    Creating new opportunities for cardiac transplantation after circulatory death (DCD) using a novel pharmacological agent

    Contexte : Au cours de la dernière décennie, le nombre de personnes en attente d’une transplantation cardiaque a augmenté d’environ 25%, tandis que le nombre de greffes effectuées chaque année est resté stable. Le taux de décès des patients en attente d’une greffe cardiaque est d’environ 15-20%. Le don d’organe suite à un décès cardiocirculatoire (DDC) est une alternative au don après décès neurologique (DDN) qui a permis d’augmenter le nombre d’organes disponibles comme les poumons, les reins et les foies. Compte tenu de la survenue d’une mort cardiovasculaire dans les protocoles DDC, le cœur est rarement greffé à cause des dommages infligés durant la période d’ischémie chaude. Notre équipe a précédemment démontré que l’utilisation du Celastrol, ainsi que notre analogue synthétique inhibiteur de la HSP90 ont des effets cardioprotecteurs, quand administrés au moment de la reperfusion dans des modèles in vitro de culture cellulaire et ex vivo dans des cœurs de rats montés sur le système de perfusion Langendorff. L’objectif est d’évaluer les mécanismes cardioprotecteurs rapides d’une nouvelle formulation de l’inhibiteur HSP90, et de comprendre l’efficacité de ce nouveau composé synthétique sur deux lignées de cellules : les cardiomyoblastes H9c2 issus de rats et les cardiomyocytes dérivés de cellules souches pluripotentes humaines (iPSC-CMs). Méthodes/Résultats : Les cellules H9c2 et iPSC-CMs ont été cultivées. La signalisation cellulaire a été analysée par western blot pour évaluer le niveau d’activation de ces différentes voies. Suite à l’optimisation des conditions pour les cellules iPSC-CMs, les deux lignées cellulaires ont été mises en condition ischémique (sans glucose, 95% N2, 5% CO2) durant la nuit, puis reperfusées, en conditions normales, avec différentes concentrations de l’inhibiteur HSP90. La viabilité cellulaire ainsi que l’ouverture des pores mitochondriaux (mPTP) ont été évaluées à l’aide de kits d’analyses, la production de radicaux libres d’oxygène à l’aide de kits de fluorescence et l’expression des ARN messagers de gènes antioxydants à l’aide de la réaction en chaîne par polymérase (PCR). Les résultats ont montré une augmentation de l’activation des voies cytoprotectrices quand les deux lignées cellulaires étaient traitées à la concentration 10-6M du composé sans stress 4 ischémique : augmentation de HO-1 and HSP-70 dans les 30 premières minutes et AKT et ERK après 1 heure de traitement et 3 heures de récupération. Contrairement à nos attentes, le traitement au moment de la reperfusion à la concentration 10-6M a montré une diminution de la viabilité des cellules, alors que la concentration 10-7M l’a augmenté. À une concentration de 10- 7M, il y a eu diminution de la production de radicaux libres comparativement au groupe témoin. Comme attendu, cette concentration a aussi démontré une diminution de l’ouverture des mPTP. Tous ces résultats ont été observés, autant dans les cellules humaines que celles de rats. Une évaluation préliminaire de l’expression des gènes antioxydants dans les cellules H9c2 a seulement montré une augmentation de l’expression des gènes CAT et HO-1. Conclusion : Notre groupe de recherche a précédemment démontré l’efficacité des composés issus du Celastrol sur la réduction des dommages myocardiques dus à la reperfusion dans les modèles d’ischémie, incluant l’infarctus du myocarde et la donation après décès cardiocirculatoire. Ces expériences ont montré les effets bénéfiques du nouveau composé synthétique sur l’expression des gènes antioxydants, et sur l’activation d’une série de voies cytoprotectrices permettant la stabilisation de la membrane mitochondriale, réduisant aussi la production de radicaux libres, et améliorant ultimement la survie cellulaire. Des études supplémentaires sont en cours afin d’améliorer la compréhension des modes d’action, des mécanismes et des dosages optimaux du médicament, ce qui nous permettra de commencer les essais sur animaux dans le but d’introduire cette molécule en clinique dans le contexte de don d’organes.Background: During the last decade, the number of people waiting for a cardiac transplantation has increased by about 25%, while the number of yearly transplant surgeries performed has remained steady. The death rate of patients awaiting heart transplant is about 15-20%. Organ donation after circulatory death (DCD) is an alternative to donation after neurological death (DND) that has allowed to increase the number of available