A mechanistic model of ADHD as resulting from dopamine phasic/tonic imbalance during reinforcement learning
Attention deficit hyperactivity disorder (ADHD) is the most common neurodevelopmental disorder in children. Although the involvement of dopamine in this disorder seems to be established, the nature of dopaminergic dysfunction remains controversial. The purpose of this study was to test whether the key response characteristics of ADHD could be simulated by a mechanistic model that combines a decrease in tonic dopaminergic activity with an increase in phasic responses in cortical-striatal loops during learning reinforcement. To this end, we combined a dynamic model of dopamine with a neurocomputational model of the basal ganglia with multiple action channels. We also included a dynamic model of tonic and phasic dopamine release and control, and a learning procedure driven by tonic and phasic dopamine levels. In the model, the dopamine imbalance is the result of impaired presynaptic regulation of dopamine at the terminal level. Using this model, virtual individuals from a dopamine imbalance group and a control group were trained to associate four stimuli with four actions with fully informative reinforcement feedback. In a second phase, they were tested without feedback. Subjects in the dopamine imbalance group showed poorer performance with more variable reaction times due to the presence of fast and very slow responses, difficulty in choosing between stimuli even when they were of high intensity, and greater sensitivity to noise. Learning history was also significantly more variable in the dopamine imbalance group, explaining 75% of the variability in reaction time using quadratic regression. The response profile of the virtual subjects varied as a function of the learning history variability index to produce increasingly severe impairment, beginning with an increase in response variability alone, then accumulating a decrease in performance and finally a learning deficit. Although ADHD is certainly a heterogeneous disorder, these results suggest that typical features of ADHD can be explained by a phasic/tonic imbalance in dopaminergic activity alone
Corrélats neuronaux de la mémoire de travail en magnétoencéphalographie à l’état de repos
Purpose: There are few studies demonstrating the link between neural oscillations in magnetoencephalography (MEG) at rest and cognitive performance. Working memory is one of the most studied cognitive processes and is the ability to manipulate information on items kept in short-term memory. Heister & al. (2013) showed correlation patterns between brain oscillations at rest in MEG and performance in a working memory task (n-back). These authors showed that delta/theta activity in fronto-parietal areas is related to working memory performance. In this study, we use resting state MEG oscillations to validate these correlations with both of verbal (VWM) and spatial (SWM) working memory, and test their specificity in comparison with other cognitive abilities.
Methods: We recorded resting state MEG and used clinical neuropsychological tests to assess working memory performance in 18 volunteers (6 males and 12 females). The other neuropsychological tests of the WAIS-IV were used as control tests to assess the specificity of the correlation patterns with working memory. We calculated means of Power Spectrum Density for different frequency bands (delta, 1-4Hz; theta, 4-8Hz; alpha, 8-13Hz; beta, 13-30Hz; gamma1, 30-59Hz; gamma2, 61-90Hz; gamma3, 90-120Hz; large gamma, 30-120Hz) and correlated MEG power normalised for the maximum in each frequency band at the sensor level with working memory performance. We then grouped the sensors showing a significant correlation by using a cluster algorithm.
Results: We found positive correlations between both types of working memory performance and clusters in the bilateral posterior and right fronto-temporal regions for the delta band (r2 =0.73), in the fronto-middle line and right temporal regions for the theta band (r2 =0.63) as well as in the parietal regions for the alpha band (r2 =0.78). Verbal working memory and spatial working memory share a common fronto-parietal cluster of sensors but also show specific clusters. These clusters are specific to working memory, as compared to those obtained for other cognitive abilities and right posterior parietal areas, specially in slow frequencies, appear to be specific to working memory process.
Conclusions: Slow frequencies (1-13Hz) but more precisely in delta/theta bands (1-8Hz), recorded at rest with magnetoencephalography, predict working memory performance and support the role of a fronto-parietal network in working memory.L’étude et la caractérisation des oscillations cérébrales au repos en magnétoencéphalographie

fronto-pariétale droite. Notre projet utilise la MEG au repos pour valider ces corrélations avec des tests
standardisés de mémoire de travail verbale et spatiale, et tester leur spécificité en comparaison avec d’autres
capacités cognitives.
Méthodologie: Nous avons enregistré 18 participants volontaires (6 hommes et 12 femmes) en
magnétoencéphalographie de repos. Nous avons évalué les capacités de mémoire de travail verbale et spatiale au
moyen de l’Indice de Mémoire de Travail de l’Échelle d'Intelligence de Wechsler pour Adultes - quatrième
édition (WAIS-IV), et le sous test d’Addition Spatiale de l’Échelle Clinique de Mémoire de Wechsler - quatrième
édition. Nous avons aussi calculé les corrélations avec les autres Indices du WAIS-IV pour évaluer la spécificité
des patrons de corrélation observés avec la mémoire de travail. Nous avons moyenné la puissance de la
décomposition spectrale pour chaque bande de fréquence (delta, 1-4Hz; thêta, 4-8Hz; alpha, 8-13Hz; beta, 13-
30Hz; gamma1, 30-59Hz; gamma2, 61-90Hz; gamma3, 90-120Hz; large gamma, 30-120Hz) puis nous avons
corrélé cette puissance normalisée par le maximum de chaque bande au niveau des capteurs avec la performance
dans un test de mémoire de travail. Nous avons ensuite regroupé les capteurs significatifs en cluster d'intérêt.
