    Upfront surgery for N2 NSCLC: a large retrospective multicenter cohort study

    Background: The optimal sequence and combination of surgery, chemotherapy and radiotherapy in patients with N2 non-small cell lung cancer (NSCLC) remain undefined. The aim of our study was to compare two treatment options for N2 NSCLC-induction therapy with subsequent surgery versus upfront surgery with adjuvant treatment. Methods: We retrospectively reviewed 405 patients with N2 disease in two centers, between January 2010 and December 2016. They were divided into two groups: the Induction Group, composed of patients who received neoadjuvant chemotherapy, and the Upfront surgery Group, composed of patients who underwent surgery as first-line therapy. Propensity score-matched (PSM) analysis was performed, and 52 patients were included in each group. Primary endpoints were: recurrence, overall survival (OS) and disease-free survival (DFS). Results: After the PSM, no differences were observed in general characteristics, perioperative results, rates and severity of complications, and histopathology results. Seventeen patients (32.7%) of the induction group and 21 (40.4%) of the upfront surgery group had mediastinal lymph nodal involvement with skipping (p = 0.415). Recurrence rate was not different between the two groups (57.7% vs 50.0%, p = 0.478). No differences were observed in terms of OS (40.98 ± 35.78 vs 37.0 ± 40.69 months, p = 0.246) and DFS (29.67 ± 36.01 vs 27.96 ± 40.08 months, p = 0.697). The multivariable analysis identified the pT stage and skipping lymph node metastasis as independent predictive factors for OS. Conclusions: Upfront surgery followed by adjuvant therapy does not appear inferior in terms of recurrence, OS and DFS, compared to induction chemotherapy with subsequent surgery

    A Multicenter Retrospective Cohort Study on Superior Vena Cava Resection in Non-Small-Cell Lung Cancer Surgery

    Background: Surgery for non-small-cell lung cancers (NSCLCs) invading the superior vena cava (SVC) is rarely performed due to surgical complexities and reported poor prognoses. Different methods have been described to reconstruct the SVC, such as direct suture, patch use or prosthesis, according to its circumferential involvement. The aim of our study was to analyze the short- and long-term results of different types of SVC resection and reconstruction for T4 NSCLCs. Methods: Between January 2000 and December 2019, 80 patients received an anatomical lung resection with SVC surgery in this multicenter retrospective study. The partial resection and direct suture or patch reconstruction group included 64 patients, while the complete resection and prosthesis reconstruction group included 16 patients. The primary endpoints were as follows: long-term survival and disease-free survival. The secondary endpoints were as follows: perioperative complications and 30- and 90-day mortality. Unpaired t-tests or Mann–Whitney U tests for non-parametric variables were applied to discrete or continuous data, and the chi-square test was applied to dichotomous or categorical data. Survival rates were calculated using the Kaplan–Meier method and compared using the log-rank test. Results: No differences were found between the two groups in terms of general characteristics and surgical, oncological and survival outcomes. In particular, there were no differences in terms of early (50.0% vs. 68.8%, p = 0.178) and late complication frequency (12.5% vs. 12.5%, p = 1.000), 30- and 90-day mortality, R status, recurrence, overall survival (33.89 ± 40.35 vs. 35.70 ± 51.43 months, p = 0.432) and disease-free survival (27.56 ± 40.36 vs. 31.28 ± 53.08 months, p = 0.668). The multivariate analysis demonstrated that age was the only independent predictive factor for overall survival. Conclusions: According to our results, SVC resection has good oncological and survival outcomes, regardless of the proportion of circumferential involvement and the type of reconstruction

    Gaius’ “Institutiones” by Cezary Kunderewicz

    In his work the author presents the process of creating the first complete Polish translation of Gaius’s Institutiones by Cezary Kunderewicz (in the 19th c. the first two books were translated into Polish by Dydyński). The translation was published by PWN in 1982 (in 2003 another Polish edition was published by Rozwadowski). The author supposes that professor Kunderewicz probably started his translation in the early 1970s. He refers to his own memories as he is now the oldest disciple of Professor Kunderewicz. The author mentions that Kunderewicz was excellently prepared for this difficult work. Several years earlier Kunderewicz translated the treatise on Roman Aqueducts written by Frontinus. The article is clearly based on memories. This is the reason why the author is quoting only one review of Kunderewicz’s translation and why he does no try to evaluate the translation. In the end the author writes also about Kunderewicz’s another translation – Polish edition of Justinian’s Institutes, which was published in 1986

