Is the carotid plaque rupture a pivotal event in stroke pathogenesis? Update on the role of the intraplaque inflammatory processes
Stroke, a leading cause of death or disability worldwide, is frequently dependent on the rupture of carotid atherosclerotic plaques. It is therefore extremely important to study the mechanisms of formation, progression and eventually rupture of the plaques. Vulnerability of the plaque, the intrinsic tendency to lose its integrity and consequently to induce a dramatic atherothrombotic or embolic event, is still an elusive concept because many players are involved and the clinical picture is frequently the sum of different contrasting (pro-inflammatory and anti-inflammatory) phenomena. This review will discuss recent advancements in the comprehension of this topic. In particular inflammatory activation at the level of the carotid plaque will be described in the attempt to underline the main factors for the formation, the chronic persistence and the risk of rupture. Since these topics can be studied in humans only with plaque samples obtained following endarterectomy, research has tried to evaluate the role of different biomarkers which could be useful for the definition of the vulnerability of a carotid plaque, or, with more clinical relevance, of a patient; some recent results from our group will be discussed. A significant help for clinical decisions may also come from imaging tools, both well established ultrasound and more sophisticated options, such as magnetic resonance and positron emission tomography. This large amount of scientific information should allow the development of new therapeutic approaches for the effective prevention of cerebrovascular events
Effect of rosuvastatin in patients with chronic heart failure (the GISSI-HF trial): a randomised, double-blind, placebo-controlled trial
Background: Large observational studies, small prospective studies and post-hoc analyses of randomised clinical trials have suggested that statins could be beneficial in patients with chronic heart failure. However, previous studies have been methodologically weak. We investigated the efficacy and safety of the statin rosuvastatin in patients with heart failure. Methods: We undertook a randomised, double-blind, placebo-controlled trial in 326 cardiology and 31 internal medicine centres in Italy. We enrolled patients aged 18 years or older with chronic heart failure of New York Heart Association class II-IV, irrespective of cause and left ventricular ejection fraction, and randomly assigned them to rosuvastatin 10 mg daily (n=2285) or placebo (n=2289) by a concealed, computerised telephone randomisation system. Patients were followed up for a median of 3·9 years (IQR 3·0-4·4). Primary endpoints were time to death, and time to death or admission to hospital for cardiovascular reasons. Analysis was by intention to treat. This study is registered with ClinicalTrials.gov, number NCT00336336. Findings: We analysed all randomised patients. 657 (29%) patients died from any cause in the rosuvastatin group and 644 (28%) in the placebo group (adjusted hazard ratio [HR] 1·00 [95·5% CI 0·898-1·122], p=0·943). 1305 (57%) patients in the rosuvastatin group and 1283 (56%) in the placebo group died or were admitted to hospital for cardiovascular reasons (adjusted HR 1·01 [99% CI 0·908-1·112], p=0·903). In both groups, gastrointestinal disorders were the most frequent adverse reaction (34 [1%] rosuvastatin group vs 44 [2%] placebo group). Interpretation: Rosuvastatin 10 mg daily did not affect clinical outcomes in patients with chronic heart failure of any cause, in whom the drug was safe. An inflammatory approach in the therapeutic choices for the prevention of atherosclerotic events
Effects of blood pressure control in cardiovascular prevention
Background: High blood pressure is the main risk factor worldwide for mortality and morbidity. Subjects with uncontrolled hypertension increased in the last decades. Methods: This review is based on the material searched for and obtained via MEDLINE and PubMed up to June 2016. The search terms used were “hypertension, blood pressure control” in combination with “pathophysiology, lifestyle, antihypertensive drugs, target organ damage, target values and comorbidity”. Results: This narrative review focused its attention on the diagnosis, the pathophysiology, the clinical consequences of arterial hypertension, and on the factors that must be considered for a better blood pressure control. In fact, the attainment of an adequate blood pressure control is a challenge at both a population and an individual level. Conclusion: The review will discuss the best strategy to reduce uncontrolled hypertension and cardiovascular events in hypertensive patients identifying the main conditions which determine and maintain uncontrolled hypertension also highlighting the new possible strategies for a better blood pressure control and including the results of very recent multicenter randomized controlled trials

Gipogo and Pumbu a mfumu: chiefly power masks among the Pende of Kasai
The aim of this article is to indicate the political dimension of two wooden mask types of the Kasai Pende. Attention is drawn to both formal and functional aspects. Research has been dictated by the available literature on the subject and by a morphological analysis of a vast number of masks. Although the chief’s houses are the most impressive artistic achievements with regard to Pende chiefly power, forceful dominant behaviour is expressed in a dynamic way in Pende masquerades. Among the Kasai or eastern Pende the Gipogo and Pumbu a mfumu mask types glorify and celebrate political leadership in order to saveguard the existing power relations. As mediators between opposite realms and as regulating forces, the maskers appear as extensions of the chief’s authorit

Update on the evidence of statin treatment to reduce plaque vulnerability
Ischemic stroke represents one of the major causes of mortality and disability in the adult population in both developed and developing nations. Considering these epidemiological issues, the improvement of preventive therapeutic strategies has been recently targeted as a pivotal research field for scientific community. On the basis of the results from several large clinical studies and met analyses, some Task forces of researchers from recognized Scientific Societies have recently produced some consensus Guide lines, recommending the use of statins as a key pharmacologic approach in stroke prevention. In the present narrative review, we will update evidence on safety and efficacy of statins to prevent ischemic cerebral stroke
