Developmental changes in rat surfactant lipidomics in the context of species variability.
Lung surfactant comprises mainly phosphatidylcholine (PC) species together with phosphatidylglycerols and surfactant proteins (SP) SP-A to -D. Changes in the concentrations of its principal components dipalmitoyl-PC, palmitoylmyristoyl-PC, palmitoylpalmitoleoyl-PC relative to developmental, structural and physiological differences are only partially understood. Particularly, their attribution to differences in air-liquid interface curvature, compared with dynamic parameters, such as respiratory rate, are controversial.We postulated that during alveolarization the changes in these principal PC components of surfactant differ from those in other phospholipid parameters, and that across endothermic vertebrates their concentrations are related to lung physiology rather than structure. We therefore investigated in rats from postnatal day (d)1 to d42 the pattern of surfactant phospholipids relative to alveolarization (d4-d14), and we discuss these changes in terms of molecular adaptation to pulmonary structure or physiology. Contrary to mammals with advanced alveolarization and increased respiratory rate (RR) at term, concentrations of dipalmitoyl-PC (49-52%) and palmitoylmyristoyl-PC (7-9%) in lung lavage fluid were identical at d1 and d42. At d7-d14, when in rats RR is increased, palmitoyl-myristoyl-PC transiently increased by 2.5- to 3.9-fold at the expense of dipalmitoyl-PC (-32% to 34%) and palmitoyl-palmitoleoyl-PC (-16%). Other lipidomic changes followed essentially different patterns of increase or decrease. Palmitoyl-myristoyl-PC was increased in large aggregates suggesting that it is an integral component of active surfactant. In the overall context of vertebrates, irrespective of age and lung structure, fractions of palmitoyl-myristoyl-PC, dipalmitoyl-PC and palmitoyl-palmitoleoyl-PC correlate with differences in RR rather than alveolar curvature. Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Randomized controlled trial of oral kreatine supplementation to infants with apnea of prematurity
Hintergrund: Sowohl hypoxische Atemdepression als auch muskuläre Erschöpfung spielen in der Genese von Frühgeborenenapnoen eine Rolle. Kreatin ist zum einen eine entscheidende Substanz in Regulation und Homöostase des muskulären Energiestoffwechsels, zum anderen wurde in Tierversuchen gezeigt, dass durch orale Substitution mit Kreatin das Ausmaß der hypoxischen Atemdepression verringert werden konnte. Mit dieser Studie sollte untersucht werden, ob durch orale Kreatinsubstitution bei Frühgeborenen die idiopathische Apnoesymptomatik verbessert werden kann.
Methodik: In dieser doppelblinden, randomisierten und kontrollierten Studie wurden 34 Frühgeborene mit einem Gestationsalter < 32 Wochen und koffein-bedürftiger Apnoesymptomatik für 2 Wochen oral entweder mit 200 mg/kg/d Kreatin oder Placebo behandelt. Jedes Kind erhielt an Tag 0, 7 und 14 eine jeweils 6-stündige Polygrafie mit Aufzeichnung von EKG, nasalem Luftstrom, Atembewegungen, pulsoxymetrischer O2-Sättigung und Pulskurve zur Erkennung von Artefakten. Diese Aufzeichnungen wurden auf die kombinierte Anzahl von Bradykardien (Herzfrequenz < 2/3 der Basalfrequenz) und Hypoxämien (SpO2 < 80%) pro Stunde Aufzeichnungszeit, sowie Anzahl von Apnoen, Bradykardien und Hypoxämien und Basalwerte für Sättigung, Herz- und Atemfrequenz hin untersucht. Außerdem wurde am Tag jeder Messung die Kreatinausscheidung im Urin bestimmt.
