    Fertility preservation requires close coordination between reproductive medicine specialists, reproductive biologists and oncologists in various disciplines. Therefore, it is inevitable that fertility-preservation activities should organize themselves into a network structure both as a medical-logistic network and as a professional medical society. The necessary network structures can differ significantly at regional, national and international levels, as the size of the regions to be integrated and the local cultural and geographical conditions, as well as the political conditions, are very different. To address these issues, this chapter aims to point out the basic importance and the chances, but also the difficulties of fertility-protection networks

    Counselling on fertility-preservation therapy should be provided for all women and men who are highly likely to survive the disease, in whom pregnancy or paternity is compatible with the disease, who are at risk of a relevant reduction in fertility and for whom fertility-preservation therapy with a low risk to health is feasible. This means that a balance must be struck between advising as many patients with an indication for fertility-preservation therapy as possible, but also only treating those who will have a sufficiently high probability of benefiting from such therapy, thus avoiding overtreatment

    The administration of gonadotoxic agents (e.g. during chemotherapy) in premenopausal women can lead to vaginal bleeding disorders and pose a difficult treatment situation for oncological specialists. This chapter describes practical treatment options for serious vaginal bleeding events by considering hormonal and non-hormonal drugs (such as e.g. oral contraceptives, GnRH agonists, tranexamic acid). A distinction is made between prevention of bleedings and treatment for acute bleeding, stating possible medications, medical combinations and dosages. Radiological or surgical interventional procedures are also discussed, although they are rarely used

    Turner Syndrome

    Turner syndrome (TS) summarises a heterogeneous group of patients with different chromosomal pathologies, including 45,X0 monosomy, mosaicism and structural abnormalities of the X chromosome. Monosomies usually lead to very early premature ovarian insufficiency, whereas mosaicism and structural abnormalities may preserve fertility. The genotype should therefore be taken into account in the indication for and implementation of a fertility preservation measure. The increased pregnancy risks in Turner syndrome must also be considered. Fertility preservation measures include freezing of oocytes and possibly ovarian tissue

    FertiPROTEKT Network

    The FertiPROTEKT network was founded in 2006 and coordinates the fertility-preservation activities of physicians and embryologists in 143 centres in Germany, Austria and Switzerland. In 2015, FertiPROTEKT became a society. Centres that would like to become members of the FertiPROTEKT network must prove with their application that they not only advise on fertility protection, but that they can also implement all fertility-protective methods themselves or in established co-operations. One of the key elements of FertiPROTEKT is centralized cryobanks for ovarian tissue. Special achievements of FertiPROTEKT are the introduction of luteal phase stimulation, the combination of hemiovariectomy to freeze tissues and ovarian stimulation to freeze oocytes, the introduction of a central registry and the preparation of practical recommendations
