    This video, titled Elon Campaign Victory News2 coverage, was an off-air recording in 1994. The North Carolina State Advisory Budget Commission met at Elon College to discuss the 1995 budget. The state’s independent colleges wanted to increase state tuition grants to students who attend private colleges. The video piece was produced by Dave Wright of local News2 (WFMY), Greensboro, North Carolina. In the video piece, J. Fred Young, President of Elon College, showed Elon’s campus to the North Carolina Budget Commissioners. In addition, he boasted that Elon College has enrolled the largest and most academically-talented student body in Elon’s history. The new Moseley Center was designed for Elon’s student organizations. Elon College junior, John Keegan, was interviewed for the news piece. Keegan spoke of how students enjoy the fireplace in Moseley Center. He also spoke of the reasons why he enrolled in Elon College.     The Role of NEWS2

    Background: We investigated the parameters of National Early Warning Score 2 (NEWS2) + lactate + D-dimer in predicting the intensive care unit (ICU) admission and in-hospital mortality in patients hospitalized with COVID-19. Methods: Patients, who applied to the emergency department of a tertiary university hospital and were taken to the COVID-19 zone with suspected COVID-19 between March 2020 and June 2020, were retrospectively examined. In this study, 244 patients, who were hospitalized and had positive polymerase chain reaction test results, were included. NEWS2, lactate, and D-dimer levels of the patients were recorded. Patients were grouped by the states of in-hospital mortality and ICU admission. Results: Of 244 patients who were included in the study, 122 (50%) were male, while their mean age was 53.76 +/- 17.36 years. 28 (11.5%) patients were admitted to the ICU, while in-hospital mortality was seen in 14 (5.7%) patients. The levels of D-dimer, NEWS2, NEWS2 + lactate, NEWS2 + D-dimer, NEWS2 + lactate + D-dimer were statistically signifi cantly higher in patients with in-hospital mortality and admitted to ICU (p < 0.05). The area under the curve (AUC) values of D-dimer, lactate, NEWS2, NEWS2 + lactate, NEWS2 + D-dimer, NEWS2 + lactate + D-dimer in predicting ICU admission were as 0.745 (0.658-0.832), 0.589 (0.469-0.710), 0.760 (0.675-0.845), 0.774 (0.690-0.859), 0.776 (0.692-0.860), and 0.778 (0.694-0.862), respectively; while the AUC values of these parameters in predicting in-hospital mortality were found to be as 0.768 (0.671-0.865), 0.695 (0.563-0.827), 0.735 (0.634-0.836), 0.757 (0.647-0.867), 0.752 (0.656-0.848), and 0.764 (0.655-0.873), respectively. Conclusions: Compared to using the NEWS2 value alone, a combination of NEWS2, lactate, and D-dimer was found to be more valuable in predicting in-hospital mortality and ICU admission

Det har tidigare gjorts studier där patientsäkerheten har setts öka med hjälp av NEWS2. Det framkommer dock inte tydligt i tidigare forskning hur vårdpersonalen ställer sig till bedömningsskalan. Därför har vi valt att undersöka sjuksköterskans perspektiv på användningen av NEWS2.  Syfte: Att undersöka sjuksköterskans perspektiv på att använda bedömningsskalan NEWS2 på vårdavdelning.  Metod: En kvantitativ enkätstudie där urvalet bestod av 124 legitimerade sjuksköterskor från fem olika vårdavdelningar på länssjukhuset i Kalmar.  Resultat: Totalt 59 sjuksköterskor (svarsfrekvens 48%) besvarade enkäten. NEWS2 tillför trygghet i sjuksköterskans bedömning av patienten, som ett komplement till den kliniska blicken. Det finns en komplexitet i hur riktlinjerna för NEWS2 följs relaterat till de patienter som har ett högt NEWS2-värde i sitt habitualtillstånd. Det framkommer en signifikant skillnad i hur sjuksköterskor med längre yrkeserfarenhet (&gt;10 år) mäter de vitala parametrar som ingår i NEWS2 jämfört med de som arbetat i mindre än 10 år.  Slutsats: NEWS2 bidrar till ökad patientsäkerhet ur ett sjuksköterskeperspektiv. Det finns dock förbättringspotential gällande hur sjuksköterskan använder sig av åtgärdsskalan i NEWS2. Riktlinjerna borde tydliggöras eller anpassas för att NEWS2 ska fungera optimalt på alla vårdavdelningar.

    NEWS2 and the older person

    The National Early Warning Score (NEWS) (published in 2012) made no specific adjustments for older people. The updated NEWS2 (2017) incorporated new confusion as a category for consciousness. In this article we consider the role of NEWS2 in detection of acute clinical deterioration in older people and how the score may be used to inform care, highlighting the additional aspects such as care escalation decisions that may ensue. We consider the evidence of NEWS and NEWS2 in assessment of the older person in different settings, including the potential benefits and limitations for care home residents. We suggest that NEWS2 may need adaptation for older people in future iterations, and that it should be used in conjunction with other clinical assessments such as the clinical frailty scale and the 4’A’s test (4AT) for delirium