Ergebnisse: Jeweils 17 Frühgeborene erhielten Kreatin (K) oder Placebo (P). Gestationsalter (Median und Bereich) bei Geburt (27 Wo (25-30) vs. 26 (24-30)) und zum Zeitpunkt der Untersuchung (27 Wo (26-36) vs. 27 (27-33)) sowie Geburtsgewicht (1040g (580-1400) vs. 1015g (440-1525)) waren in beiden Gruppen gleich. Die orale Gabe wurde gut vertragen, Nebenwirkungen nicht beobachtet. Die Kreatinsubstitution hatte weder einen Effekt auf den primären (Bradykardie & Hypoxämie nach 1 Wo: K 5,2/h (0,6-52,8), P 5,2/h 1,3-43,8); nach 2 Wo: K 7,1/h (1,0-45,2), P 4,9/h (0,4-16,0)), noch auf die sekundären Outcome-Parameter. Die Kreatinausscheidung war vor Therapie niedrig (27 (18-102) mmol/mol Kreatinin) und zeigte einen deutlichen Anstieg unter Kreatinsubstitution (6949 (1427-11807 mmol/mol Kreatinin) in der K-Gruppe.
Schlussfolgerungen: Die Auswertung der erhobenen Daten ergab mit Ausnahme der Kreatinausscheidung keine signifikanten Unterschiede zwischen beiden Gruppen. Im Vergleich zu reifen Neugeborenen war die Kreatinausscheidung bei den Frühgeborenen vor Behandlungbeginn deutlich erniedrigt, zeigte aber in der Behandlungsgruppe einen drastischen Anstieg unter Kreatinsubstitution. Diese Zunahme nach oraler Zufuhr von Kreatin spricht für eine gute intestinale Resorption. Trotzdem wurde die Apnoesymptomatik der Frühgeborenen durch Kreatinsubstitution nicht verbessert.
In diesem Zusammenhang muss hinterfragt werden, ob die Dosis zu gering war, möglicherweise ein früherer Zeitpunkt der Substitution (z.B. pränatale Behandlung über die Mutter) effektiver gewesen wäre oder aber die hypoxische Atemdepression in der Ätiologie der Frühgeborenenapnoen nicht die entscheidende Rolle spielt, bzw. Kreatin aufgrund seines Wirkmechanismus zur Behandlung der muskulären Erschöpfung nicht geeignet ist. Eine Prophylaxe von Frühgeborenenapnoen mit Kreatin kann derzeit nicht empfohlen werden.Background: We hypothesized that creatine supplementation (CS) may improve symptoms of apnea of prematurity (AOP) because it reduced hypoxic ventilatory depression (HVD) in rodents and muscle fatigue in men which both appear involved in the pathophysiology of AOP.
Methods: A prospective, double blind controlled study was conducted on 34 spontaneously breathing infants with a gestational age = 4 s), bradycardia (heart rate <= 2/3 of baseline) and desaturation (SpO2 <= 80%) as well as for baseline heart rate, respiratory rate and SpO2.
Results: 17 infants each received CS or P. Median (range) gestational age at birth (27 wk (25-30) vs 27 wk (25-30), birthweight (1040g (580-1400) vs 1015g (440-1525)) and post-conceptional age at study (29 wk (26-36) vs 29 wk (27-33)) were similar between groups. Oral CS was well tolerated, no side effects were noted. Urinary creatine excretion (mmol/mol creatinine) was low in the P group (median 27, range 18-102) and increased significantly in the CS group (6949, 1427-11807, p<0.05)). After 14 days of treatment there was no effect of CS on the combined rate of bradycardias and desaturations/h (P group: median 5, range 0,4-16, CS group:7, 1-45) nor there was a decrease in the number of apneas/h (P group: 12, 4-34, CS group 11,5, 2,5-69).
