    Trastuzumab (Herceptin (R)): Monoclonal antibody in the treatment of HER2/neu-overexpressing breast cancer in the metastatic and (neo)adjuvant situation

    Trastuzumab (Herceptin (R)) is a humanized monoclonal antibody that specifically targets HER2/neu (human epidermal growth factor receptor-2) breast cancer cells, which are overexpressed in about 25-30% of breast carcinomas. After phase I and II trials, several phase III studies of trastuzumab alone or in combination with various chemotherapies were conducted. Patients with HER2/neu overexpression levels of 3+ determined by immunohistochemical assay or gene amplification (fluorescence in situ hybridization) derive most clinical benefit from trastuzumab. Taking into consideration efficacy and side effect profile, the combination of trastuzumab and paclitaxel showed an improvement of all clinical parameters, including overall survival, for the first time in the history of palliative breast cancer therapy. The application of trastuzumab has meanwhile become an established part of systemic therapy of metastastic breast cancer, and excellent data of its application in the adjuvant setting now exist (NSABP-B31, NCCTG-N9831, HERA), with significantly better relapse-free survival in the treatment arms with trastuzumab. Ongoing trials investigate the role of trastuzumab in the neoadjuvant setting. Trastuzumab is generally well tolerated. Cardiotoxicity is the main concern, thus monitoring of cardiac function is recommended

    Modest effect of p53, EGFR and HER-2/neu on prognosis in epithelial ovarian cancer: a meta-analysis

    Background: P53, egfr and her-2/neu are the most frequently studied molecular biological parameters in epithelial ovarian cancer, but their prognostic impact is still unequivocal. We performed a meta-analysis to more precisely estimate their prognostic significance. Methods: Published studies that investigated the association between p53, egfr and her-2/neu status and survival were identified. Meta-analysis was performed using a dersimonian-laird model. Publication bias was investigated using funnel plots and sources of heterogeneity were identified using meta-regression analysis. Results: A total of 62 studies were included for p53, 15 for egfr and 20 for her-2/neu. P53, egfr and her-2/neu status had a modest effect on overall survival (Pooled hr 1.47, 95% Ci 1.33-1.61 For p53; Hr 1.65, 95% Ci 1.25-2.19 For egfr and hr 1.67, 95% Ci 1.34-2.08 For her-2/neu). Meta-regression analysis for p53 showed that figo stage distribution influenced study outcome. For egfr and her-2/neu, considerable publication bias was present. Conclusions: Although p53, egfr and her-2/neu status modestly influences survival, these markers are, by themselves, unlikely to be useful as prognostic markers in clinical practice.     Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Both spending and tax policies have been implemented in the United States with the goal of stimulating private sector research and development (R&D). Karier questions whether current R&D policy, especially the research and experimentation tax credit, can contribute to closing the gap between nondefense expenditures on R&D in the United States and such expenditures in other countries, such as Japan and Germany. He also explores possible changes to our current R&D policy to make it more effective.

PSYNDEX Tests is offered by the Leibniz Institute for Psychology as open access documentation.Das ist ein PSYNDEX Tests Review zu FPI-R (2020) - Freiburger Persönlichkeitsinventar - revidierte und neu normierte Fassung 2020. PSYNDEX Tests Reviews beschreiben und bewerten zentrale psychologische und pädagogische Testverfahren, die in den deutschsprachigen Ländern eingesetzt werden, nach einem standardisierten Raster. PSYNDEX Tests wird durch das Leibniz-Institut für Psychologie als Open Access Dokumentation angeboten.Diagnostische Zielsetzung: Das FPI-R dient der Erfassung von Persönlichkeitsmerkmalen über Selbsteinschätzungen und kann bei Personen ab 16 Jahren eingesetzt werden. Aufbau: Der Test besteht aus 138 Items, die 10 Standardskalen und 2 Zusatzskalen zugeordnet sind: (1) Lebenszufriedenheit, (2) Soziale Orientierung, (3) Leistungsorientierung, (4) Gehemmtheit, (5) Erregbarkeit,(6) Aggressivität, (7) Beanspruchung, (8) Körperliche Beschwerden, (9) Gesundheitssorgen, (10) Offenheit, (11) Extraversion und (12) Emotionalität. Die Items sind als Aussagen über bestimmte Verhaltensweisen oder Einstellungen formuliert, deren Zutreffen mit "stimmt" oder "stimmt nicht" zu bewerten ist. Grundlagen und Konstruktion: Grundlage der Konstruktion ist die Klassische Testtheorie. Das Verfahren bezieht sich ausdrücklich nicht auf eine bestimmte Persönlichkeitstheorie. Die Skalen wurden theoretisch-deduktiv und empirisch-induktiv konstruiert entlang der Forschungsinteressen der Testautoren. Der FPI-R 2000 ist die 9., überarbeitete und neu normierte Auflage der Vorgängerversion. Unter anderem wurden 13 Items sprachlich überarbeitet, die Normwerte der Altersgruppen 16-19 und 20-29 Jahre wurden angepasst und erstmals bildungsbezogene Normen für die Skalen Lebenszufriedenheit, Gehemmtheit und Emotionalität vorgelegt. Empirische Prüfung und Gütekriterien: Reliabilität: Die interne Konsistenz der Skalen liegt zwischen Alpha = .72 und Alpha = .82. Die Retestreliabilitäten bei Herz-Kreislauf-Patienten liegen nach vier Wochen zwischen rtt = .57 und rtt = .85, nach einem Jahr zwischen rtt = .47 und rtt = .72. Validität: Die faktorielle Validität wurde durch Faktorenanalysen belegt. Die kriterienbezogene Validität wurde mit Hilfe verschiedener Kriteriumsvariablen aufgezeigt.     Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.
