    Assessment of human Papillomavirus vaccination among adolescent males by general practitioners in Dordogne

    Introduction : les infections aux Papillomavirus humains (HPV) font partie des infections sexuellement transmissibles les plus fréquentes au niveau mondial et près d’un tiers des cancers concernés touchent les hommes. Le vaccin Gardasil® est recommandé chez les adolescents de sexe biologique masculin depuis le 1er janvier 2021. L’état des lieux de cette vaccination à distance de son introduction est essentiel pour mettre en évidence des éventuels freins à sa promotion. Matériels et méthodes : nous avons mené une étude observationnelle quantitative transversale des pratiques professionnelles déclarées par les médecins généralistes de Dordogne par le biais d’un questionnaire anonyme envoyé par adresse électronique pendant la période de novembre 2023 à février 2024. L’objectif principal était d’évaluer la quantité de vaccination contre les virus HPV chez les garçons de 11 à 19 ans déclarée par les médecins généralistes de Dordogne. Les objectifs secondaires étaient de déterminer les facteurs associés à une pratique déclarée de proposition de cette vaccination et de mettre en évidence les principaux freins à cette vaccination, retrouvés dans les études qualitatives. Résultats : nous avons inclus 54 médecins généralistes, dont 61% de femmes avec une majorité de médecins entre 35 et 45 ans. Selon notre objectif principal, 80% des médecins ont déclaré toujours proposer la vaccination. 73% des médecins interrogés avaient déjà vacciné des garçons contre le HPV et déclaraient en moyenne 12 adolescents vaccinés au cours des 6 derniers mois. Les principaux freins à cette vaccination étaient le manque de temps et le refus des familles. Mais, il apparaît également un manque de connaissances et de formation des médecins généralistes sur le sujet. Discussion : la vaccination des adolescents de sexe biologique masculin contre les HPV en Dordogne semble être en bien implantée malgré une divergence notable avec les chiffres nationaux jugés insuffisants. Un manque d’information semble présent chez les médecins généralistes et chez les patients. La vaccination de tous les garçons permettrait une diminution globale de la transmission du virus en protégeant de manière directe les personnes vaccinées et de manière indirecte les filles et les garçons les plus fragiles. Différentes pistes d’amélioration pourraient être envisagées : améliorer les campagnes d’information pour faire adhérer les patients et les professionnels de santé à ce vaccin, poursuivre son développement dans les collèges, intégrer les informations sur la vaccination dans le carnet de santé, ou encore proposer une consultation dédiée.Introduction: Human Papillomavirus (HPV) infections are among the most common sexually transmitted infections worldwide, with nearly one-third of the associated cancers affecting males. Gardasil® vaccine has been recommended for adolescent males since January 1, 2021. Assessing the current status of this vaccination, shortly after its introduction, is essential to identify potential barriers to its promotion. Materials and Methods: we conducted a cross-sectional observational quantitative study of professional practices reported by general practitioners in Dordogne through an anonymous questionnaire sent via email from November 2023 to February 2024. The primary objective was to assess the extent of HPV vaccination among boys aged 11 to 19 years reported by general practitioners in Dordogne. Secondary objectives were to determine factors associated with reported practice of offering this vaccination and to identify the main barriers to this vaccination, as found in qualitative studies. Results: we included 54 general practitioners, 61% of whom were female, with a majority aged between 35 and 45 years. According to our primary objective, 80% of the physicians reported always offering vaccination. 73% of the surveyed physicians had already vaccinated boys against HPV and reported an average of 12 adolescents vaccinated in the last 6 months. The main barriers to this vaccination were lack of time and family refusal. Additionally, there appears to be a lack of knowledge and training among general practitioners on the subject. Discussion : HPV vaccination among young boys in Dordogne appears to be well-established, despite a notable discrepancy with national figures considered insufficient. Lack of information seems to be present among both general practitioners and patients. Vaccination of all boys would lead to an overall reduction in virus transmission, directly protecting vaccinated individuals and indirectly protecting vulnerable girls and boys. Various avenues for improvement could be considered: improving information campaigns to encourage patients and healthcare professionals to take up this vaccine, continuing its development in colleges, integrating information on vaccination into the health record, or even offering a dedicated consultation

    Analyse du recours au médecin hospitalier par les médecins généralistes dans l’exploration et la prise en charge de l’anémie de l’adulte en Nouvelle-Aquitaine : état des lieux et perspectives

    Introduction : l’anémie est un symptôme fréquent multifactoriel rencontré en médecine de premiers recours. Son caractère multidisciplinaire implique des échanges entre les soins primaires et la médecine hospitalière tant au niveau de sa prise en charge que son exploration. Objectif : Déterminer les circonstances et modalités de recours du médecin généraliste au médecin hospitalier dans le cadre de la prise en charge et l’exploration de l’anémie. Matériel et méthode : étude descriptive transversale réalisée de juillet à novembre 2023 : 153 questionnaires ont été statistiquement analysés grâce au Logiciel R version 4.3. Résultats : 100% des médecins interrogés prenaient en charge des patients présentant une anémie et 77% d’entre eux ont déclarés l’explorer. Les critères de recours au spécialiste retenus par les médecins étaient la gravité de l’anémie à 98% et le terrain du patient à 69%, avec pour objectif la réalisation d’examen complémentaire à 53% et l’obtention d’une consultation spécialisée à 44%. Le moyen de communication privilégié était le téléphone pour 41% d’entre eux, mais celui-ci était jugé variable selon le service concerné pour 68% des médecins. 54% des médecins ont déclaré avoir déjà dû adresser un patient aux urgences faute d’interlocuteur. Le plus difficile pour les médecins était l’obtention de traitements spécialisés pour 34% d’entre eux et la réalisation d’examens complémentaires pour 31% d’entre eux. 63% des médecins étaient satisfaits de leur prise en charge. 89% des médecins estimaient que la filière anémie pouvait améliorer leurs difficultés. Conclusion : les médecins interrogés interagissent avec le médecin hospitalier dans certaines circonstances, quelques difficultés à cette interaction ont pu émerger dans ce travail. La filière anémie semble être une des pistes d’amélioration

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