Conclusion: These preterm infants had low creatine levels compared to term neonates (normal range: 40-360 mmol/mol creatinine). Oral creatine was clearly absorbed, as demonstrated by the dramatic increase in urinary excretion, but did not, in the dose and for the duration given in this study, improve symptoms of AOP. This finding may have been related to an insuffcient dose or delayed onset of supplementation. Further, it might be conceivable that HVD and muscle fatigue were less relevant to the pathophysiology of AOP than previously thought
Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more
sophisticated methods
Rotavirus infection in neonates : Survey on the prevalence in neonatal intensive care units
Im November 2002 wurde im Rahmen von Umgebungsuntersuchungen eines Rotavirusausbruchs in der Neonatologie des UK Tübingen auch in Stuhlproben asymptomatischer Früh- und Neugeborener Rotavirus nachgewiesen. Dies sprach für eine höhere Prävalenz von Rotavirusinfektionen auf den Neonatalstationen als bisher angenommen; diese sollte bestimmt werden. Weiterhin sollte die Hypothese überprüft werden, dass durch die Etablierung einer Reverse Transkriptase Polymerase-Kettenreaktion (RT-PCR) ein sensitiverer Nachweis bzw. Ausschluss einer Infektion erreicht werden kann. Außerdem interessierte, ob es charakteristische Infektionssymptome bei Früh- und Neugeborenen gibt, wie lange Neugeborene das Virus ausscheiden und ob zwischen Rotavirusinfektionen und der Entstehung einer nekrotisierenden Enterokolitis (NEC) ein Zusammenhang besteht.
Dazu erfolgte im Rahmen einer prospektiven Erhebung vom 1.1.03 bis zum 30.04.03 die Untersuchung von Stuhlproben aller auf den neonatologischen Intensivbehandlungs- und Intensivüberwachungsstationen des Uiversitätsklinikums Tübingen aufgenommenen 123 Patienten auf Rotavirus. Bei 28 Kindern (23%) konnte eine Virusausscheidung im Stuhl mittels Enzym-linked immunosorbent assay (ELISA) und/oder RT-PCR nachgewiesen werden. 6 Kinder schieden das Virus bereits in den ersten Lebenstagen aus; auch in Mekonium wurde das Virus nachgewiesen. Beide Befunde sprechen für die Möglichkeit einer perinatalen Transmission dieser Viren.
Die Übereinstimmung zwischen ELISA und RT-PC war minimal: in nur 4 von 42 Stuhlproben gelang mit beiden Nachweisverfahren ein Virusnachweis. Charakteristische Infektionssymptome bei Früh- und Neugeborenen konnten nicht identifiziert werden. Weder traten eine hohe Stuhlfrequenz noch eine große Stuhlmenge, wässrig-zerhackte Stühle oder Dehydratationen häufiger bei infizierten als bei nicht-infizierten Kindern auf.
Frühgeborene unterschieden sich nicht signifikant von Reifgeborenen. Allerdings schienen unreifere Neugeborene generell eine höhere Anfälligkeit für eine Infektion mit Rotaviren zu haben und das Virus auch über einen längeren Zeitraum auszuscheiden. Beides kann die hohen Infektionsraten auf der neonatologischen Intensivstation erklären und macht deutlich, wie wichtig die Beachtung strenger Maßnahmen zur Prävention und Infektionskontrolle ist.
Von den untersuchten Neugeborenen entwickelte ein mit Rotaviren infiziertes Frühgeborenes eine NEC. Aus diesem Einzelfall können keine Rückschlüsse gezogen werden, doch an einen möglichen Zusammenhang zwischen einer Rotavirusinfektion und der Entstehung einer NEC sollte gedacht werden.
Das Füttern von Muttermilch kann mit einem geringeren Infektionsrisiko assoziiert werden: Der Anteil der mit Muttermilch ernährten Neugeborenen in der nicht-infizierten Kontrollgruppe war signifikant höher als der unter den infizierten Neugeborenen.
