    From satisfaction to expectation: The patient's perspective in lower limb prosthetic care

    Neck pain is a common musculoskeletal complaint and a relationship with reduced work-related functional capacity is assumed. A validated instrument to test functional capacity of patients with neck pain is unavailable. The objective of this study was to develop a Functional Capacity Evaluation (FCE), which is content valid for determining functional capacity in patients with work related neck disorders (WRND). A review of epidemiological review literature was conducted to identify physical risk factors for WRND. Evidence was found that physical risk factors contribute in development of WRND. Physical risk factors were related to repetitive movements, forceful movements, awkward positions and static contractions of the neck or the neck/shoulder region. An FCE was designed based on the risk factors identified. Eight tests were selected to cover all risk factors: repetitive side reaching, repetitive reaching overhead, static overhead work, front carry, forward static bend neck, overhead lift and the neck strength test. Content validity of this FCE was established by providing the rationale, specific objectives and operational definitions of the FCE. Further research is needed to establish reliability and other aspects of validity of the neck-FCE Aim Worldwide, family- centred and co- ordinated care are seen as the two most desirable and effective methods of paediatric care delivery. This study outlines current views on how team collaboration comprising professionals in paediatric rehabilitation and special education and the parents of children with disabilities should be organized, and analyses the policies of five paediatric rehabilitation settings associated with the care of 44 children with cerebral palsy ( CP) in the Netherlands. Methods For an overview of current ideas on collaboration, written statements of professional associations in Dutch paediatric rehabilitation were examined. The policy statements of the five participating settings were derived from their institutional files. Documents detailing the collaborative arrangements involving the various professionals and parents were evaluated at the institutional level and at the child level. Involvement of the stakeholders was analysed based on team conferences. Results Also in the Netherlands collaboration between rehabilitation and education professionals and parents is endorsed as the key principle in paediatric rehabilitation, with at its core the team conference in which the various priorities and goals are formulated and integrated into a personalized treatment plan. As to their collaborative approaches between rehabilitation centre and school, the five paediatric settings rarely differed, but at the child level approaches varied. Teams were large ( averaging 10.5 members), and all three stakeholder groups were represented, but involvement differed per setting, as did the roles and contributions of the individual team members. Conclusion Collaboration between rehabilitation and education professionals and parents is supported and encouraged nationwide. Views on collaboration have been formulated, and general guidelines on family- centred and co- ordinated care are available. Yet, collaborative practices in Dutch paediatric care are still developing. Protocols that carefully delineate the commitments to collaborate and that translate the policies into practical, detailed guidelines are needed, as they are a prerequisite for successful teamwork

    Haptic exploration improves performance of a laparoscopic training task

    Background: Laparoscopy has reduced tactile and visual feedback compared to open surgery. There is increasing evidence that visual and haptic information converge to form a more robust mental representation of an object. We investigated whether tactile exploration of an object prior to executing a laparoscopic action on it improves performance. Methods: A prospective cohort study with 20 medical students randomized in two different groups was conducted. A silicone ileocecal model, on which a laparoscopic action had to be performed, was used inside an outside a ForceSense box trainer. During the pre-test, students either did a combined manual and visual exploration or only visual exploration of the caecum model. To track performance during the trials of the study we used force, motion and time parameters as representatives of technical skills development. The final trial data were used for statistical comparison between groups. Results: All included time and motion parameters did not show any clear differences between groups. However, the force parameters Mean force non-zero (p = 004), Maximal force (p = 0.01) Maximal impulse (p = 0.02), Force volume (p = 0.02) and SD force (p = 0.01) showed significant lower values in favour of the tactile exploration group for the final trials. Conclusions: By adding haptic sensation to the existing visual information during training of laparoscopic tasks on life-like models, tissue manipulation skills improve during training.Medical Instruments & Bio-Inspired Technolog

    Synthesis of Some Biologically Relevant β-<i>C</i>-Glycoconjugates

    An esterification−RCM approach to a variety of biologically relevant β-C-glycoconjugates is reported herein. A range of carboxylic acids were coupled with several different olefin alcohols 1 to provide esters 3. The esters were then converted to the final ring-closed product 6 in three steps in 49−60% overall yield. The formed compounds are biologically relevant and serve as stable carbohydrate mimics of the corresponding O-glycosides

    Investigating the significance of multiple scattering in ultrasound contrast agent particle populations

