Recurrent lower respiratory tract infections in children : a practical approach to diagnosis

    No full text
Many children are affected by recurrent lower respiratory tract infections (LRTIs), but the majority of them do not suffer from serious lung or extrapulmonary disease. The challenge for clinicians is to distinguish the recurrent RTIs with self-limiting or minor problems from those with underlying disease. The aim of this review is to describe a practical approach to children with recurrent LRTIs that limits unnecessary, expensive and time-consuming investigations. The children can be divided into three groups on the basis of their personal and family history and clinical findings: 1) otherwise healthy children who do not need further investigations; 2) those with risk factors for respiratory infections for whom a wait-and-see approach can be recommended; and 3) those in whom further investigations are mandatory. However, regardless of the origin of the recurrent LRTIs, it is important to remember that prevention by means of vaccines against respiratory pathogens (i.e. type b Haemophilus influenzae, pertussis, pneumococcal and influenza vaccines) can play a key rol

MHD OBLIQUE STAGNATION-POINT FLOW OF A MICROPOLAR FLUID

    No full text
The steady two-dimensional oblique stagnation-point flow of an electrically conducting micropolar fluid in the presence of a uniform external electromagnetic field (E0,H0) is analyzed and some physical situations are examined. In particular, if E0 vanishes, H0 lies in the plane of the flow, with a direction not parallel to the boundary, and the induced magnetic field is neglected. It is proved that the oblique stagnationpoint flow exists if, and only if, the external magnetic field is parallel to the dividing streamline. In all cases it is shown that the governing nonlinear partial differential equations admit similarity solutions and the resulting ordinary differential problems are solved numerically. Finally, the behaviour of the flow near the boundary is analyzed; this depends on the three dimensionless material parameters, and also on the Hartmann number if H0 is parallel to the dividing streamline

Influenza vaccination in children with cystic fibrosis

    No full text
Cystic fibrosis (CF) is an inherited autosomal recessive disease characterized by progressive pulmonary damage and respiratory failure. It is known that bacterial infections play a critical role in the development of significant lung damage, whereas the role of respiratory viruses in CF pulmonary exacerbations and the relationship between viral infections and the progression of lung damage are uncertain. Health authorities throughout the world recommend influenza vaccination for CF patients. The aim of this review is to analyze the impact of seasonal and pandemic influenza on CF patients and data concerning influenza vaccination in order to assess the current situation and identify areas for future study. As data are limited, further well-constructed clinical studies of the effectiveness of influenza vaccination on the main clinical outcome measures of pulmonary function and nutritional status in patients with CF are require

    Variations on the Author

    No full text
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

Our Landless Patria Marginal Citizenship and Race in Caguas, Puerto Rico, 1880-1910

    No full text
Our Landless Patria examines issues of race and citizenship in Puerto Rico, tracing how the process of land privatization accelerated a series of struggles for natural resources between the poorest sectors of society and the landed elite. The laws of privatization favored the landed elite and barred former slaves and their descendants from obtaining a formal title to a piece of land. In response, people of color developed an alternative citizenship that validated their livelihood, putting in motion a series of civil claims that protected people's mobility rights and their access to land. However, the rural poor's claims for a more egalitarian society, or what Rosa E. Carrasquillo calls "marginal citizenship," could not successfully transform the political exclusion of the racially mixed population because of its heavy borrowing from the Spanish legal system. In particular, marginal citizenship adopted patriarchy as a model to regulate social relations at home, failing to address gender inequalities and perpetuating class differences.Intro -- Contents -- Illustrations -- Acknowledgments -- Introduction: No Ground to Rest -- 1.
