Correlations between biochemical findings and nonalcoholic fatty liver disease (NAFLD) in morbidly obese patients
Devido a controvérsias sobre a freqüência, diagnóstico e possibilidade de progressão para doença crônica da degeneração gordurosa do fígado em portadores de obesidade mórbida, planejou-se uma investigação retrospectiva em pacientes candidatos à cirurgia bariátrica. Foram analisados 60 indivíduos com indicação de gastroplastia com derivação intestinal em Y de Roux. Os pacientes foram submetidos a questionário clínico, provas bioquímicas e posteriormente, à biópsia hepática intraoperatória. A freqüência de EHNA e esteatose hepática foi elevada, e detectou-se 5% de cirrose. Na análise univariada houve correlação de diversos testes bioquímicos com as enzimas hepáticas. Na análise multivariada a albumina foi a variável determinante; apesar de seu debilitado valor preditivo. Nestas condições, apenas a avaliação histológica configura-se como critério definitivo para diagnóstico e acompanhamento desta populaçãoBecause of controversies about frequency, diagnosis and possibility of progression toward chronic disease of fatty degeneration of the liver in morbidly obese patients, a retrospective study was done in patients who underwent bariatric surgery. Sixty individuals with indication for gastroplasty with Roux-en-Y jejunal bypass were enrolled. They were submitted to clinical questionnaire, biochemistry tests and subsequently, intraoperative liver biopsy. Frequency of NASH and steatosis was high, and 5.0% suffered from cirrhosis. In univariate analysis there was correlation between various biochemical measurements and liver enzymes. In multivariate analysis only serum albumin was a determinant variable, with low predictive value. Pre and postoperative nutritional evaluation in patients with chagasic megaesophagus
O megaesôfago é manifestação clínica freqüente, em pacientes na fase crônica da doença de Chagas, na região Central do Brasil. Cursa com disfagia progressiva que pode levar a desnutrição protéico-calórica. O melhor tratamento para o megaesôfago é o cirúrgico. Pacientes desnutridos têm maior probabilidade de desenvolver complicações e mortalidade pósoperatória. Megaesôfago avançado ou recidivado podem ser tratados com a operação de Serra Dória, que consiste de cardioplastia, associada à gastrectomia parcial e anastomose gastrojejunal em alça exclusa, tipo Y de Roux. Torna-se de interesse avaliar o estado nutricional de pacientes portadores de megaesôfago chagásico no pré e pós-operatório, averiguando a repercussão que o estado nutricional pré-operatório possa ter sobre a evolução cirúrgica, assim como as conseqüências nutricionais da operação de Serra Dória. No presente trabalho foi realizada avaliação nutricional préoperatória em 27 pacientes portadores de magaesôfago chagásico, candidatos à operação Serra Dória. Após o 90º dia de pós-operatório, 19 pacientes foram reavaliados. Do total de pacientes, 75% tinham magaesôfago grupo IV; o restante, megaesôfago recidivado. O estado nutricional foi avaliado por meio de avaliação nutricional subjetiva global, antropometria, exames laboratoriais, além da análise de ingestão alimentar. No período pré-operatório observou-se, em mais de 60% dos doentes, desnutrição calórico-protéica do tipo marasmática caracterizada: por redução do índice de massa corpórea; pelo déficit ponderal em relação aos valores padrões; pela depleção acentuada da prega cutânea do tríceps, com preservação da circunferência muscular do braço, da albumina e da transferrina. De acordo com a avaliação nutricional subjetiva global, 2/3 dos pacientes apresentavam desnutrição. A desnutrição diagnosticada no préoperatório pela avaliação nutricional subjetiva global associou-se com o sintoma regurgitação. O estado nutricional pré-operatório não se associou a complicação ou mortalidade pós-operatória. Na avaliação pós-operatória observou-se aumento da ingestão protéica. Houve aumento do índice de massa corpórea e da prega cutânea do tríceps, além de redução dos níveis de hemoglobina no sangue periférico. Em conclusão, pacientes com megaesôfago chagásico grupo IV e recidivado, apresentam alta freqüência de desnutrição protéico-calórica. A desnutrição pré-operatória não se associa à morbimortalidade pós-operatória. No pós-operatório os pacientes apresentam recuperação da massa gordurosa corpóreaChagasic megaesophagus is a frequent clinical manifestation in patients in a chronic phase of Chagas disease in Central Brazil. The main sympton is progressive dysphagia that can lead to protein-calorie malnutrition. Surgical intervention is the best treatment for megaesophagus. Undernourished patients present a greater probability of developing complications and postoperative