    Induction therapy before surgery for non-small cell lung cancer

    Surgery alone is currently still accepted as the principal therapy for cure for patients with localized non-small cell lung cancer. The optimal therapy in locally advanced and unresectable stage III disease remains unclear. The limited performance of each single therapeutic strategy (surgery, radiotherapy, or chemotherapy) in the treatment of locally advanced non-small cell lung cancer accounted for the rationale of the many attempts at improvement by integrating the different approaches. In recent years, to improve clinical outcome, chemotherapy or chemoradiation followed by surgery, and definitive chemoradiation have commonly been used. Despite numerous phase-II trials, little evidence from randomized phase-III trials has been generated. The ongoing randomized trials will probably provide more reliable indications to define the management of the large number of patients with locally advanced disease

    Psychometric properties of the Spanish version of the Pediatric Quality of Life Inventory Family Impact Module (PedsQL FIM)

    Introduction: This study analysed the psychometric properties of the Spanish version of the Pediatric Quality of Life Questionnaire Family Impact Module (PedsQL FIM) in the Argentinian population. Patients and methods: The sample included 232 caregivers, of who 108 were parents of children with chronic diseases (mean, 9.54; standard deviation [SD], 4.43) and 124 parents of children in the general population (mean, 12.37; SD, 4.6). Results: We assessed the validity of the instrument with the known-groups method, finding significant differences between the case and control groups in the overall and subscale scores (P < .01). We also assessed test validity by means of exploratory factor analysis, which yielded an 8-factor model that explained 74.03% of the variance. We assessed reliability with the Cronbach alpha and found a high internal consistency (α=.95). Conclusion: The PedsQL module proved to be a valid and reliable tool to assess the impact of a chronic paediatric condition on caregiver quality of life and family functioning.Fil: Ortega, Javiera.     Hegel’s Philosophy of Right (PhR) can be analyzed whether independently from the Science of Logic or on the basis of the metaphysical premises developed in the latter work. In the present paper we enumerate different positions about this question, and we classify a number of attempts which were recently carried out in order to actualize the PhR. the differentiation of five levels of analysis of the articulation of the ethical and the logical in the PhR allows us to conclude that both the thesis on the dependence and the thesis of the independence can be compatible, since they refer to different levels of analysis.La Filosofía del Derecho (PhR) de Hegel puede ser analizada, o bien con independencia de la Ciencia de la Lógica, o bien en base a las premisas metafísicas desarrolladas en esta última obra. en el presente artículo se enumeran las diferentes posiciones sobre esta cuestión y se agrupan los diversos intentos que en los últimos a.os se han llevado a cabo para actualizar la PhR. La distinción de cinco niveles de análisis de la articulación entre lo ético y lo lógico en la PhR nos permite concluir que tanto la tesis de la dependencia como la tesis de la independencia pueden ser compatibles entre sí dado que refieren a niveles distintos de análisis

    Early prediction of resistance to tyrosine kinase inhibitors by plasma monitoring of EGFR mutations in NSCLC: A new algorithm for patient selection and personalized treatment

    In Non-Small-Cell Lung Cancer (NSCLC) patients treated with Tyrosine Kinase-Inhibitors (TKIs) therapy, the emergence of acquired resistance can be investigated by plasma monitoring of circulating tumor DNA (ctDNA). A series of 116 patients with EGFR-positive lung adenocarcinomas were treated with first/second generation EGFR TKIs. At clinical progression, 64 (55%) EGFR T790M plasma positive patients were subjected to second line-treatment with osimertinib and strictly monitored during the first month of therapy. Plasma analysis by the EGFR Cobas test showed in 57 (89%) cases a substantial decrease in the levels of the sensitizing EGFR mutant allele (sEGFRma), down to a not detectable value. These patients were defined as plasmatic good responders (PGR). In 7 (11%) patients, the sEGFRma did not drop to zero (plasmatic poor responders, PPR). In these latter cases, Massive Parallel Sequencing (MPS) analysis at the end of the first month and at clinical progression showed the presence of resistant-inducing mutations, including MET and HER2 gene amplification, KRAS and PIK3CA gene mutations. PPR showed disease progression in 5 (71%) cases, stable disease in 2 (29%) cases, and a shorter median Progression-free survival (PFS) (4.3 ± 1.1 months) than that observed in PGR (13.3 ± 1.2 months) (P < 0.0001). Our data indicate that plasma monitoring by a simple RT-PCR-based EGFR mutation test in the first month of treatment may be useful for a rapid identification of patients to be subjected to further characterization by MPS. A diagnostic algorithm for an early detection of resistance-inducing mutations and patient management is reported

