporto e conservazione del campione sono state eseguite seguendo le metodiche previste dalle normative vigenti in materia (Linee guida per la prevenzione ed il controllo della Legionellosi del 2000, 2005 e del DL 81/08); la ricerca del microrganismo è stata effettuata in conformità a quanto previsto dalla norma ISO 11731/1998, che prevede le seguenti fasi: filtrazione, trattamento di decontaminazione, semina e incubazione, identificazione e determinazione della carica microbica.
Per facilitare la rilevazione e gestione delle informazioni acquisite è stata predisposta una specifica scheda di raccolta dati che riporta le variabili ambientali riscontrate durante il campionamento ed i risultati ottenuti attraverso le indagini di laboratorio effettuate.
Risultati: I risultati preliminari dell’indagine hanno permesso di ricontrare una percentuale di abbattimento di Legionella compresa tra il 37 ed il 60% dei casi; infatti, come previsto, i risultati del primo campionamento hanno evidenziato un incremento del numero di Unità Formanti Colonie di Legionella, peraltro ampiamente ridotto o addirittura azzerato nei successivi campionamenti. Si è riscontrata anche una rilevante diminuzione della carica batterica totale a 37° C (– 33,3%). In particolare i risultati ottenuti sono ottimi in due dei tre punti di prelievo predisposti dove, la carica di Legionelle, eccetto il primo campionamento, è sempre risultata inferiore alle 100 UFC/L; in un terzo punto di campionamento, invece, gravato da condizioni strutturali assai particolari e un ridottissimo flusso idrico, pur rilevando una notevole percentuale di abbattimento (compresa tra il 27,8 ed il 52,3%), permangono cariche intorno alle 4,5x103 UFC/L. L. pneumophila sierotipo 1 era presente nella maggior parte dei campioni risultati positivi, in minor misura L. pneumophila sierotipo 2-14.
Conclusioni: Nell’ambito di un più ampio processo di Water Safety Plan in corso presso l’Azienda Ospedaliera-Universitaria di Sassari, nelle more di ampliare l’osservazione ad un maggior numero di campionamenti e punti di prelievo, i risultati preliminari sono assai incoraggianti e si ritiene che, se abbinati alle classiche procedure di prevenzione (es. shock termico, iperclorazione shock, ed altri) possano addivenire a ridurre notevolmente la presenza di Legionelle ed il correlato rischio di infezione
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