Résultats: Nous avons mis en évidence une corrélation positive entre la performance en mémoire de travail et les
régions fronto-pariétale droite pour la bande delta (r2=0,73), fronto-temporale médiale droite pour la bande thêta
(r2=0,63), et pariéto-centrale pour la bande alpha (r2=0,78). Les résultats suggèrent que les mémoires de travail
verbale et spatiale partagent un même réseau fronto-pariétal. Chaque type de mémoire de travail a aussi des
corrélations spécifiques dans des régions différentes pour certaines banques de fréquence. Comparée à d'autres
habiletés cognitives, la mémoire de travail est associée à des patrons de corrélations spécifiques avec le MEG au
repos et une région pariétale droite qui semble spécialisée dans les basses fréquences à la mémoire de travail.
Conclusions: Les basses fréquences (1-13Hz) et plus précisément delta/thêta (1-8Hz), enregistrées au repos en
MEG dans les régions frontales et pariétales, permettent de prédire la performance de mémoire de travail ce qui
supporte le rôle d’un réseau fronto-pariétal
Le rôle des symptômes d'hyperactivité- impulsivité dans la performance à une tâche de pointage manuel : quel rôle pour l'inattention?
L'intégralité de ce projet a été réalisé à l'aide de logiciels sous licence libre.Une évaluation du nombre de symptômes d'hyperactivité-impulsivité et d'inattention a été faite sur 277 jumeaux au moyen d'une entrevue semi-structurée administré aux parents, basée sur une échelle validée et communément admise, et une tâche de pointage a été demandée à chacun d'eux. La tâche de pointage a été effectuée avec chaque main successivement vers une cible dont la position à 135° ou à 45° était déterminée aléatoirement. Les tracés ont été classés en trois catégories selon le type de la première correction volontaire qu'ils présentaient sur leurs profils cinématiques : absence de correction, correction de position, correction dynamique.
Ce projet veut caractériser la relation entre le taux de chaque type de correction et le nombre de symptômes de TDAH chez une population d'enfants ne répondant majoritairement pas à un diagnostic clinique de TDAH.
Il a été montré que la présence de corrections de position était fréquemment corrélée à la présence de symptômes d'hyperactivité-impulsivité, mais pas, ou rarement, de symptômes d'inattention. D'autre part, il a été aussi remarqué que le taux de correction augmentait significativement lorsque le mouvement de pointage était effectué de la main gauche, sans égard à la dominance manuelle du sujet.
Le premier résultat n'était pas attendu, étant donné la littérature relativement épaisse montrant un lien entre l'inattention et les problèmes de coordination motrice. Le second résultat, par contre, laisse entendre que la différenciation fonctionnelle hémisphérique d'un point de vue de contrôle attentionnel, ou de la motricité de la main gauche, serait affectée par le TDAH.Symptoms linked to ADHD were assessed on 277 twins according to the commonly recognized dimensions formed by hyperactivity-impulsivity and inattention. A semi-structured interview administered to parents was used for the assessment. Subjects were asked to perform a pointing task. Left and right hands were tested in turn, and to reach a target placed either at 135° or 45°. Order of appearance was randomly determined. All traces were categorized according to the first voluntary correction as shown in their kinematic profiles : no correction, position correction, and dynamic correction.
This project proposes to characterize the link between each correction type frequency and hyperactivity-impulsivity and inattention symptoms. The population studied was mostly subclinical and did not reach ADHD diagnosis criteria.
Position corrections were shown to be correlated quite reliably to hyperactivity- impulsivity symptoms, but not to inattention symptoms. Deficits were most reliably linked with left hand use.
The first result was unexpected considering abundant literature confirming the link between inattention and fine motor deficits, but the second one is more in line with a less exploited hypothesis stating that hemispherical functional differentiation related to control of the hand, or control of attention is affected by ADHD
Child and Adolescent Mental Health Services and Interventions: Improving Quality of Care Through Collaboration
Child and adolescent mental health conditions have gained increased attention in recent years, prompting the exploration of novel approaches for effective treatment and management. This work focuses on a comprehensive population health perspective in addressing these conditions, with a particular emphasis on Attention-Deficit/Hyperactivity Disorder (ADHD). The presented chapters encompass a diverse range of research studies aimed at identifying barriers and facilitators to mental health access, introducing clinical protocols, understanding decision-making strategies, and exploring the impact of internal and external factors on treatment outcomes, all while considering the critical lens of health equity.
