    Development and validation of a situational anxiety scale screening assessment among adults with type 2 diabetes during COVID-19 at a tertiary centre in Chennai, India

    Aim: The aim of this study was to develop and validate the situational anxiety scale (SAS) during COVID-19 among adults with type 2 diabetes attending a tertiary diabetes center in Southern India. Materials and Methods: A total of 100 individuals aged from 18 to 65 years with type 2 diabetes attending a tertiary care diabetes center completed a structured SAS at two visits. The first visit (visit 1) survey was conducted in April 2021 and the second visit (visit 2) survey was conducted in March 2022. The SAS was administered to all 100 individuals. The State Trait Anxiety Inventory Scale (STAI-S) consisting of 20 questions was administered to the same 100 individuals in addition to the SAS during Visit 2. Results: The SAS showed good internal consistency for visit 1 (α = 0.855) and visit 2 (α = 0.795). Exploratory factor analysis showed four factors and explained 69% of variance. The four factors identified were as follows: (1) fear, (2) desire for COVID-free state, (3) lack of interest and energy, and (4) financial worries. A weak positive correlation was observed between SAS visit 2 and STAI-S, and it was statistically significant (r = 0.223; P = 0.026). Conclusion: The SAS is a valid and reliable tool for measuring situational anxiety during pandemics and post-COVID anxiety levels, which can help in the development of a holistic approach

    Reliability and Validity of a Modified PHQ-9 Item Inventory (PHQ-12) as a Screening Instrument for Assessing Depression in Asian Indians (CURES - 65)

    Abstract Objectives: To evaluate the validity and reliability of the modified Patient Health Questionnaire(PHQ) 12 item instrument as a screening tool for assessing depression compared to the PHQ -9 in a representative south Indian urban population. Methods: The Chennai Urban Rural Epidemiology Study [CURES] is a large cross-sectional study conducted in Chennai, South India. In Phase 1 of CURES(urban component), 26,001 individuals aged ≥ 20 years individuals were selected by a systematic sampling technique of whom one hundred subjects were randomly selected, using computer-generated numbers, for this validation study. Two self-reported questionnaires (modified PHQ-12 item and PHQ 9 item) were administered to the subjects to compare their effectiveness in detecting depression. Reliability and validity were assessed and Receiver Operating Characteristic (ROC) curves were plotted. Pearson’s correlation was used to compare the two questionnaires. Results: The mean age of the study was 38.6±11.6 years and 48% were males. Pearson’s correlation coefficient between the modified PHQ-12 and the PHQ-9 item was 0.913 [p<0.0001]. Factor Analysis revealed that the modified PHQ 12 item scale can be used as a unidimensional scale and had excellent internal consistency(Cronbach’s alpha:0.88). A cut point of >4 calculated using the ROC curves for the modified PHQ- 12 item had the highest sensitivity (92.0%) and specificity (90.7%) using PHQ-9 as the gold standard. The positive predictive value was 76.7%, and the negative predictive value, 97.1% and the area under the ROC curve, 0.979 (95% Confidence Interval: 0.929 - 0.997, p<0.0001). Conclusion: The modified PHQ-12 item is a valid and reliable instrument for large scale population based screening of depression in Asian Indians and a cut point score of 4 or greater gave the highest sensitivity and specificity.

    Not AvailableWilt of sugarcane is a serious disease affecting sugarcane production in many sugarcane growing regions. Since the associated pathogen has not been clearly established with the disease, detailed studies were conducted to identify the fungal species by pathogenicity and molecular variation. Selected 10 isolates of Fusarium associated with sugarcane wilt were tested for their pathogenicity on the cv Co 86032 and among them the isolates Fs 032 TN4L2 and Fs 121 UP1 originated from Tamil Nadu and Uttar Pradesh, respectively had shown severe disease and proved to be more virulent. Also pathogen inoculation by plug method combined with abiotic stress on selected sugarcane varieties at Coimbatore, caused typical wilt symptoms in cvs Co 6304 and Co 86249 and partial symptoms on CoC 90063 and CoC 92061 under field conditions. However, disease at endemic location in Gujarat the cvs Co 86002, Co 95020, Co 97008, Co 98010 and Co 0323 exhibited typical wilt in response to pathogen inoculation by plug method. Further studies on molecular variation using specific RAPD and ISSR markers were taken up to confirm the pathogenic isolates of Fusarium. The molecular markers such as 1000 bp fragment of RAPD primer OPA13, 650 bp fragment in ISSR1, 720 bp fragment in ISSR5 and 880 bp fragment in ISSR9 very clearly demarcated pathogenic and nonpathogenic isolates. The field and laboratory studies very clearly confirmed that the wilt pathogenic isolates belong to F. sacchari and this is the first attempt that generated molecular markers which correlated with pathogenicity of F. sacchari isolatesNot Availabl

    Glipizide improves plasma C-peptide level over a three year period after diagnosis of type 2 diabetes

    Twenty type 2 diabetic patients treated with glipizide monotherapy were followed for a period of three years. Fasting and postprandial plasma glucose values, glycosylated haemoglobin and fasting and stimulated C-peptide levels were measured annually. At the end of three years, there was a significant reduction in fasting plasma glucose (p = 0.002), post prandial plasma glucose (p = 0.001) and glycosylated hemoglobin levels (p &lt; 0.001). The fasting and stimulated C-peptide levels increased from 0.4 pmol/l to 0.8 ± 0.4 pmol/l (p = 0.045) and 0.9 pmol/l to 1.4 pmol/l (p = 0.028) respectively. The study shows that after glipizide monotherapy for first three years after diagnosis of type 2 diabetes, the pancreatic beta cell reserve is not only preserved but also marginally improved