organs like lungs, livers, and kidneys. However, because of the cardiac death in DCD protocols, the heart is rarely used because of the injuries suffered by the warm ischemia period. Our group has previously shown that Celastrol, along with a synthetic HSP90 inhibitor analog, have cardioprotective effects when given as postconditioning agents at the moment of reperfusion in an in vitro model on cellular cultures and an ex vivo model on rat hearts mounted on a Langendorff perfusion system. The objective is to evaluate the rapid cardioprotective mechanisms of a novel formulation of the HSP90 inhibitor compound, and to understand the efficacy of this new synthetic compound on two cell lines: rat H9c2 cardiomyoblasts and human induced pluripotent stem cell-derived cardiomyocytes (iPSCCMs). Methods/Results: H9c2 rat cardiomyoblasts and human iPSC-CMs were cultured. Cell signaling was analyzed by western blot to evaluate pathway activations. Both cell lines were put in ischemic conditions (no glucose, 95% N2, 5% CO2) overnight, then reperfused (normal conditions) with different concentrations of HSP90i after optimizing the human iPSC-CMs’ stress experiment. Cell viability and mitochondrial permeability transition pore (mPTP) opening were evaluated using assays, oxygen-free radical production by fluorescence assay and antioxidant gene messenger RNA expression via polymerase chain reaction (PCR). Results showed an increase in cytoprotective pathway activation when both cell lines were treated with 10-6M of the compound without any stress: HO-1 and HSP-70 in the first 30 minutes while AKT and ERK after 1 hour of treatment and 3 hours of recuperation. Interestingly, treatment with the compound at 10-6M at the moment of reperfusion showed decreased viability of the cells while 10-7M improved it. Free radical production was also decreased at a concentration of 10-7M 6 when compared to the baseline, and as expected, the compound also decreased mPTP opening. These results were seen in both human and rat cell lines. Preliminary evaluation of antioxidant gene expression in H9c2 cells only showed an increase in the expression of the cytoprotective CAT and HO-1 genes. Conclusion: Our research group has previously demonstrated the efficacy of Celastrol compounds in reducing reperfusion damage in myocardial ischemia models, including myocardial infarction and donation after circulatory death. These experiments have shown that the beneficial effects of this new synthetic compound include the expression of antioxidant genes and the launching of a series of cytoprotective pathways that stabilize the mitochondrial membrane, reduce free radical production, and improve cell survival. Additional studies to fully understand the mode of action, the mechanisms and the optimal dosages are underway to allow us to move to animal trials in order to ultimately introduce the molecule in the clinical field in the context of organ donation

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Identification en thérapie cellulaire des patrons d’expression transcriptomique de cellules souches utilisées pour traiter la défaillance cardiaque afin d’en améliorer le potentiel thérapeutique

    La cardiopathie ischémique incluant l’insuffisance cardiaque est la deuxième cause de mortalité annuelle au Canada. Bien que de nombreuses stratégies préventives et des thérapies pharmacologiques retardent la progression de la maladie, il n’existe aucune solution qui module directement aux les remaniements pathologiques et la perte de cardiomyocytes. Au cours des 25 dernières années, de multiples progrès dans les domaines de la médecine régénérative et de la thérapie cellulaire ont annoncé des résultats prometteurs, mais les résultats d’études cliniques contemporaines demeurent plutôt mitigés. COMPARE-AMI, une étude randomisée-contrôlée de phase II, a évalué l’effet d’injections intracoronariennes de cellules souches hématopoïétiques CD133+ chez des patients souffrant d’infarctus aigu. IMPACT-CABG, une ÉRC de phase II a également évalué l’effet d’injections intramyocardiques de cellules CD133+ chez les patients souffrant de cardiomyopathie ischémique chronique nécessitant une revascularisation chirurgicale. Nous avons émis l’hypothèse que les cellules CD133+ utilisées dans des études cliniques de cardiomyopathies ischémiques aiguës et chroniques des patients répondant à la thérapie cellulaire exhibent des signatures transcriptomiques communes responsables de leur effet thérapeutique. En classant les patients en tant que répondants et non-répondants selon leur fonction cardiaque, nous