    Congenital cystic adenomatoid malformation of the lung type II: Two case report

    Congenital cystic adenomatoid malformation (CCAM) is a rare hamartomatous lesion of the lung, characterized by a cystic mass of dysplastic pulmonary tissue with proliferation of bronchial structures and loss of alveoli. From a clinical point of view, CCAM may present at birth (30-35%) with respiratory distress sometimes lifethreatening; another group of patients have a late onset (60-65%), with recurrent pulmonary infections or growth retard; the lesion can sometimes be completely asymptomatic and may only be observed with a fortuitous radiography. The evolution of the lesion may be various. Spontaneous prenatal reduction or resolution of CCAM is well reported in the literature, as well as postnatal spontaneous and complete resolution. The prognosis of neonatal CCAM is related to early diagnosis and relative treatment, and therefore is also related to the prognosis of pulmonary resection. The authors describe 2 cases of type II CCAM, observed at the Section of Pediatric Surgery of the University of Siena.     Introduction: Klippel- Trenaunay Syndrome (KTS) is characterized by a series of malformation concemig lymphatic vessels, capillaries and veins, without evidence of arterial-venousos shunt, that mainly involve one side. Typical pathological aspects are: cutaneous capillary malformations; regarding soft tissues or limbs bones hypertrophy; abnormal development of deep and superfici al veins and persistence of embryonal veins. Clinical case: We refer 11 months old patient's experience with a diagnosis of KTS, both for clinical picture, and for age was performed a conservative therapy with pressure bandage. Discussion: KTS is a rare congenital pathology. Its etiology is still unknown, but there are several embryo-genetic theories trying to explain altered angiogenesis, as VEGF (Vascular Endothelian Grow Factor) is considering the main responsible factor. The sintomathology is related to extension and site of the hypertrophy and vascular malformations. Presence of ulcer and edema at the limb, with or without pain is typical. Diagnostic protocol foresees: dermatological, orthopaedic and vascular evaluation; Rx of injuried limb; Color-Doppler; RMN; preoperative and intra-operative flebology. Regarding KTS treatment, it was proposed in literature a marginai veins "stripping", a binding of varicosity, abscission of lymphatic malformations, photocoagulation or sclerotherapy. However, the therapeutic approach remains related to the age of the patient and to the symptomatology and complications that eventually occurred. Surgical treatment, during paediatric age, is not properly opportune because of the high percentage offailure. For this reason children affected by KTS, we consider more useful a conservative treatment with pressure bandage and annual revaluation of clinical picture

    La stipsi è una condizione clinica frequente nell'età pediatrica, caratterizzata dalla scarsa frequenza di evacuazioni, con emissione di feci voluminose e di consistenza aumentata. Nei casi più ostinati diviene necessario il ricorso a clismi evacuativi, praticati di solito senza particolari difficoltà. Tuttavia, possono verificarsi episodi traumatici anche di grave entità, tali da condurre il paziente all'osservazione chirurgica urgente. Gli Autori riportano il caso clinico di una bambina affetta dalla nascita da stipsi cronica e sottoposta a intervento chirurgico di colostomia alla Mikulitz sul colon discendente, per una perforazione rettale indotta durante l'esecuzione di un clisma di pulizia. Dopo la ricanalizzazione e la contemporanea resezione di 18 cm di dolicosigma, verosimile causa della stipsi cronica, la bambina ha recuperato un normale ritmo defecatorio

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     OMENTAL CYSTS IN THE PEDIATRIC AGE: MINI-INVASIVE APPROACH. Omental cysts are benign abdominal neo-formation, rarely found at any age. Their higher frequency is reported between the 1st and the 5th year of life, and their incidence is around 1:140.000 hospital admittances. It is to underline, however, that the majority of these lesions remain unknown, because not always clinical manifestations are present and sometimes they constitutes an accidental finding during a laparotomy made for other causes. Rarity of these lesions and lack of defined clinical characteristics make diagnosis very hard. In fact, even in presence of clinical manifestations, clinical picture is always various and non specific, ranging from the simple objectivity of abdominal mass, to recurrent abdominal pain, to intestinal occlusion due to "ab estrinseco" compression on nearest intestinal loop.