    The majority of the existing models describing the behavior of microbubble ultrasound contrast agents consider single, isolated microbubbles suspended in infinite media. The behavior of a microbubble population is predicted by summing the results for single microbubbles and ignoring multiple scattering effects. The aim of this investigation is to determine the significance of multiple scattering in microbubble populations and establish whether an alternative approach is required. In the first part of the work, linear models are derived to identify approximately the conditions under which multiple scattering may be expected. A nonlinear model for sound propagation in a microbubble suspension then is developed and used to examine multiple scattering at higher insonation pressures. Broadband attenuation measurements are described for two different types of microbubble suspension (albumin encapsulated octofluropropane and copolymer encapsulated isobutane) to ascertain whether or not multiple scattering may be observed experimentally. The results from the simulation work indicate that multiple scattering effects would be discernible at moderate concentrations (106 microbubbles/ml) such as may be present in vivo. The effect upon attenuation in the suspension would be pronounced, however, only if the population contained a sufficient proportion of relatively large (> 4 mu m radius) microbubbles excited at their resonance frequency. This also is found to be the case experimentally. These findings may have important implications for the characterization of ultrasound contrast agents and their use in quantitative diagnostic techniques

    Static and dynamic angles of repose in loose granular materials under reduced gravity

    Granular materials avalanche when a static angle of repose is exceeded and freeze at a dynamic angle of repose. Such avalanches occur subaerially on steep hillslopes and wind dunes and subaqueously at the lee side of deltas. Until now it has been assumed that the angles of repose are independent of gravitational acceleration. The objective of this work is to experimentally determine whether the angles of repose depend on gravity. In 33 parabolic flights in a well-controlled research aircraft we recorded avalanching granular materials in rotating drums at effective gravitational accelerations of 0.1, 0.38 and 1.0 times the terrestrial value. The granular materials varied in particle size and rounding and had air or water as interstitial fluid. Materials with angular grains had time-averaged angles of about 40° and with rounded grains about 25° for all effective gravitational accelerations, except the finest glass beads in air, which was explained by static electricity. For all materials, the static angle of repose increases about 5° with reduced gravity, whereas the dynamic angle decreases with about 10°. Consequently, the avalanche size increases with reduced gravity. The experimental results suggest that relatively low slopes of granular material on Mars may have formed by dry flows without a lubricating fluid. On asteroids even lower slopes are expected. The dependence on gravity of angle of repose may require reanalysis of models for many phenomena involving sediment, also at much lower slope angles.Control & OperationsAerospace Engineerin

    The relevance of reducing Veress needle overshooting

    Safe insertion of the Veress needle during laparoscopy relies on the surgeons’ technical skills in order to stop needle insertion just in time to prevent overshooting in the underlying organs. To reduce this risk, a wide variety of Veress needle systems were developed with safety mechanisms that limit the insertion speed, insertion depth or decouple the driving force generated by the surgeon’s hand on the needle. The aim of this study is to evaluate current surgeons’ perceptions related to the use of Veress needles and to investigate the relevance of preventing overshooting of Veress needles among members of the European Association of Endoscopic Surgery (EAES). An online survey was distributed by the EAES Executive Office to all active members. The survey consisted of demographic data and 14 questions regarding the use of the Veress needle, the training conducted prior to usage, and the need for any improvement. A total of 365 members residing in 58 different countries responded the survey. Of the responding surgeons, 36% prefer the open method for patients with normal body mass index (BMI), and 22% for patients with high BMI. Of the surgeons using Veress needle, 68% indicated that the reduction of overshoot is beneficial in normal BMI patients, whereas 78% indicated that this is beneficial in high BMI patients. On average, the members using the Veress needle had used it for 1448 (SD 3031) times and felt comfortable on using it after 22,9 (SD 78,9) times. The average years of experience was 17,6 (SD 11,1) and the surgeons think that a maximum overshoot of 9.4 (SD 5.5) mm is acceptable before they can safely use the Veress needle. This survey indicates that despite the risks, Veress needles are still being used by the majority of the laparoscopic surgeons who responded. In addition, the surgeons responded that they were interested in using a Veress needle with an extra safety mechanism if it limits the risk of overshooting into the underlying structures.Medical Instruments & Bio-Inspired TechnologyBiomechanical Engineerin

    A large spherical HPD for a novel deep-sea neutrino experiment

    An underwater neutrino experiment has been proposed which provides precise measurements of the neutrino mixing parameters &#952;23 and and permits an increase of sensitivity for the small angle &#952;13 by more than one order of magnitude. A Cherenkov detector of about 1.5 Mt active mass, deployed in the Gulf of Taranto, utilizes the CNGS beam in off-axis configuration which represents an essentially mono-energetic source of muon neutrinos. A unique feature of the experiment is the possibility to move the detector and therefore exploit different baselines around 1200 km where the oscillation pattern is fully developed. The conceptual detector design consists of O(30,000) large area and acceptance photosensors arranged in a matrix of 300×300 m2 size. Hybrid photon detectors are considered as promising candidates as they provide clean signal characteristics and uniform collection efficiency. We discuss the design and expected performance of a large spherical HPD with 380 mm diameter, which is housed in a high-pressure glass container. A scaled prototype HPD of 208 mm diameter is currently under development using the existing CERN HPD facility