    Early administration of antibiotics is associated with better survival in sepsis, thus screening and early detection for sepsis is of clinical importance. Current risk stratification scores used for bedside detection of sepsis, for example Quick Sequential Organ Failure Assessment (qSOFA) and National Early Warning Score 2 (NEWS2), are primarily validated for death and intensive care. The primary aim of this study was to compare the diagnostic accuracy of qSOFA and NEWS2 for a composite outcome of sepsis with organ dysfunction, infection-related mortality within <72 h, or intensive care due to an infection. Retrospective analysis of data from two prospective, observational, multicentre, convenience trials of sepsis biomarkers at emergency departments were performed. Cohort A consisted of 526 patients with a diagnosed infection, 288 with the composite outcome. Cohort B consisted of 645 patients, of whom 269 had a diagnosed infection and 191 experienced the composite outcome. In Cohort A and B, NEWS2 had significantly higher area under receiver operating characteristic curve (AUC), 0.80 (95% CI 0.75-0.83) and 0.70 (95% CI 0.65-0.74), than qSOFA, AUC 0.70 (95% CI 0.66-0.75) and 0.62 (95% CI 0.57-0.67) p < 0.01 and, p = 0.02, respectively for the composite outcome. NEWS2 was superior to qSOFA for screening for sepsis with organ dysfunction, infection-related mortality or intensive care due to an infection both among infected patients and among undifferentiated patients at emergency departments

    Background: In January 2019, National Early Warning Score 2 (NEWS2) was introduced at Sahlgrenska University Hospital. This is a tool for assessing vital parameters to improve detection of, and response to, clinical deterioration in patients with acute illness. There is a lack of studies and literature that describe how nurses perceive NEWS2. Aim: To investigate what registered ward nurses know and think about the NEWS2 instrument. Is education given according to NEWS2? How is the instrument NEWS2 used in the clinical work in the wards? What are the possibilities or challenges nurses experience when using NEWS2? Method: Quantitative, non-experimental, cross-sectional study. Development of questionnaire with pre- test. Data collection with a web-based survey to registered nurses in wards at a hospital in the Region of Västra Götaland (VGR), Sweden. Results: A total of 171 nurses were invited to participate, 59 completed the survey (34.5%). Nurses were familiar with NEWS2 and found it to be a good instrument for assessment of vital parameters. The instrument provided structure, safe procedures and guidance. NEWS2 assessments were carried out by the assistant nurses. The nurses assessed the results and made decisions for the follow up of the patient. The nurse had at the time for the assessment rarely seen the patient. NEWS2 supported communication within the team. NEWS2 provided support for the nurse in contact with doctors and Medical Emergency Team (MET). Conclusion: Nurses found NEWS2 to be a good tool that helped them in their daily work with the patients. NEWS2 is used because it is structured, and evidence based. NEWS2 assessments were carried out by the assistant nurses. The nurses assessed the results. NEWS2 gave the nurses structure when assessing patients and facilitated the task of prioritizing care for the patients. Pre-test of questionnaire showed good qualities in relation to strength in the construction of the questions. Developing a questionnaire is a complex process. The questions had consensus. Resultat: Av totalt 171 tillfrågade sjuksköterskor besvarades enkäten av 59 sjuksköterskor (34,5%). Sjuksköterskor var insatta i NEWS2 och uppfattade att det är ett bra bedömningsinstrument som ger överblick, trygghet och prioriteringsmöjligheter. NEWS2 kontroller utfördes av undersköterskan, och sjuksköterskan bedömde resultat och upprättade en plan för åtgärder. Sjuksköterskan hade ofta inte sett patienten vid mättillfället. I teamarbetet var NEWS2 ett kommunikationsstöd. Vid kontakt med patientansvarig läkare och Mobil Intensivvårdsgrupp gav NEWS2 ett strukturerat stöd. Slutsats: Sjuksköterskor ansåg att NEWS2 var ett bra mätinstrument som hjälper dem i det dagliga arbetet. NEWS2 användes i enlighet med evidensbaserad vård. Undersköterskan genomförde NEWS2 kontroller och sjuksköterskan bedömde resultatet. NEWS2 gav sjuksköterskan översikt, överblick, trygghet och prioriteringsförmåga. Utveckling av frågeformulär är komplext men frågorna visade på god styrka och samstämmighet. Pre- testfrågeformulär gav bra resultat men behöver vidare utveckling

    “NEWS2” as an Objective Assessment of Hospitalised COPD Exacerbation Severity

    Introduction: There is currently no accepted way to risk-stratify hospitalised exacerbations of chronic obstructive pulmonary disease (COPD). We hypothesised that the revised UK National Early Warning Score (NEWS2) calculated at admission would predict inpatient mortality, need for non-invasive ventilation (NIV) and length-of-stay. Methods: We included data from 52,284 admissions for exacerbation of COPD. Data were divided into development and validation cohorts. Logistic regression was used to examine relationships between admission NEWS2 and outcome measures. Predictive ability of NEWS2 was assessed using area under receiver operating characteristic curves (AUC). We assessed the benefit of including other baseline data in the prediction models and assessed whether these variables themselves predicted admission NEWS2. Results: 53% of admissions had low risk, 24% medium risk and 23% a high risk NEWS2 in the development cohort. The proportions dying as an inpatient were 2.2%, 3.6% and 6.5% by NEWS2 risk category, respectively. The proportions needing NIV were 4.4%, 9.2% and 18.0%, respectively. NEWS2 was poorly predictive of length-of-stay (AUC: 0.59[0.57– 0.61]). In the external validation cohort, the AUC (95% CI) for NEWS2 to predict inpatient death and need for NIV were 0.72 (0.68– 0.77) and 0.70 (0.67– 0.73). Inclusion of patient demographic factors, co-morbidity and COPD severity improved model performance. However, only 1.34% of the variation in admission NEWS2 was explained by these baseline variables. Conclusion: The generic NEWS2 risk assessment tool, readily calculated from simple physiological data, predicts inpatient mortality and need for NIV (but not length-of-stay) at exacerbations of COPD.