avons évalué, a posteriori, ces patrons d’expression. Les cellules CD133+ autologues de patients jugés répondants expriment des signatures qui sont hautement conservées entre elles (incluant l’angiogénèse, la régulation de la réponse au stress et la survie cellulaire) et uniques d’un modèle à l’autre et qui pourraient, en partie, exprimer les issus cliniques des patients. Afin de maximiser les effets de la thérapie cellulaire aux cellules souches, nous avons par la suite tenté de reproduire ces phénotypes par stimulation pharmacologique avec des inhibiteurs d’HSP90 pour leurs effets qui semblent reproduire ces signatures. Ainsi, nous avons démontré qu’une stimulation de cellules souches mésenchymateuses humaines (CSMh) au Célastrol (inhibiteur HSP90) pouvait répliquer certains de ces phénotypes. Notamment, des CSMh conditionnées activent des voies de signalisation de type ‘RISK’ et augmentent leur sécrétion de protéines en lien avec la réponse au stress ainsi que d’exosomes contenant des molécules impliquées dans la communication intercellulaire sans être liées à un changement de type cellulaire. De plus, les CSMh traitées semblent améliorer la guérison de plaie par activité paracrine et sont plus résistante à la sénescence oxydative. Ces résultats encourageants nous permettent d’envisager des stratégies plus poussées de pré-conditionnement cellulaire ex vivo de cellules CD133+ avant leur implantation. À terme, cela pourrait mener à une optimisation de la thérapie cellulaire afin d’en maximiser les bénéfices cliniques et d’en exploiter leur plein potentiel.Ischemic cardiomyopathy and heart failure are the second annual cause of mortality in Canada. Despite rigorous prevention strategies and drug regimens preventing progression, no therapeutic modality can currently reverse the pathologic modifications of the disease. In the last quarter century, numerous advances in the field of regenerative medicine and cell therapy have demonstrated promising effects. COMPARE-AMI, a phase II randomized controlled trial (RCT), evaluated the effect of intracoronary injection of CD133+ cells in acute myocardial infarction following percutaneous intervention. IMPACTCABG, also a phase II RCT, evaluated the effect of intramyocardial injection of CD133+ hematopoietic stem cells in chronic ischemic cardiomyopathy at the time of surgical revascularization. That being said, we believe that the CD133+ cells used in therapy have shared transcriptomic signatures that are responsible for their clinical effects. By classifying patients into responders and non-responders according to an improvement in ejection fraction, we evaluated, a posteriori, those expression patterns. Autologous CD133+ cells of patients classified as responders expressed highly conserved transcriptomic signatures that could be responsible for the clinical outcomes of patients. Notably, these signatures were related to cardioprotective mechanisms including angiogenesis, stress response regulation and cell survival. In order to harness the full potential of stem cell therapy, we attempted to reproduce the identified phenotypes by pharmacological intervention with HSP90 inhibitors which are known to mimic some of these effets. Conditioned human mesenchymal stem cells (hMSC) activated ‘RISK’-type signaling pathways and augmented exosome and protein secretion relating to the response to cellular stress; this activation was unrelated to a switch of cell type. Furthermore, treated hMSC seemed to favour improved wound healing by paracrine activity and were more resistant to oxidative senescence. These encouraging results allow us to develop novel, more advance, strategies of ex vivo cell preconditioning before implantation, including of CD133+ cells. Ultimately, we hope that optimisation of cells through this mechanism and others will allow us to unleash the full potential of stem cell therapy

    Développement d’un modèle animal de choc cardiogénique pour l’évaluation des dispositifs d’assistance ventriculaire percutanés

