Schizophrenia guidelines across the world: A selective review and comparison
Treatment guidelines provide evidence-based recommendations to assist practitioners in specific clinical situations. They are a major tool to assure and enhance treatment quality and to overcome existing disparities. However, guideline quality itself varies and needs to be considered. Based on a former review, schizophrenia guidelines with high methodological quality were identified and examined regarding updated versions. Five guidelines were selected, of which three updates have been newly evaluated with the AGREE instrument. In addition, clinical content regarding seven core topics in schizophrenia treatment decisions was compared. Guideline quality on average is good, with highest AGREE scores for the NICE guideline. Updating of the German guideline resulted in noticeable quality improvements. Regarding content, recommendations largely correspond in five areas across guidelines, whereas discrepancies or vagueness exist in two areas due to newly emerging (drug choice) or still restricted evidence (duration of antipsychotic treatment). There are increasing efforts to develop guidelines with improved quality. Also, there is a need to equalize and improve healthcare quality across countries. Since many formal and content-related issues are 'universal', development of trans-national guidelines seems indicated. Nevertheless, core guideline recommendations should be adapted to regional conditions using available tools for adaptation

Should early improvement be re-defined to better predict the maintenance of response in first-episode schizophrenia patients?

Should early improvement be re-defined to better predict the maintenance of response in first-episode schizophrenia patients? Objective: To evaluate the predictive validity of early response in first-episode schizophrenia within a 1-year follow-up trial and to compare the resulting cutoff to the currently proposed early response definition (20% improvement by week 2). Method: Receiver operator characteristic (ROC) analyses were used to identify the predictive validity of the psychopathological improvement of treatment from week 1 to week 8, regarding the maintenance of response until week 52 as well as to define the most reasonable cutoff in 132 first-episode patients. The Youden Index (maximum of sensitivity and specificity) was used to compare the newly developed and the commonly used early response definition. Results: Starting with week 6, a reasonable validity to predict the maintenance of response was found (area under the curve=0.721) with the best fitting cutoff being a 51.6% PANSS total score improvement. Using this cutoff 74 patients (56%) were correctly identified to become responder and maintain response during follow-up (sensitivity: 0.747). The Youden Index was higher applying the newly developed early response cutoff featuring higher specificity compared to the commonly used early response definition. Conclusion: Regarding long-term treatment, it seems more appropriate to base predictions of the patient's maintenance of response not before 6weeks of treatment

The reality of fiction: the ECO by Mathias Goeritz
Abstract
This article covers the full biography of a building, the Experimental Museum El Eco, designed by Germanborn and Mexican émigré artist and architect Mathias Goeritz. It provides an approach intersecting the biography of the author, the history of the building, and prominent individuals of the two cultural traditions, German and Mexican, who participated in the creation of a very special and unique building: El Eco. On the one hand, the ethics of Expressionism, the interest in non-European art, the cult of primitivism and the aesthetic system of the pair of concepts abstraction-empathy, all stemming from German culture. On the other, the pantheistic religiosity of landscape, zoomorphism and anthropomorphism, the interest in masks, and the aesthetics of monumental scale, stemming from pre-Cortesian Mexican culture. Taking the stance of intertwining Mathias Goeritz parcours with those of individuals and issues from his German past and his Mexican future – highlighting the figures of Wilhelm Worringer, Paul Westheim, Luis Barragán, Edmundo O'Gorman, and Ida Rodríguez Prampolini – this article proposes a return trip from fiction to reality, following in the footsteps of the author and comparing them with the pathway of the very building El Eco
