Applicazione della tecnica VISTA nella disinclusione dei canini inclusi: considerazioni chirurgiche
La disinclusione di un canino rappresenta sempre un trattamento ortodontico impegnativo, soprattutto quando il canino permanente incluso è in una posizione alta o in stretta relazione con le radici degli incisivi superiori.
I metodi di trattamento convenzionali solitamente non sono in grado di applicare una corretta direzione della forza, consistente nel movimento contemporaneo in direzione distale e vestibolare della corona del canino, provocando spesso, come effetti collaterali, dovuti ad una trazione rettilinea su un’arcata curvilinea, la presenza di decubito sulle mucose della gengiva, delle labbra e delle guance o un aspetto finale non soddisfacente del supporto parodontale del canino disincluso.
Tra i diversi approcci, la tecnica di accesso tramite tunnel sub-periostale con incisione verticale (VISTA) mostra buone prestazioni per quanto riguarda la direzione delle forze e migliori condizioni parodontali finali del canino erotto; la trazione sottoperiostale è solitamente realizzata tramite una catena elastica collegata ad un dispositivo di ancoraggio temporaneo (TAD) nella zona posteriore.
L’obiettivo del lavoro è presentare un protocollo riproducibile per il trattamento dei canini inclusi quando la metodica tradizionale non è sufficiente.
In questo lavoro viene presentato anche un protocollo diverso per il metodo VISTA, sia a livello chirurgico che a livello ortodontico, a cui ricorrere nei casi di difficile posizionamento della minivite a causa delle condizioni anatomiche o dello stadio della dentatura
Transient cutaneous hypomelanosis after cryoanesthesia
A superficial skin abscess in a young black female patient was incised after cryoanesthesia with tetrafluoroethane spray. A hypopigmented area was found at the first follow-up visit in the treated skin area; complete repigmentation occurred during the next 4 months. There are no other similar reported cases in the literature. Since long duration and possibly permanent hypopigmentation can occur after cryoanesthesia with tetrafluoroethane, especially in black people, this technique should be used with extreme caution
Are there difficulty variables in maxillary third molar surgery? A prospective observational cohort study
BACKGROUND: The main aim of the present study was to assess which factors were related to surgical difficulty in maxillary third molar extraction. Intra-and postoperative complications were also evaluated.METHODS: A prospective observational cohort study was performed on out-patients who underwent impacted maxillary third molar extraction. Principal component analysis and multiple linear regression were used to assess the correlation among variables as well as the influence of different preoperative variables on surgical difficulty, suture duration and risk of intraoperative complications.RESULTS: Eighty-six teeth were extracted. There was a positive correlation between pre-and postoperative evaluations and surgery duration. Available space for surgical access in the upper fornix, second molar contact, crown palatal posi-tion, depth of the elevation point and surgeon's seniority had a significant effect on the overall difficulty index. Risk of in-traoperative complications increased considerably as the percentage of reduction of maximum mouth opening increased (2.03 +/- 0.98) as well as in cases in which ostectomy and tooth sectioning were performed (7.02 +/- 2.68). The surgeon's seniority was associated with a decreased risk of intraoperative complications (-1.52 +/- 0.72). Surgeons were able to predict the difficulty of surgery only to a limited extent.CONCLUSIONS: The percentage of maximum mouth opening reduction with an object in the fornix, crown palatal posi-tion, and contact with the second molar were found to be related to surgical difficulty
Is telephone follow-up really effective in early diagnosis of inflammatory complications after tooth extraction?
To establish whether telephone follow-up is really able to intercept post-extraction complications and to evaluate the degree of patient satisfaction with this kind of post-surgical monitoring. Six hundred and thirty-eight patients were enrolled and randomly assigned to a test or control group. Test group patients were monitored by telephone follow-up 24 and 72 hours after surgery to investigate the presence of local symptoms that are frequently associated with surgical wound infection and inflammation. Both test and control group patients were examined 7 days at suture removal. Patients with systemic diseases, those in which intra-operative accidents occurred during surgery and those for whom extraction suture was not required, were excluded. At least one complication among alveolar osteitis, alveolar inflammation, alveolar infection and dehiscence involved 15.70% of the patients in the test group and 30.70% of the patients in the control group and telephone follow-up proved to be useful in early identification of anomalies in the post-extraction wound healing process. Comparable results were recorded in all extraction subgroups divided according to the type (surgical and non-surgical) and the number (single and multiple) of extractions performed in the same session. Telephone follow-up showed an 8.60 ± 1.17 (0 to 10 score scale) average acceptance. All cases of alveolar osteitis and infection occurred in patients who underwent antibiotic prophylaxis. Telephone follow-up seems to allow early detection of any possible wound healing complications, it is widely accepted by patients and it could therefore be considered a valid method for wound healing monitoring after tooth extractions, due to its effectiveness, feasibility and low costs
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases.