Libro electrónicoEn los últimos tiempos se ha desarrollado una nueva línea de investigación dentro del campo de la ética, a saber, el análisis de la relación entre ética –o bio-ética– y biopolítica como perspectiva desde la cual reflexionar sobre conflictos morales en los que se percibe la fuerte impronta de la “administración de la vida”, según los términos propuestos por Michel Foucault en su Historia de la Sexualidad I. Así, cuestiones que van desde la eugenesia hasta la eutanasia, pueden ser enfocadas de modo tal que se revelen las condiciones sociales y políticas en las que se enmarcan estos conflictos de la ética aplicada. De igual manera, los problemas sociales y políticos de la actualidad pueden ser analizados desde una perspectiva bioética, desde el punto de vista de los problemas morales que desencadenan. Todo parece indicar que esta colaboración entre los análisis biopolítico y bioético será fructífera en la medida en que sean cuidadosamente delimitados los ámbitos de competencia de cada uno de ellos pero sin desalentar, a la vez, su interacción. Se trata, de acuerdo con nuestra propuesta, de efectuar una lectura biopolítica de las discusiones bioéticas y de una lectura en clave bioética de las intervenciones del poder biopolítico, de modo tal que estas lecturas pongan en evidencia, en el primer caso, la dimensión social y política de los problemas bioéticos y, en el segundo caso, el impacto en los cuerpos de un biopoder impersonal y difuso por definición.1a ed

    Intervento psicoeducazionale di gruppo per pazienti prostatectomizzati: uno studio pilota in Italia

    Sono stati condotti tre focus groups (FGs) con 16 pazienti prostatectomizzati di tre Centri Oncologici. La maggior parte dei pazienti ha espresso il bisogno di più informazioni, soprattutto sugli effetti collaterali dei trattamenti, la necessità di un approccio multidisciplinare nel percorso di cura, di essere trattati da un punto di vista olistico e l’importanza dell’assistenza post-chirurgia anche con un supporto psicologico. Sulla base di quanto emerso dai FGs, sono stati condotti tre interventi psicoeducazionali di gruppo suddivisi in 3 incontri con 16 pazienti (tasso di aderenza 95.8%). Ricevere informazioni mediche e condividere la propria esperienza di malattia con altri pazienti, sono i principali benefici citati dai pazienti al termine dell’intervento. I gruppi psicoeducazionali sono interventi a basso costo, facili da organizzare e ben accettati dai pazienti prostatectomizzati. Sarebbe auspicabile che fossero effettuati all’interno delle Prostate Cancer Units con cadenza regolare per soddisfare i bisogni informativi e psicologici dei pazienti

    Resumen: Introducción: Este trabajo analiza las propiedades psicométricas de la versión en castellano del Cuestionario de calidad de vida pediátrica módulo de impacto familiar (PedsQL FIM) en población argentina. Pacientes y métodos: Se obtuvo una muestra de 232 cuidadores, 108 de ni˜nos con enfermedades crónicas (M = 9,54, DE = 4,43) y 124 de ni˜nos de población general (M = 12,37, DE = 4,6). Resultados: La validez del instrumento se estudió a través del método de grupos contrastados, encontrando diferencias significativas en la escala total y subdimensiones de la escala (p < 0,01). A su vez, se realizó un análisis factorial exploratorio en el que se encontró un modelo de 8 factores explicando el 74,02% de la varianza total. La confiabilidad fue estudiada a través del coeficiente alfa de Cronbach y se encontró un valor alto de consistencia interna = 0,95. Conclusiones: El instrumento PedsQL demostró ser una herramienta válida y confiable para estudiar el impacto que tiene una condición pediátrica crónica a nivel de la calidad de vida del cuidador y del funcionamiento familiar

    Disease Characteristics and Psychiatric Comorbidities in Adolescents with Anorexia Nervosa Hospitalized During COVID-19 Pandemic

    Background: Since the beginning of COVID-19 pandemic, an increase in new diagnoses and pediatric hospital admission for anorexia nervosa (AN) or atypical AN in adolescents have been reported, suggesting an adverse effect of COVID-19 on youth mental health. We hypothesized possible differences in prevalence of hospitalization and/or disease severity, related to socio-economic status and/or ethnicity. Methods: Retrospectively, patients were divided into two subgroups and compared according to the date of first hospital admission: “pre SARS-COV2 era” group (n. 45, 8th March 2016–8th March 2020) and “SARS-COV2 era” group (n. 43, 9th March 2020–8th March 2022). Results: During the two years of the SARS-COV2 era, we reported an increase in hospital admission incidence more than doubled respect to the “pre-SARS-COV2 era”. The “SARS-COV2 era group” showed a more rapid weight loss (p = 0.005), a minor duration of weight loss from lifetime maximum to admission (p = 0.019) and needed most frequently treatments with intravenous fluids (p < 0.0001), oral dietetic supplements (p <0.001) and enteral nutrition by nasogastric tube (p = 0.002). The same group presented higher prevalence of psychiatric comorbidities (63% vs. 22%, p < 0.0001) and required most frequently treatments with psychotropic drugs (56% vs. 24%, p = 0.002). We found higher family socioeconomic status (SES) in our patients with AN in both the periods and we did not find a shift in social class distribution over time. Conclusions: Our study confirms a significant increase in incidence of hospitalization and of psychiatric comorbidity in the pediatric population with AN during the second year of COVID-19 pandemic, regardless of SES or ethnic background. Further studies are needed to understand potential mechanisms that during COVID-19 pandemic trigger eating disorder symptoms