One of the last judgments of the Strasburg Tribunal (Łaszkiewicz v. Poland case of January 15, 2008, ECHR nr 28481/03) constitutes the background for the deliberation. Violation of article 5 § 4 was announced because the procedure of the judicial application of the detention in preparatory proceedings in the Polish 1997 Code of Criminal Procedure does not fulfil the requirements of the adversarial procedure based on the principle of equal weapons. According to the Polish procedural regulations, the court grants the defence access to the files of not only preparatory proceedings but also or even most of all, to the detention motion, upon the approval of the prosecutor. Those regulations constituted basis for the recognition of the complaint by the Strasburg Tribunal. Based on the statistical data, author analyzes the reasons of weakness of the judicial control over the detention in the provisions of the 1997 Code of Criminal Procedure and reaches to the procedural practice based on the provisions of the previous Code of 1969. He indicates the “mudded” responsibility between the organs deciding upon the detention: applying prosecutor and deciding court. The article also deals with the problem of favor procuratori that is the need of some cooperation between the advocate and prosecutor necessary for the effective defence in the preparatory proceedings. Author emphasizes dangers arising from an increasing significance of the operational and identification actions in the Polish criminal procedure. He calls for the amendment of the provisions of the 1997 Coder of Criminal Procedure challenged by the ECHR.Prof. dr hab. Cezary Kulesza was born in Rajgród, Poland, on August, 3, 1960. He was graduated in Economy (1983) and in Law (1985) at Warsaw University, Branch in Białystok. Ph.D. in Law since 1989 after graduation with the dissertation “Examining justice in the preparatory proceedings’ model in the comparative perspective”(published Białystok 1991) at the Warsaw University in Warsaw. Also, at Warsaw University he has defended on November, 1996, his habilitation dissertation “The injured person in the Polish criminal proceedings in the light of law and practice of the selected western countries (published Białystok 1995) .On March, 2007, he received Professor degree & since November 2007 he is Chief of the Chair of Criminal Proceedings at the University in Białystok. 1993-1998 Member of the Supervisory Board of the Polish Society on Victimology. In 1990 he passed judge exam & legal advisor exam in Białystok. Since 2003, professor Cezary Kulesza is practicing advocate in Białystok. He has presented numerous papers at national and international conferences and symposia. He was visiting professor at Villanowa University (Fulbright Scholarship, Philadelphia, 1993), University in Tübingen (1992) and University in Saloniki (2004). Author of many academic publications on a very wide range of subjects, including criminal procedure, victimology, economic & tax crimes, the role of the defence counsel in criminal proceedings.     A Mature Artist and Digital Culture. Online Presence and Activity Strategy using the Example of Cezary Pazura

    The article tries to analyse the YouTube activity of Cezary Pazura and his presence in what is widely understood as digital culture. He is the only actor in Poland who successfully develops his work using that platform. That is why it is important to take into consideration his mature age and professional experience which indicate how to develop the growth of the channel. The author explains why Pazura decided to become a YouTuber, then podcaster, moreover the author presents his concerns and personality traits which predestine him to internet activities. She examines the structure and the main themes of the videos, such as: the backstage work of blockbuster movies, cult film sets, a detailed description of Polish and foreign films, art of acting, special guests, subscribers’ jokes, memes, and unboxing. The article reveals the strategy which helped Pazura to build an engaged community around the channel, for example the cooperation with popular, young YouTubers. It points out that digital activity influenced the change regarding his image — Pazura is no longer seen only through the prism of comedy performances and receives more serious roles. He gained a wider audience as tributes to their idol’s achievements appeared in the comments section. So far, he is the only person in Poland working effectively as an actor, YouTuber and podcaster

    Impact of Pulmonary Ligament Resection in Upper Lobectomies: A Multicenter Matched Cohort Study

    Background: Division of the pulmonary ligament is standard in lower lobectomies, but its application in upper lobectomies remains controversial due to potential complications like atelectasis and bronchial kinking. This retrospective matched cohort study aimed to evaluate the efficacy and safety of ligament resection in upper lobectomies for oncological purposes. Methods: From January 2015 to December 2020, 988 patients who underwent minimally invasive upper lobectomies across multiple centers were identified. They were categorized into ligament resection and no ligament resection groups, with propensity score matching (PSM) to minimize confounding factors. Endpoints included operative time, pleural effusion, complications (frequency and Clavien–Dindo scores), chest drainage removal, length of stay, pleural space, collapse rate, and bronchial kinking. Results: Following PSM, 276 patients were included in each group, with no significant differences in baseline characteristics. Ligament resection correlated with longer operative times, increased lymphadenectomy sampling at station #9 (p < 0.001), and a bigger change in the bronchial angle (p < 0.001). No statistically significant differences were observed for the other endpoints. Conclusions: Ligament resection during upper lobectomy may impact the bronchial angle without immediate postoperative outcome changes. Further research is necessary to comprehensively assess the risks and benefits of ligament resection in upper lobectomies for neoplastic disease

Są najczęstszą przyczyną zgłaszania się do lekarza i mają znaczący wymiar ekonomiczny. W związku z rozwojem cywilizacyjnym, stresem , siedzącym trybem życia i ograniczeniem aktywności procesy degeneracyjno - wytwórcze w kręgosłupie szyjnym stają się bardziej powszechne. Radikulopatia szyjna jest neurologicznym stanem wywołanym kompresją lub uszkodzeniem korzeni nerwowych szyjnego odcinka kręgosłupa. Stan ten generuje ból w obrębie szyi nie rzadko promieniuje do obręczy barkowej i kończyn górnych. Postępowanie diagnostyczne jest procesem wieloetapowym. Wywiad i badanie fizykalne pełnia znaczącą rolę a badania obrazowe uzupełniają pełną diagnozę. Farmakoterapia i ograniczenie obciążeń redukują ból w ostrej fazie. W przypadku nieskuteczności leczenia zachowawczego , przewlekłego bólu czy tez postępujących deficytów neurologicznych przeprowadzenie zabiegu operacyjnego obarczającego uciśnięte struktury nerwowe staje się jedynym słusznym postępowaniem . Słowa kluczowe: szyjna dyskopatia, radikulopatia, ból kręgosłupa szyjnego Abstract The cervical spine pain represents serious diagnostic medical problem .It is the most common reason for visiting the doctor and has economic dimension . According to Development of Civilization , stress , sitting lifestyle and limitation of physical activity Degenerative and Osteoarthritic syndromes become more common. Cervical radiculopathy is a neurological condition caused by compression or damage of the nerve roots of the cervical spine. This leads to the occurrence of the neck pain radiating to the shoulders and upper extremities. Diagnostic path is a multistage process. Medical interview and physical examination perform essential role . Imaging tools complete proper diagnosis . Pharmacotherapy and short term immobilization reduce pain in a acute phase of disease . However when conservative treatment fails and the patient reports chronic pain, progressive neurological deficits appears , surgery may be considered in order to decompress constricted neurological structures, as a best relieving pain treatment .