mortality. Advanced or recurrent megaesophagus can be treated with Serra Dória\'s surgical procedure, which consists of cardioplasty, associated with partial gastrectomy and gastrojejunal Roux-en-Y anastomosis. An evaluation is worthwhile of the nutritional state of patients with chagasic megaesophagus in the pre- and postoperative period, as the findings have repercussions on the surgical course. The nutritional consequences of Serra Dória operation should also be observed. In the present work, a preoperative nutritional evaluation was performed in 27 patients with chagasic megaesophagus who were candidates for the Serra Dória operation. After postoperative day 90, 19 patients were reevaluated. Of the total patients, 75% presented megaesophagus group IV and the remainder recurrent megaesophagus. The nutritional state was assessed through subjective global assessment, anthropometry and laboratorial exams, besides the analysis of alimentary ingestion. In the preoperative period it was observed that over 60% of the patients had protein-calorie malnutrition of the marasmic type, characterized by: reduction in the body mass index; ponderal index deficit in relation to the standard values; marked depletion in the skin fold of the triceps, with preservation of the muscular circumference of the arm and of the albumin and transferrin levels. In agreement with the global subjective nutritional evaluation, 2/3 of the patients presented malnutrition. The diagnosis of malnutrition in the preoperative by subjective global assessment was associated with the symptom of regurgitation. The preoperative nutritional state did not correlate with complications or postoperative mortality. The postoperative evaluation showed an increase in the ingestion of proteins. There was an increase in the body mass index and the tricipital skin fold, besides a reduction in the hemoglobin levels of the peripheral blood. In conclusion, patients with group IV and recurrent chagasic megaesophagus presented a high frequency of protein-calorie malnutrition. Antireflux valve self-expanding stent in the treatment of advanced gastroesophageal junction tumors
Introdução: A colocação de próteses através da junção esofagogástrica propicia o refluxo gastroesofágico. Objetivos e métodos: Avaliação da paliação da disfagia, manifestações broncoaspirativas, parâmetros nutricionais e qualidade de vida de 30 pacientes com tumores avançados da junção esofagogástrica tratados por prótese metálica auto-expansível com válvula anti-refluxo de 2001 a 2004. Resultados: Melhora da disfagia e regurgitação. Sem alteração na tosse, eructação, pirose, parâmetros nutricionais e qualidade de vida. Conclusões: A prótese com válvula anti-refluxo foi eficaz na paliação da disfagia, impediu a ocorrência de manifestações broncoaspirativas, não interferiu nos parâmetros nutricionais e qualidade de vidaBackground: Placement of stents through the gastroesophageal junction can cause reflux. Objective and methods: Evaluate dysphagia palliation, aspiration symptoms, nutritional parameters and quality of life in 30 patients treated for advanced gastroesophageal junction tumors with a self-expanding metal stent with anti-reflux valve from 2001 to 2004. Results: Dysphagia and regurgitation symptoms improved after placement. There was no changes in heartburn, cough, belching symptoms and also no improvement in nutritional parameters or quality of life. Gastroplastia a Collis associada à fundoplicatura parcial ou total: estudo experimental
RACIONAL: A esofagite de refluxo complicada com estenose e encurtamento do órgão é doença de difícil tratamento cirúrgico com frequente insucesso. A gastroplastia a Collis associada à fundoplicatura parcial ou total pode ser empregada nesta situação. OBJETIVOS: Avaliar a técnica da gastroplastia a Collis com fundoplicatura parcial ou total em cães. MÉTODOS: Sete cães foram submetidos a Collis-Lind (Grupo A) e quatro a Collis-Nissen (Grupo B) após miotomia da junção esofagogástrica. O refluxo foi avaliado com medidas de pH e as pressões na transição esofagogástrica com microtransdutores. RESULTADOS: Após a miotomia ocorreu refluxo livremente em todos os animais. Entretanto, após a operação de Collis-Lind ou Collis-Nissen o refluxo espontâneo não ocorreu. A aplicação de pressão manual sobre o estômago, promoveu refluxo em 28,5 % dos animais do Grupo A. A avaliação das pressões na peça cirúrgica mostrou que não houve diferença na zona de alta pressão, nos dois grupos. CONCLUSÃO: A operação de Collis-Lind ou Collis-Nissen impediu o refluxo gastroesofágico. As medidas das pressões e a extensão da zona de alta pressão foram semelhantes nos dois grupos.</jats:p

Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases.
