Evans Syndrome as the Initial Presentation of Systemic Lupus Erythromatosus in a Rural Kenyan Hospital: A Case Report
No full textAbstract: Evans syndrome is a rare autoimmune disorder characterized by the occurrence of autoimmune hemolytic anemia and immune thrombocytopenic purpura with or without immune neutropenia. The syndrome may be idiopathic, but in typical clinical settings, secondary causes, predominantly autoimmune diseases, may be the underlying etiology. We present a case of lupus presenting initially as Evans syndrome.

No full textThis study goes through the analysis about the constitution of video art both as an artistic-technological supportand as a renewal movement in the field of art. Starting from its origin naming the author explores the experimental and questioning character of video art as specific trend, specially related to dominant opinions and technologies. The study analyses the concept and esthetics sides of video art and its similarities and differences with otherarts, supports and formats. The text goes not only through historic aspects of curator video art but through itsspecific present features and challenges as well. In addition, video art situation, referents and works in Argentinaare fully pointed out.Este artículo se propone señalar aspectos históricos y actuales sobre la creación musical con medios electroacústicos. Asimismo, se desarrollan ideas vinculadas a distinguir entre el uso de la tecnología como concepto lineal en contraposición a una práctica de creación experimental. Se describen ejemplos y autores que promueven los usos creativos de los medios electrónicos y en particular relacionados con estrategias de interactividad. Se han procurado describir experiencias de arte interactivo en su perspectiva histórica y se ha puntualizado sobre la limitación o falacia de considerar a los medios tecnológicos como un fin en sí mismos.Este artigo propõe sinalar aspectos históricos e atuais sobre a criação musical com médios eletroacústicos. Acoustical properties of compressed earth blocks: Effect of compaction pressure, water hyacinth ash and lime
No full textInternational audienceThis study assessed the potential use of a mixture of lime and water hyacinth ash (WHA) as binders in fabrication of cylindrical compressed earth blocks (CEBs) with good acoustic absorption properties for building and construction. Different concentrations of the binders and compaction pressures were employed so as to vary the acoustical properties of the fabricated blocks. The geometric and transport parameters of their porous microstructure were recovered through probing using acoustic waves. A low-frequency acoustic wave guide was built for this purpose. It was found out that the transmission coefficient decreased with the compaction pressure, and with addition of lime, while WHA increased the transmission coefficient. The non-acoustical parameters recovered using the equivalent fluid model (JCAL) showed that the variation of the geometry of the microstructure of the blocks is what influences the acoustic transmission coefficient. Thus, the properties of the CEBs can be steered using binder concentration and compaction pressure in a controlled manner
Youth engagement in livestock production and marketing in East Africa
No full textThere is limited empirical evidence about how youth engage in livestock production in East Africa. The primary objective of this article is to better understand youth engagement in the livestock sector, namely commercialization and marketing. We compare youth to non-youth in four areas: livestock asset ownership, access to and use of services and improved practices and lastly, market participation. Our framework draws upon agricultural transformation concepts to suggest a tentative pathway through which livestock can support youth in transitions. We draw upon quantitative data and analyze and compare youth to nonyouth in intensive and extensive livestock systems in Kenya, Uganda, Ethiopia, and Tanzania and investigate four livestock systems, dairy, extensive ruminants, pig, and chicken systems. The results showed that there were no systematic youth-specific trends across the asset ownership, levels of commercialization or market participation. Also, low asset ownership did not generate observable youth specific differences in market participation. Rather, education and improved practices correlate with higher levels of market participation. Exploring how livestock are acquired and implications upon asset-based approaches is recommended to better understand the potential of livestock as a source of income during youth transitions in different livestock systems. More evidence is needed to inform and tailor policy approaches to support equitable opportunities in the livestock sector better. A scoping review of qualitative studies on sexual and reproductive health and rights in Uganda: Exploring factors at multiple levels