Chapters 2 and 3 examine the implementation of a shared-care approach to ADHD treatment in the unique context of China, delving into the identification of both barriers and facilitators that influence the adoption and effectiveness of such an approach across different metropolitan areas of the country, and within different pediatric healthcare settings. Socio-cultural factors, healthcare infrastructure, and the role of stigma are explored to shed light on the challenges and opportunities in expanding access to quality care for children and adolescents with ADHD. Presenting an innovative clinical protocol, Chapter 4 introduces a shared decision-making approach to stimulant titration for ADHD treatment. By involving patients, parents, and healthcare providers in the decision-making process, the protocol aims to enhance treatment outcomes and adherence. The significance of patient autonomy and tailored interventions is discussed, emphasizing the need to empower individuals in their healthcare journeys. Building upon the findings of Chapter 4, Chapter 5 explores the reasons behind the limited clinical adoption of placebo-controlled stimulant titration interventions for ADHD. Chapter 5 investigates the ethical, methodological, and practical considerations that have hindered, (or in some cases enabled) its use as a clinical standard of care. Chapter 6 examines how different decision-making strategies employed in stimulant titration trials lead to varying recommended maintenance doses following the trials. Herein, a shared decision-making approach that emphasizes parental collaboration, an expert decision-making approach which mimics classical top-down medicine, and a computer-based approach using a ranked-choice voting algorithm are compared. Against the backdrop of the COVID-19 pandemic, Chapter 7 delves into the experiences of parents whose children have been diagnosed with mental health conditions. The study investigates challenges in accessing care, adapting to remote interventions, and addressing the exacerbated psychosocial stressors during the pandemic.
Health equity is underscored as a central concern, as disparities in care access and support are magnified during times of crisis. Together, these works present a comprehensive exploration of child and adolescent mental health treatment from a population health perspective. The individual chapters collectively contribute to a deeper understanding of barriers, facilitators, protocols, decision-making strategies, and real-world challenges that healthcare users, practitioners, and organizational or system-level providers face, while emphasizing the critical role of health equity in shaping effective and inclusive interventions

Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Prediction of the clinical response to psychostimulant by the basal and reactive salivary cortisol in children with ADHD
Le trouble du déficit de l’attention/hyperactivité (TDA/H) est un des troubles
comportementaux le plus commun chez les enfants. TDAH a une étiologie complexe et des
traitements efficaces. Le médicament le plus prescrit est le méthylphénidate, un
psychostimulant qui bloque le transporteur de la dopamine et augmente la disponibilité de
la dopamine dans la fente synaptique. Des études précliniques et cliniques suggèrent que le
cortisol peut potentialiser les effets de la dopamine. Un dysfonctionnement du système
hypothalamo-hypophyso-surrénalien (HHS) est associé avec plusieurs maladies
psychiatriques comme la dépression, le trouble bipolaire, et l’anxiété. Nous avons fait
l’hypothèse que le cortisol influence l’efficacité du traitement des symptômes du TDAH
par le méthylphénidate.
L’objectif de cette étude est de mesurer les niveaux de cortisol le matin au réveil et
en réponse à une prise de sang dans un échantillon d’enfants diagnostiqué avec TDAH âgé
de 8 ans. Le groupe était randomisé dans un protocole en chassé croisé et en double aveugle
avec trois doses de méthylphénidate et un placebo pour une période de quatre semaines.
Les enseignants et les parents ont répondu aux questionnaires SWAN et à une échelle
d’évaluation des effets secondaires.
Les résultats ont démontrés qu’un niveau de cortisol élevé au réveil prédit les sujets
qui ne répondent pas au traitement du TDAH, si on se fie aux rapports des parents. En plus,
la réactivité au stress élevé suggère un bénéfice additionnel d’une dose élevée de
méthylphénidate selon les enseignants. Aussi, les parents rapportent une association entre la
présence de troubles anxieux co-morbide avec le TDAH et une meilleure réponse à une
dose élevée.
Cette étude suggère qu’une forte réactivité de l’axe HHS améliore la réponse
clinique à des doses élevées, mais qu’une élévation chronique du niveau de cortisol pourrait
être un marqueur pour les non répondeurs. Les résultats de cette étude doivent être
considérés comme préliminaires et nécessitent des tests plus approfondis des interactions

cortisol may enhance dopamine effects. Dysregulation of the hypothalamic-pituitaryadrenal
axis is also associated with many psychiatric disorders such as depression, bipolar
disease, and anxiety. We hypothesized that cortisol has an influence on the efficacy of the
treatment of ADHD symptoms with methylphenidate.
The objective of this study was to measure the salivary level of cortisol in a sample
of 8-year-old children with ADHD upon waking and in response to a venipuncture. The
children were then randomized to three doses of methylphenidate and a placebo in a
double-blind cross-over design. Teachers and parents rated the behaviour of the children
using the SWAN and a side effect rating scale.
The results showed that high morning cortisol is a good predictor of a nonresponder
under active medication, as reported by parents. Also, the high stress reactivity
group, but not the low stress reactivity group, demonstrated a greater benefit going to a
higher dose of methylphenidate, according to teachers. In addition, parents demonstrated an
association between anxiety comorbid disorders and a better response to a high dose of
methylphenidate.
This study suggests that a strong reactivity of the HPA axis improves the clinical
response at high dose, but that chronically elevated cortisol might be a marker for non
responders. The results of this study should be seen as preliminary and require further
testing of the possible interactions between ADHD medication and HPA activity