    Le choc cardiogénique (CC) est un état d’hypoperfusion critique des organes cibles causé par une dysfonction profonde du myocarde. Cette situation dangereuse et dynamique nécessite des interventions rapides de la part d'une équipe multidisciplinaire pour sauver la vie du patient, mais le risque de décès demeure encore très élevé́. Actuellement, l’utilité des dispositifs d’assistance ventriculaire percutanés (DAVp) pour traiter le CC n’est pas suffisamment étudiée. Concevoir un modèle artificiel de CC pourrait faciliter la compréhension du CC ainsi que le développement de nouveaux DAVp. Au cours de ce projet, nous nous sommes premièrement intéressés au sujet en synthétisant les données cliniques sur l’utilisation des DAVp dans un contexte de CC compliquant un infarctus du myocarde (IM-CC) sous forme de revues systématiques. Par la suite, nous avons conçu un projet expérimental visant à démontrer la faisabilité d’un modèle animal stable d’IM-CC en induisant par méthode percutanée un infarctus étendu de la paroi antérieure in vivo chez le porc qui pourrait être utilisé pour fournir des données physiologiques supportant la création d’un modèle artificiel d’haute-fidélité. L’état de CC stable a été confirmé par une combinaison de données hémodynamiques et de laboratoire et l’ampleur de l’infarctus a été validée par des techniques de coloration ex vivo. Ayant atteint notre objectif primaire de ≥50% de survie suite à l’infarctus et l’induction d’un état de CC chez 50% des cochons, nous concluons que notre modèle animal est suffisamment stable pour procéder à la prochaine étape de notre programme.Cardiogenic shock (CS) is a state of critical end-organ hypoperfusion resulting from profound myocardial dysfunction that is both dangerous and dynamic and requires rapid, coordinated multidisciplinary care in order to prevent mortality. However, despite appropriate medical management, the risk of early mortality remains high. Percutaneous mechanical support devices (pMCS) offer the promise of correcting pump dysfunction, but their clinical utility in CS remains debated and understudied. Developing a reliable synthetic model of CC could both improve our understanding of CS and accelerate the development of the next generation of pMCS devices. In this work, we first present the results of two systematic reviews of the comparative effectiveness of currently available pMCS devices in the setting of post-acute myocardial infarction CS (AMI-CS). We then sought to demonstrate the feasibility of creating a stable animal model of AMI-CS by inducing an anterior myocardial infarction in vivo in a pig in order to generate the physiologic data required to develop a high-fidelity three-dimensional AMI-CS simulator. The CS state was confirmed by a combination of hemodynamic and laboratory data and the size of the infarct was confirmed thereafter by ex vivo staining techniques. We achieved our primary goal of ≥50% short-term survival post-infarction and induction of a CS state in 50% and therefore conclude that our model is sufficiently stable to warrant proceeding with the next phase of our progra

    Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    The effects of celastrol on endothelial cells survival and proliferation

    Introduction: Coronary artery bypass grafts are most commonly performed using saphenous vein grafts to complement the internal thoracic artery. The saphenous vein will remain popular despite its lower patency rate because it is easily accessible and lengthy enough to perform multiple bypasses. Therefore, several approaches have been studied, with the common goal of finding the optimal conditions that reduce graft failure. They include novel harvest techniques, new preservation preparations, innovative genetic therapies and experimental drugs. We believe a pharmacological pre-conditioning with an anti-oxidative and anti-inflammatory drug during the crucial time of harvest may spark beneficial survival response from the endothelial cells. One particular compound is Celastrol, an HSP90 inhibitor, which displays those antioxidant and anti-inflammatory properties. Methods: Human umbilical vein endothelial cells (HUVEC) were pretreated with various concentrations of Celastrol (10-10M, 10-8M and 10-6M). In order to reproduce oxidative stress found in ischemia/reperfusion, cells were exposed to hydrogen peroxide for a short and extended period (1h and 24h). To mimic storage condition encountered in clinical settings, cells were also exposed in heparinized normal saline. The viability was assessed by LIVE/DEAD assay. As for migrative and proliferative properties, scratch tests were performed. Finally, various protective intracellular pathways were evaluated by Western blot. Results: This study shows that pre-treatment with Celastrol promotes survival in HUVEC submitted to oxidative stress. Notable improvement in cellular viability was detected as early as 1 hour after oxidative stress (H2O2 4 mM), 76.6% vs 66.1% (p=0.005). Significant survival benefits are also reported after prolonged oxidative stress (H2O2 0.5 mM for 24 hours); viability was 93.7% vs 76.9% (p=0.001) for Cel 10- 8 M and 96.6% vs 