Die Ursache(n) für die geringe Übereinstimmung zwischen ELISA und RT-PCR bleibt (bleiben) unklar. Zu diskutieren, aber aufgrund der hier erhobenen Daten nicht zu beweisen, sind falsch positive Ergebnisse aufgrund von Kreuzreaktionen des ELISA mit bakteriellen Antigenen und/oder Veränderungen viraler RNA durch das in dieser Studie unvermeidliche Einfrieren und Auftauen der Stuhlproben. Aufgrund der Ergebnisse zum Methodenvergleich kann zur Zeit weder der ELISA-Antigen-Test noch die RT-PCR als optimaler Test für den Nachweis einer Rotavirusinfektion bzw. für den Ausschluss einer Infektiosität bei Neugeborenen betrachtet werden.
Rotavirusinfektionen treten bei Neugeborenen häufig auf, und trotz der diskreten Symptomatik kann ein Zusammenhang mit anderen schwerwiegenderen Erkrankungen nicht ausgeschlossen werden. Letzteres wie auch die hohe Kontagiosität sprechen für ein Screening bei Neugeborenen auf Rotavirusinfektionen. Die diskrete Symptomatik der meisten Infizierten lässt dagegen ein Screening nicht als erforderlich erscheinen. Bevor hier eine Entscheidung getroffen werden kann, sollten weitere Untersuchungen zum optimalen Nachweisverfahren für Rotavirusinfektionen bei Neugeborenen durchgeführt werden.During an outbreak of Rotavirus (RV) on all neonatal units at the Department of Neonatology at the University Children’s Hospital in November 2002, RV was even detected in stool samples of asymptomatic neonates. This indicated a higher prevalence of RV infections on neonatal wards than previously assumed; we wanted to determine this prevalence. Furthermore, the hypothesis was tested that a higher sensitivity of RV detection can be achieved by establishing a Reverse Transcriptase – Polymerase Chain Reaction (RT-PCR) for RV. Besides, we examined if there are characteristic symptoms of infected neonates, how long neonates excrete the virus and if there is a link between RV infection and emergence of necrotizing enterocolitis (NEC).
Stool samples of all 123 patients admitted to the neonatal intensive care unit (NICU) between January 1, 2003 to April 30, 2003 were prospectively tested for RV. RV was detected in 28 neonates (23%) via Enzyme-linked immunosorbent assay (ELISA) and/or RT-PCR.
Virus shedding during the first days of life occurred in 6 newborns; RV was even detected in meconium. This indicates a possible perinatal transmission of the virus.
Agreement between the results of ELISA and RT-PCR was poor: only in 4 samples (out of 42) RV could be detected with both methods.
Characteristic symptoms for infection in neonates could not be identified. Neither a higher stool frequency nor a higher stool quantity, watery stools or signs of dehydration occurred more often in infected than in uninfected newborns.
There was no significant difference between term and preterm neonates. However premature infants seemed to have a higher likelihood of being infected with Rotavirus as well as a prolonged shedding of the virus. This can explain the high infection rates on the NICU and reveals the importance of taking precautions for prevention and infection control.
One of the premature neonates who became infected with RV subsequently developed a necrotising enterocolitis (NEC). It is not possible to draw conclusions out of this individual case but the possibility of a connection between RV infection and emergence of NEC should be considered.
Breastfeeding can be associated with a reduced risk of RV infection: The proportion of breast-fed neonates in the non infected control group was significantly higher than among the infected neonates.
The reason for the poor agreement between ELISA and RT-PCR results remains unclear. False positive results resulting from cross reactions in the ELISA with bacterial antigens and/or alterations of the viral RNA through the inevitable cycles of freezing and thawing have to be discussed, but cannot be proved on the basis of the data included in this study. Based of these results, neither the ELISA nor the RT-PCR can be considered as an optimal test for detection of RV infection in neonates.
Infections with RV are common in newborns. Despite the discrete symptoms, a relationship with other, more serious sequelae cannot be excluded. This and the high contagiosity argue in favor of a screening for RV infections. In contrast, however, the mild symptoms argue against a routine RV screening. Before a decision can be reached, further studies on detection methods for RV in neonates should be performed

We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
