Palliative treatment: selective arterial embolisation for bone metastases.
Treatment options in patients with bone metastases are mostly aimed at palliation. The goals of treatment in these patients are pain control, prevention and treatment of fractures, maintenance of independence and prevention of tumor progression, and improvement of quality of remaining life [1,2]. Traditional palliative treatments include surgery, if the metastatic lesion is accessible, and/or external beam radiation therapy [3,4]. However, all metastatic lesions are progressive, causing bone failure by producing angiogenic and bone-reabsorbing factors [4]. In addition, most, if not all metastatic lesions are hypervascular. Some lesions such as renal and thyroid metastases are highly hypervascular [5–7]. This may cause technical difficulties with respect to the extent of surgery and primary stability for pain relief [2,8,9]

On the multivariate circulant rational covariance extension problem
Partial stochastic realization of periodic processes from finite covariance data leads to the circulant rational covariance extension problem and bilateral ARMA models. In this paper we present a convex optimization-based theory for this problem that extends and modifies previous results by Carli, Ferrante, Pavon and Picci on the AR solution, which have been successfully applied to image processing of textures. We expect that our present results will provide an enhancement of these procedures.</p

The use of digital videoannotation in teacher training: the teachers’perspectives
The use of digital video offers interesting opportunities in teacher training, particularly the possibilities provided by video annotation, whereby people can add and share comments and opinions on the same videos, even from different places. This exploratory study aims to examine teachers’ perspectives of this technology, taking into account both their explicit and implicit evaluations. Different methods of using video annotation for training are compared, one based on its individual use, another supported by various types of tutorship. The data were collected and analysed first through a quantitative phase, followed by an in-depth qualitative phase. It is pointed out that to make this technology fully operational it is important to address the cultural and psychosocial aspects that control the emotional conditions which arise when one’s teaching behaviour is being observed and assessed
Surgical treatment and results of 62 patients with epithelioid hemangioendothelioma of bone.
OBJECTIVE: Purpose of this retrospective study was (1) to evaluate overall survival and survival to local recurrence of patients with unifocal and multifocal tumor and (2) to evaluate survival to local recurrence after intralesional or wide surgery.
MATERIAL AND METHODS: We reviewed 62 patients with epithelioid hemangioendothelioma of bone, treated from 1985 to 2010. Histological sections and immunohistochemistry were evaluated. Tumor presented as unifocal in 49 patients and as multifocal in 13.
RESULTS: Mean follow-up was 9 years. Five patients (10%) with unifocal tumor progressed to multifocal. Overall survival was 92% at 10 years. Survival with unifocal tumor was significantly higher than survival with multifocal tumor. Survival to local recurrence was 76% at 10 years, significantly higher after wide resection than after intralesional surgery, while there was no statistical difference comparing unifocal and multifocal tumor. At multivariate analysis both variables showed no statistically significance.
CONCLUSION: Wide surgical excision reduces the risk of local recurrence, but functional results and morbidity need to be considered individually when defining surgical indications. Due to the risk of radiation-induced sarcomas, radiation therapy should be reserved to those cases not amenable to wide surgery or when lesions are seated locations difficult to treat. J. Surg. Osteoblastoma of the Sacrum
This book provides an up-to-date overview on the epidemiology, clinical presentation, and imaging characteristics of sacral tumors, discusses the available treatment options, and reports the published outcomes. The diagnostic roles of conventional radiology, CT, and MRI are thoroughly described and imaging appearances are compared with the histologic features. The coverage of therapeutic approaches includes chemotherapy, radiotherapy, radiosurgery, and surgery (partial or total sacrectomy and spinopelvic reconstruction). Special attention is paid to the specific anatomic constraints that make tumors in this region of the spine more difficult to manage effectively than those in the extremities and the mobile portions of the spine.
All components of the sacrum can give rise to benign or malignant tumors, which pose significant diagnostic and therapeutic challenges. Although these tumors are often diagnosed at an advanced stage, good clinical outcomes may be achieved if an aggressive multidisciplinary approach is used. This book will be of value for a range of practitioners; it will assist in prompt diagnosis and help to overcome lack of familiarity with the required treatment strategies