76.9% (p=0.002) for Celastrol 10-10M when compared to the vehicle. Celastrol, however, did not significantly affect viability of HUVEC stored in heparinized normal saline. Celastrol at 10-6 M promotes faster and more complete wound closure compared to the vehicle or to lower dosages. Celastrol triggers early activation of the RISK pathway, inducing activation of both Akt and ERK1/2 within the first 15 minutes of treatment. Celastrol also induces the expression of HSP70 and HO-1, effectors of the Heat Shock Response and the anti-oxidative response respectively. Conclusion: Pre-treatment by Celastrol provides survival benefits in endothelial cells under oxidative stress. It also stimulates endothelial cell proliferation and migration, promoting faster and more complete re-endothelialisation. Celastrol can potentially be used as an additive to storage solutions to limit endothelial injury and promote graft protection.Introduction: La chirurgie de pontage coronarien requiert, dans la grande majorité des cas, l’utilisation de l’artère mammaire interne en combinaison avec un ou des greffons provenant de la grande veine saphène. Malgré le taux de perméabilité inférieur aux artères, la veine saphène reste un choix populaire de conduit en raison de son accessibilité et de sa longueur. De ce fait, le greffon veineux devient la cible de multiples approches et le sujet de nombreuses études visant à optimiser sa perméabilité. Celles-ci incluent le raffinement des techniques de prélèvement, les solutions de préservations, les agents pharmacologiques ainsi que la thérapie génique. Il est davantage intéressant de combiner les approches afin de joindre leurs bénéfices, comme, par exemple, ajouter un agent pharmacologique à une solution de préservation. Un agent potentiel serait le Celastrol, connu pour être un inhibiteur du HSP90 et possède des propriétés antioxydantes et anti-inflammatoires. Méthodologie: Des cellules endothéliales humaines provenant de la veine ombilicale (HUVEC) sont pré-conditionnées à de multiples concentrations de Celastrol (10-10M, 10-8M and 10-6M) pendant une heure avant d’être soumises aux conditions de stress. Pour reproduire les conditions per-opératoires de prélèvement, les cellules endothéliales ont été préservées dans du salin (NS) héparinisé. Pour mimer le stress secondaire à l’ischémie/reperfusion, les cellules ont aussi été soumises à diverses concentrations de H2O2. Une analyse de la viabilité cellulaire fut conduite par le test de LIVE/DEAD. La capacité de ré-endothélialisation est étudiée grâce à l’épreuve de scratch test. Les voies intracellulaires de survie telles que le RISK pathway (Akt, ERK1/2), le Heat shock response (HSP70) et la réponse anti-oxydante (via l’activité de HO-1) ont été examinées par immunoblot. Résultats: Les résultats démontrent que la préservation des cellules endothéliales dans du NS héparinisé est associée à une augmentation de la mortalité comparativement au milieu de culture (20.4% vs 1.9%, p=0.004). Toutefois, un traitement au Celastrol n’affecte pas significativement la survie des cellules endothéliales dans le NS héparinisé. Le stress oxydatif induit aussi une augmentation de la mortalité, et ce à dose-dépendante. Suivant un court stress 6 oxydatif (H2O2 4 mM), un pré-traitement au Celastrol 10-10M est associé à une meilleure viabilité comparativement au véhicule (76.6% vs 66.1%, p=0.005). Lorsque soumises à un stress oxydatif prolongé (H2O2 0.5 mM pendant 24h), les HUVEC pré-traitées au Celastrol à 10-8M et 10-10M démontrent une amélioration significative de la viabilité, 93.7% vs 76.9% (p=0.001) et 93.6% vs 76.9% (p=0.002) respectivement. Quant à la ré-endothélialisation, un traitement au Celastrol 10- 6M est associé à une fermeture plus rapide et complète comparativement au véhicule. Un court traitement au Celastrol active précocement les kinases de la voie de RISK (Akt et ERK). Le traitement induit aussi l’expression de HSP70 et HO-1 qui reste soutenue jusqu’à 48 heures posttraitement. Conclusion: Le Celastrol active plusieurs voies de protection intracellulaire tels que le RISK pathway, le Heat Shock Response et la réponse antioxydante via l’activité de HO-1. En corrélation avec cette réponse, il améliore la survie des cellules endothéliales dans un milieu oxydatif. Le Celastrol promeut aussi une ré-endothélialisation plus complète et rapide. Cette étude met en valeur les bénéfices potentiels du Celastrol sur les cellules endothéliales. Afin d’optimiser la protection du greffon, le Celastrol pourrait donc être considéré comme agent adjuvant à une solution de préservation
