    Trabalho Final do Curso de Mestrado Integrado em Medicina, Faculdade de Medicina, Universidade de Lisboa, 2016Background/objective: Vitamin D is implicated in bone homeostasis and its deficiency is associated with a progressive bone loss and fragility fractures. Improving vitamin D status may be important to reduce fractures. Our aim was to characterize vitamin D levels in Portuguese post-menopausal woman and correlate them with prevalent osteoporotic vertebral fractures and bone mineral density. Methods: The study population was women ≥ 65years-old from EpiReumaPt (882 participants). Laboratory tests were performed on Central Diagnostic Laboratory Germano de Sousa. As there isn’t a consensus in Vitamin D levels we determined Vitamin D insufficiency as <30ng/ml and Vitamin D deficiency <10ng/ml. Prevalence estimates were computed as weighted proportions for take into account the sampling design. Results: The prevalence of osteoporosis was 15% and of vertebral fractures 16.2% on radiographs and on spine CT scan 11.7%. Hypovitaminosis D was present in 38.4% of these women, 14.2% of them with levels below <10ng/ml. Regarding the vitamin D levels distribution according the NUTS II regions, Norte, Algarve and Madeira had the lower levels and this was associated with bone mineral density and higher prevalence of fractures independently of the season. Vitamin D deficiency was independently associated with prevalent vertebral fragility fractures (OR 2.046; 95%CI 0.081,4.011; p-value 0.041). Conclusion: Levels of vitamin D are below the recommended levels for a large proportion of post-menopausal Portuguese women and this fact is associated with vertebral fragility fractures. Vitamin D supplementation recommendations should be reviewed and public health policies should address this issue.Introdução/objectivo: A vitamina D está implicada na homeostasia óssea e a sua deficiência encontra-se relacionada com uma perda progressiva de massa óssea e ocorrência de fracturas de fragilidade. Uma melhoria nos níveis de vitamina D pode ser importante na diminuição de fracturas. O objective deste estudo foi caracterizar os níveis de vitamina D em Portugal e correlacioná-los com fracturas osteoporóticas e densidade mineral óssea. Métodos: A população de estudo foi definida como mulheres ≥ 65 anos do EpiReumaPt (882 participantes). Os testes laboratoriais foram realizados no laboratório 4 Germano de Sousa. Uma vez que não existem consenso nos níveis de vitamina D, determinámos insuficiência de vitamina D como <30ng/ml e deficiência de vitamin D como <10ng/ml. As estimativas de prevalência foram calculadas como proporções ponderadas de forma a ter em conta o desenho da amostra. Resultados: A incidência de osteoporose foi de 15% e de fracturas 16.2% nas radiografias e na TC de coluna 11.7%. Hipovitaminose D estava presente em 38.4% destas mulheres, 14.2% com níveis inferiores a <10ng/ml. Relativamente à distribuição destes níveis consoante as diferentes regiões NUTS II, o Norte, Algarve e Madeira apresentaram os níveis mais baixos o que estava associado com a densidade mineral óssea e maior prevalência de fracturas independentemente da estação do ano. Níveis de Vitamina D <10ng/ml estavam independentemente associados com fracturas de fragilidade vertebrais prevalentes (OR 2.046; 95%CI 0.081,4.011; p-value 0.041). Conclusão: Os níveis de vitamina D estão abaixo do recomendado numa grande percentagem das mulheres portuguesas pós-menopáusicas e este facto está associado com fracturas vertebrais de fragilidade. As recomendações de suplementação de vitamina D devem ser revistas e as políticas de saúde devem englobar este assunto

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    Variations on the Author

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