No full textBackground: Uganda is burdened by high unintended and teen pregnancies, high sexually transmitted infections, and harm caused by unsafe abortion. Objectives: Explore factors influencing sexual and reproductive health and rights (SRHR) in Uganda by synthesizing evidence from qualitative studies using a scoping review. Eligibility Criteria: Original qualitative peer-reviewed research studies published between 2002 and 2023 in any language exploring factors influencing SRHR in Uganda. Sources of Evidence: Eight databases searched using qualitative/mixed methods search filters and no language limits. Charting Methods: Information extracted included author, article title, publication year, study aims, participant description, data collection type, sample size, main findings, factors at the individual, interpersonal, community, and policy levels, implications for SRHR in Uganda, and study limitations. Quality of the selected articles was assessed using the Critical Appraisal Skills Programme tool. Results: One hundred seventy-three studies met inclusion criteria. At the individual level, knowledge and attitudes toward SRHR, risky sexual behavior, and access to maternal SRHR services were identified as critical factors influencing health outcomes. Interpersonal factors included communication with sexual partners and relationships with family, school, and community members. Healthcare organization factors included adolescent access to education, SRHR services, and HIV prevention. Cultural and social factors included gendered norms and male involvement in SRHR. Policy-level factors included the importance of aligning policy and practice. Conclusions: Multiple factors at individual, interpersonal, community, healthcare, cultural, and policy levels were found to influence SRHR in Uganda. The findings suggest that interventions targeting multiple levels of the socio-ecological system may be necessary to improve SRHR outcomes. This review highlights the need for a holistic approach that considers the broader socio-ecological context. Reducing identified gaps in the literature, particularly between policy and practice related to SRHR, is urgently needed in Uganda. We hope this review will inform the development of policies and interventions to improve SRHR outcomes
Janosch e a “arte de viver”
No full textFreudian “art of living” is applied here to study German children’s literature author Janosch. The purpose is mainly focused studying this writer from an analytic perspective in Civilisation and Its Discontents by Sigmund Freud. It is selected a corpus of the Germanauthor, where previously is study the concepts of pleasure and displeasure in Freud work. Thus, it is suggested how Janosch works propose a different choice to overcome the discontents against the traditional libidinal economy. The analysis shows how the charactersbuild their own strategies, which allow to speak regarding the “art of living” as an intransitive desire; that jeopardizes traditional logic, a revaluation of the other from solidarity and aradical option for pleasure over culture. The results display a renewed and complex vision of an author of children’s literature. The creation by Janosch questions the values of thecontemporary society and appeals to the reader to review its hierarchies.Situamos la obra del escritor infantil alemán Janosch dentro de la corriente del “arte de vivir”. Este busca la plena realización del sujeto desde un planteamiento personal y crítico. El propósito central de este artículo será determinar la particularidad de Janosch abriendo un diálogo desde laproblematización de Freud en El malestar en la cultura. Para el análisis optamos por una revisión de un corpus escogido de Janosch. Previa síntesis de la problemática del placer y el displacer según Freud, se determinan cuáles son las principales salidas de Janosch al malestar y cómo se diferencia de una economía libidinal tradicional. El análisis muestra cómo los personajes construyen sus propias estrategias, lo que permite hablar de un “arte de vivir”. Estas son: un “deseo intransitivo” que pone en jaque la lógica tradicional, una revaloración del otro desde la solidaridad y una radical opción por el placer por sobre la cultura. Los resultados de la reflexión permiten una visión renovada y compleja de un autor de literatura infantil. El mundo creado por Janosch cuestiona desde una inusitada frescura los valores de nuestra sociedad contemporánea y apela al lector a una revisión de sus jerarquías.Classifica-se a obra do escritor infantil alemão Janosch dentro da corrente da “arte de viver”. Essa procura a total realização do sujeito a partir de um posicionamento pessoal e crítico. O propósito central deste artigo será determinar a particularidade de Janoschabrindo um diálogo a partir da problematização de Freud em O mal-estar na cultura. Para a análise decidiu-se por uma revisão de um corpus escolhido de Janosch. Prévia síntese da problemática do prazer o do desprazer segundo Freud, determinam-se quais são as principais saídas de Janosch ao mal-estar e como se diferencia de uma economia libidinal tradicional.A análise expõe como as personagens constroem as suas próprias estratégias, o que permite falar de uma “arte de viver”. Essas são: um “desejo intransitivo”, que põe em xeque a lógicatradicional, uma revalorização do outro por meio da solidariedade e uma radical opção pelo prazer em detrimento da cultura. Os resultados da reflexão possibilitam uma visão renovadae complexa de um autor da literatura infantil.
