However, in order to meet future emissions legislations, Nitrogen Oxides (NOx) and particulate matters’ (PM) emissions will need to be reduced simultaneously. Nowadays researchers are focused on different combustion modes which can have a great potential for both low soot and low NOx. In order to achieve this, different injection strategies have been investigated. This study investigates the effects of split injection strategies with high levels of Exhaust Gas Recirculation (EGR) on combustion performance and emissions in a single cylinder direct injection optical diesel engine. The investigation is focused on the effects of injection timing of split injection strategies. A Ricardo Hydra single cylinder optical engine was used in which conventional experimental methods like cylinder pressure data, heat release analysis and exhaust emissions analysis were applied. Optical techniques like direct spray and combustion visualization were applied by means of a high speed imaging system with a copper vapor laser illumination system and a high-speed two-color system was applied to obtain in-cylinder diesel combustion temperature and soot measurements distributions

    This study used EMA data from smokers engaged in an earnest attempt to quit smoking to assess complex relations among coping, affect, and smoking. Analyses tested hypotheses about the main effects of coping and both mediators and moderators of coping effects on subsequent outcomes of interest (i.e., affect, coping effort, coping efficacy, and smoking behavior). Results of multilevel models indicated that coping does not improve negative affect within 4 hours of coping efforts, but that coping does improve positive affect and increase the odds of engaging in temptation coping in the short-term. Lapses were more likely to happen when recent coping was reported within 48 hours. None of the putative mediators of coping were predictive of later lapse risk as anticipated. Analyses also revealed that pre-quit coping practice moderated the effects of post-quit coping to deal with stressful events on later affect. To resolve this emergency, urethral catheterization is performed. If the procedure fails and permanent transurethral catheterization is required, the patient’s quality of life is significantly affected. Therefore, catheter-free treatment is the ideal goal of therapy for patients with acute urinary retention. Especially, for women, placement of a catheter poses a cosmetic problem. Therefore, the aim of this study was to treat female patients who had already received urapidil/distigmine bromide with acotiamide. Acotiamide was administered at a dose of 100 mg three times daily for 2 weeks, and the outcome of trial without catheter was evaluated. Only female patients were enrolled for this study.     Recovery of sensory disturbance after arthroscopic decompression of the suprascapular nerve

    Background: Recently, the existence of sensory branches of the suprascapular nerve (SSN) has been reported, and sensory disturbance at the lateral and posterior aspect of the shoulder has been focused on as a symptom of SSN palsy. We have performed arthroscopic release of SSN at the suprascapular notch in patients with the sensory disturbance since 2006. The purposes of this study were to introduce the arthroscopic surgical technique and to investigate postoperative recovery of sensory disturbance. Methods: Twenty-five shoulders (11 men and 14 women, average age: 63.9 years (range, 41-77)) followed for more than 1 year were included in this study. Arthroscopic decompression of SSN was performed using a suprascapular nerve portal (SN portal) as a landmark for approaching the suprascapular notch. Sensory disturbance of the shoulder was evaluated pre- and postoperatively. The follow-up period was 12-30 months (avg. 18.5). Results: The arthroscopic procedures was able to be performed safely. The preoperative sensory disturbance fully recovered postoperatively in all shoulders. Conclusion: Arthroscopic release of SSN is considered to be a useful procedure for the entrapment of SSN at the suprascapular notch. The sensory disturbance at the lateral and posterior aspect of the shoulder can be used as one of the criteria of diagnosing SSN palsy especially in shoulders with massive rotator cuff tear, which usually have difficulty in diagnosing and assessing the treatment results of associated SSN palsy

    Relations among affect, abstinence motivation and confidence, and daily lapse risk among smokers trying to quit

    Aims: This study prospectively tested the hypothesis that changes in momentary affect, abstinence motivation, and confidence would predict lapse risk over the next 12-48 hours using Ecological Momentary Assessment (EMA) data from smokers attempting to quit smoking. The moderating effects of high-risk contexts on relations between cognitions (motivation and confidence) and lapse risk were also tested. Method: 79 adult, daily smokers recorded their momentary affect, motivation to quit, abstinence confidence, and smoking behaviors in near real time with multiple EMA reports using electronic diaries post-quit. Results: Multilevel models indicated that increases in negative affect predicted greater lapse risk up to 12 hours, but not 24 hours later. Neither positive nor negative affect had significant effects on subsequent cognitions. High levels of motivation appeared to reduce increases in lapse risk that occur over hours. Momentary increases in confidence predicted greater lapse risk over 12 hours in high-risk situations, but not in the absence of potent smoking triggers. Conclusion: Momentary changes in negative affect, motivation, and confidence, during a quit attempt all had short-term effects on smoking lapse. Negative affect had short-lived effects on lapse risk, whereas high levels of motivation protected against the risk of lapsing that accumulates over hours. Contrary to expectations, an acute increase in confidence may increase vulnerability to lapse in the context of potent smoking triggers. Relations observed among affect, cognitions, and lapse seem to depend critically on the timing of assessments and the contexts in which the assessments occur.Ph.
