    The influence of red blood cell storage on complement activation

    EINLEITUNG: Lagerschäden in Erythrozytenkonzentraten sind vielfältig und ein limitierender Faktor in der Transfusionsmedizin. Welche Rolle Komplement in diesem Zusammenhang einnimmt, wurde bislang nicht hinreichend untersucht. METHODIK: Erythrozytenkonzentrate wurden 42 Tage gelagert, dabei wöchentlich Proben entnommen und diese durchflusszytometrisch auf CD35, CD55, CD47, CD59 und C3d untersucht. Des Weiteren erfolgte ein direkter Antiglobulintest auf C3d und die Immunglobuline IgA, IgG und IgM. Um den Kontakt des Erythrozytenkonzentrats mit Serum zu Simulieren, wurde eine Transfusionssimulation durchgeführt, wobei Erythrozyten für 30 Minuten bei 37° mit homologem Serum inkubiert wurden. ERGEBNISSE: C3d stieg über die Lagerungszeit an, wobei die Zunahme nach Transfusionsimulation stärker war (ANOVA < 0.0001) als vor Transfusionsimulation (ANOVA p = 0.0103). Im direkten Agglutinations Test wurden diese Beobachtungen bestätigt. Die Abnahme von CD35, CD55 und CD47 über den Zeitraum der Lagerung war signifikant (jeweils ANOVA p < 0.0001), wobei die meisten Veränderungen ab Tag 14 auftraten. CD59 zeigte keinen Abfall, und Immunglobuline wurden nicht nachgewiesen, unabhängig der Transfusionssimulation. ZUSAMMENFASSUNG: Die Transfusion macht Erythrozyten sensibler für spontane Komplementaktivierung. Ursächlich hierfür könnte die zunehmende Anfälligkeit von CD35 gegenüber proteolytischer Spaltung oder Vesikularisierung sein. Die Transfusion von Erythrozytenkonzentraten jenseits einer Lagerungszeit von 14 Tagen könnte in kritisch kranken Patienten durchaus relevant sein.BACKGROUND: Storage lesions in Red Blood Cell Concentrates are manifold and a limiting factor within transfusion medicine. Up to now the role of complement within this context has not sufficiently been researched. STUDY DESIGN AND METHODS: Red Blood Cell Concentrates were stored for 42 days, with samples being retrieved on a weekly basis. Such samples were examined by flow cytometry for CD35, CD55, CD47, CD59 and C3d. In addition a direct Agglutination Test was carried out for C3d and the Immunglobulins IgA, IgG and IgM. To simulate the contact of Red Blood Cell Concentrates with serum, a transfusion was simulated by incubating erythrocytes with homologue serum for 30 minutes at 37 degree Celsius. RESULTS: C3d increased during the storage period. The increase was stronger after the transfusion was simulated (ANOVA < 0.0001) than before the transfusion was simulated (ANOVA p = 0.0103). The direct Agglutination test confirmed these observations. The decline in CD35, CD55 and CD47 during the storage period was significant (in each case ANOVA p < 0.0001), with the largest changes arising after 14 days. CD59 showed no decline and there were no Immunoglobulins detected, irrespective of the transfusion simulation. CONCLUSION: RBCs are becoming increasingly following TS, which might be associated with the increased susceptibility of CD35 to proteolytic cleavage and vesiculation. The use of RBCs stored longer than 14 days might, in fact, be clinically significant in critically ill patients

RNDr. Dana Gášková, CSc., Fyzikální ústav UK Abstrakt: Kandidózové infekce jsou častou příčinou úmrtí hospitalizovaných pacientů. U nejvýznamnějšího kvasinkového patogenu, Candida albicans, je za jeho klinicky významnou rezistenci vůči používaným lékům (azolům) zodpovědná nadprodukce proteinů transportních pump, které léčivé látky odstraňují z buněk, a vzniká tak fenomén mnohačetné lékové rezistence - MDR. Cílem této práce bylo ověřit vhodnost použití kmenů kvasinky Saccharomyces cerevisiae využívajících heterologní expresi MDR pump z patogenní kvasinky Candida albicans, a na tomto systému potvrdit výsledky získané přímo na C. albicans s homologní expresí. Skutečnost, že azolová léčiva mikonazol, bifonazol a ketokonazol, a potenciometrická fluorescenční sonda diS-C3(3), jsou substráty pump CaCdr1p a CaCdr2p, ale nikoliv, nebo minimálně, pumpy CaMdr1p, tedy odpovídají dříve publikovaným výsledkům. The main reason for drug resistance in the most common yeast pathogen Candida albicans is an increased production of transport proteins, which are removing the drug from the cell cytosol and thus producing the phenomenon called multidrug resistance - MDR. The goal of this thesis was to verify the suitability of the yeast strain Saccharomyces cerevisiae with heterologously expressed MDR pumps from the pathogenic yeast Candida albicans and comparison to the results from C. albicans with homologous expression. The results that the azole drugs miconazole, bifonazole, and ketoconazole, together with potentiometric fluorescent probe diS-C3(3), are substrates of the CaCdr1p and CaCdr2p pumps, but not, or minimally, of the pump CaMdr1p, are consistent with the previously published work.     The Correlation Between TP53 Expression and Ki-67 Proliferation with Bartl Malignancy Degree of Plasma Cell Neoplasm

    BACKGROUND: Plasma cell neoplasm (PCN) is a neoplastic plasma cell proliferation which includes solitary bone plasmacytoma (SBP), extramedullary plasmacytoma (EMP) and multiple myeloma (MM). Bartl classifies the degrees of PCN as low, intermediate and high. The aim of this study is to find the correlation between tumor suppressor gene p53 (TP53) expression and Ki-67 proliferation with Bartl Malignancy degree of PCN. Therefore earlier PCN diagnostic method to prevent the development of PCN into MM can be found.METHODS: Thirty-two PCN cases were classified into three groups based on Bartl's degrees of Malignancy. TP53 and Ki-67 immunohistochemical staining were performed on samples and the percentage of positivity was evaluated.RESULTS: The Bartl's low degree of Malignancy was found in 10 MM cases (31.2%), intermediate degree in 5 SBP cases (15.6%) and high in 2 SBP and EMP cases (6.2%). TP53 expression was obtainable at 4% of low, 16% of intermediate and 10% of high degree. There was a significant difference between TP53 expression in low and intermediate degree (p=0.004). Mean proliferation index of Ki-67 was 57% in low, 44.6% in intermediate, and 32.6% in high degree. There was no significant difference of Ki-67 proliferation indexes among the group (p=0.339).CONCLUSION: Increasing expression TP53 was in accord with Bartl's degrees of Malignancy, especially in low and intermediate degree, but there was no significant difference between Ki-67 proliferation index and Bartl's degrees ofmalignancy

Some are responsible for flow of nutrients to the inside of the cell, others for disposing of waste and foreign substances and some for transport of small ions or protons across the membrane. The focus of this work is on the activity of specific transport membrane proteins, so-called MDR pumps, which are responsible for transport of foreign substances or drugs, out of the cell. Using the series of mutant strains of the yeast Saccharomyces cerevisiae (AD1-3, AD1-8 and AD12) differentiated in the presence of specific MDR pumps in their membrane, an influence of various chemical substances on the intracellular concentration of the potentiometric fluorescent probe diS-C3(3), which is actively being transported out of the cell by some of the MDR pumps, was observed. By the examination of the effect of 2-deoxyglucose we proved the active contribution of not only the main MDR pump, Pdr5p, but also of some other pumps, in lowering the intracellular probe concentration.
