The clinical meaning of histamine skin reactivity
The definition of the “atopic state”, i.e. subjects presenting at least one skin wheal with a minimum diameter of 3
mm induced by an allergen skin-prick test (ASPT), is based on the assumption that wheal size depends entirely on
the amount of histamine produced in the antigen-antibody reaction. Several epidemiological studies have, however,
demonstrated that an ASPT-elicited wheal is heavily modulated by “histamine skin reactivity” (HSR), i.e. the size of
the wheal induced by a prick test performed with a given solution of histamine. HSR not only varies widely
depending on the individual characteristics and geographical setting, but also changes over time; these differences
in HSR markedly influence the amount of specific IgE required to produce a wheal of at least 3 mm in an ASPT. We
should therefore ideally conceive the existence of two types of” atopic patients”: one type in whom “atopy” is mainly
the result of an increased level of specific IgE antibodies, and another type in whom positive ASPTs are mainly the
result of marked skin reactivity to even small amounts of histamine. If hyper-reactivity to histamine occurs not only in
the skin but in parallel also in other parts of the organism, especially at the mucosal level, “normal” histamine
production may cause chronic or recurrent clinical symptom
[Childhood asthma].
The concept of chronicity in asthma, as emphasized by recent definitions of the disease, rests on the major characteristics of inflammatory response of the airways and progressive development of irreversible structural and functional alterations, or so-called airway remodeling. In childhood, however, such characteristics as chronicity and irreversibility are debatable. Various clinical phenotypes with variable degrees of severity of persistence are found in children. Furthermore, many patients with a history of recurrent wheezing in early infancy do not develop asthma later in life. The prevalence of asthma, especially in its mild forms, has increased markedly in recent years. Although the trend has stabilized in Italy, it continues to rise in other Western countries. Our research has shown that increased cutaneous response to histamine determines a major prevalence of positive skin tests. The rise in clinical forms of the disease accompanied by gastrointestinal symptoms is partly attributable to the dietary intake of food and beverages processed from environmentally engineered products. The features of the new forms of asthma demand accurate clinical and functional assessment. In addition to pulmonary function tests, determinations for eosinophils and inflammation markers in the blood and sputum, noninvasive methods have recently become available to assess airway inflammation. Among these, particularly useful studies include test for nitric oxide in exhaled air, along with tests for other markers of allergic inflammation and oxidative stress in the droplets of the exhaled air. Because in paediatric age, prolonged use of inhaled steroids increase the risk of growth impairment, asthma therapy should be guided by clinical criteria and examinations, rather than by rigid treatment guidelines. Moreover, to secure successful treatment, the parents and the child as well should be involved in monitoring the course of the disease
2014-2015 Tritium values in small and shallow aquifers in northern Apennines
Tritium data relating to actual rainfall in north of Italy and in particular in the northern Apennines are rare or missing. The reasons of this lack of data frequently depends on the high cost of analysis and the necessity of high amount of water to perform the analysis itself. In order to obtain these data a valid alternative can be analyze the amount of Tritium in unconfined, shallow and small aquifer not affect by human activities (such as sewage).
Recent studies, applied to the hydrogeology of the Po plain or of the Apennine slopes, highlight, in rainfall water recharging shallow aquifer, tritium values ranging between 6 T.U. and 12 T.U., higher than those detected in other and different areas of Italy or of the South Europe.
The aim of this paper is to highlight first results of tritium analyses performed on spring draining shallow aquifers in northern Apennines, characterized by the absence of human activities. The peculiarity of sampling point (spring are characterized by small and well defined catchment areas as well small differences between the infiltration/recharge elevation and the spring elevation) makes results representative of mean tritium value of rainfall recharge in the studied area.
In detail, during 2014-2015 three springs located at different elevation in Secchia Valley have been sampled and analyzed. Tritium analyses performed on a total of 5 samples highlight the following results: the maximum value
(5.0±0.7 T.U.) is detected in water collected in November whereas the minimum value ( 3.7±0.6 T.U.) is obtained in May. Therefore a mean annual value of 4.2±0.7 T.U. in the studied area have been highlighted
Why Chlamydia pneumoniae is associated with asthma and other chronic conditions? Suggestions from a survey in unselected 9 yr old schoolchildren
Despite numerous studies demonstrating an association between asthma and many other chronic conditions and signs of Chlamydia pneumoniae (Cp) infection, the role of Cp in the pathogenesis of these illness remain still unclear. We investigated the prevalence of Cp antigen in the upper airways and the prevalence of detectable Cp serum antibodies in an unselected population of 207 9-yr-old schoolchildren. We also sought the presence of asthma, chronic or recurrent respiratory symptoms by means of questionnaire completed by the parents. Nasal aspirate, blood sampling and allergen skin prick tests were also performed. None of the children had obvious signs of acute infection at physical examination. Cp DNA was detected in nasal aspirates from 20 of the 207 children tested and serum IgG antibodies for Cp in 68 children. No association was found between atopy or history of atopic illness and the presence of Cp DNA or antibody production. This finding is explained by the fact that our study was conducted in an unselected childhood population, inherently including few children with asthma. A strong association between the status of antigen carrier and the presence of detectable Cp serum immunoglobulin (Ig)G or IgM suggests that subjects with detectable Cp antibodies have an impaired ability to eliminate this pathogen when infected. Because Cp eradication requires a strong Th1 lymphocyte response, the previously proven association between Cp and asthma, might reflect the known association of asthma with Th2-oriented lymphocytic activity
