Efficacy of low-level laser versus high-intensity laser therapy in the management of adhesive capsulitis: A randomized clinical trial
Background: Low-level laser therapy (LLLT) and high-intensity laser therapy (HILT) are effective in alleviating pain and improving functionality in patients with adhesive capsulitis (AC); however, no study has compared the efficacy of these two laser treatments.
Objective: To compare the effectiveness of LLLT and HILT in improving the shoulder joint range of motion and functional status and in reducing pain level in patients with AC.
Trial Design: Prospective, randomized, parallel group, patient- and assessor-blinded.
Methods: A total of 45 patients (aged: 18–65 years) with complaint of shoulder pain were evaluated for inclusion criteria, which included being aged 18–65 years and a diagnosis of AC based on physical examinations. Using computer-generated random numbers, eligible patients were randomized into two groups: HILT + stretching exercise and LLLT + stretching exercise groups. Both HILT and LLLT were performed three times/week for 3 weeks. Functional status and pain of the patients were evaluated with Shoulder Pain and Disability Index (SPADI) and Visual Analog Scale (VAS), while shoulder joint range of motion was measured with goniometry. All assessments were done before and 3 weeks after treatment.
Results: A total of 40 patients (20 in each group) completed the study. At baseline, there was no statistically significant difference in the demographic and clinical characteristics between both groups. Both the LLLT and HILT groups showed significant improvement in the VAS and SPADI scores 3 weeks after treatment; however, the improvement was significantly higher in the HILT group than the LLLT group. There was no significant improvement in goniometric scores in both groups compared with baseline. No injury or other musculoskeletal complications were recorded during or after the treatments.
Conclusion: HILT + stretching exercise treatment was more effective than LLLT + stretching exercise for improving functional parameters and pain in patients with AC.

Dolayısıyla bu yazıda öncelikle uzun yıllar gazetecilik yapan Suat Derviş’in gazeteci kimliği üzerinde durulacak, ardından Fosforlu Cevriye, yazarın yaptığı röportajlar çerçevesinde yorumlanacaktır.When interpreting the works of Suat Derviş, who has become one of the names that literature researchers have emphasized, in recent years, it is necessary to mention her journalist identity that shaped her view of life and her understanding of art and her articles in newspapers. Among these articles, especially the interviews she published in newspapers are important. It is possible to say that Suat Derviş, as a journalist, carried the events she saw, the people she met, to her interviews, and therefore she used these interviews many times in her fictional works, both when creating the characters and shaping the event. Through these interviews, which shaped both her journalist identity and her artistic imagination, the author, who went down to the back streets of İstanbul, used the experiences she gained through these interviews as material in her works. In this context, the novel Fosforlu Cevriye, which was first serialized in the newspaper Gece Postası in 1945, is also a novel written based on Suat Derviş’s interviews. Therefore, this article will primarily focus on the journalist identity of Suat Derviş, who has been a journalist for many years, and then Fosforlu Cevriye will be interpreted within the framework of the author’s interviews.</p

material flows among customers is the combination of the vehicle routing
problem with simultaneous delivery and pick-up and the pickup and delivery
problems. In these problems, custumers are able to send materials to each other.
In daily life, we are confronted with this problem. For exemple, a hypermarket
has to transport not only the materials initially available at its depot to its
agencies as well as those from the agencies to the depot, but also extra materials
available at their agencies to each other because of demand fluctuations.
In this thesis, the vehicle routing problem with simultaneous delivery and pickup
and material flows among customers has been defined, a new heuristic
algorithm has been suggested to solve this type of problems, an MATLAB codes
of the proposed algorithm have been developed, and finally 60 author-generated
test instances have been solved using four different insertion criterion in the

Investigating the Real Effects of Isotretinoin on Enthesopathy in Acne Vulgaris Patients: A Prospective, Controlled Study
Background: Isotretinoin is frequently used for treating severe, treatment-resistant, and scarring acne. A wide range of musculoskeletal side effects are reported due to isotretinoin, and one of them is enthesopathy. Objectives: In this study, we used ultrasound to evaluate tendon thickness and the presence of enthesophytes, erosions, calcifications, and bursitis. Overall, the main objective was to determine the effects of isotretinoin on enthesopathy. Methods: The study included 30 patients diagnosed with acne vulgaris (AV) and on isotretinoin treatment for >= 3 months. Thirty patients with AV without treatment or on topical treatments were included as the control group. In both groups, 5 areas in the lower limb were examined bilaterally for tendon thickness, erosion, calcifications, enthesophytes, and bursitis and compared among themselves. Moreover, the Glasgow Ultrasonographic Enthesitis Scoring System (GUESS) scores were also calculated and compared between the 2 groups. Results: The quadriceps tendon, distal patellar ligament, and right Achilles tendon thicknesses were statistically higher in the isotretinoin group than in the control group (P = .02, P = .03, P = .04). The GUESS score was also statistically higher in the isotretinoin group (P = .02).Conclusion: This study showed that isotretinoin has certain effects on tendon thickness. Clinicians should be alert that patients under isotretinoin treatment may develop enthesopathy even with no clinical signs
From mansions to streets: Interviews, stories and novels by Suat Derviş
This study aims to analyze Suat Derviş, who existed as both a literary writer and a journalist for nearly half a century of her life, from her early youth until her death, but who was ignored despite her huge oeuvre and who was thought to have been underappreciated during her lifetime, from the perspective of sociology of literature. In order to understand the reasons why the author, who dealt with personal issues until the end of the 1930s, turned towards society after this period, the impact of both her life and her journalist identity on her works was tried to be understood. For this purpose, the introduction provides a general information about the field of sociology of literature and clarifies the field of study. In the first part, Suat Derviş's life, art and works are discussed; in the second part, the course of development of the Turkish novel and the Socialist Realist approach are discussed; and in the third part, Suat Derviş's interviews conducted in the newspapers Son Posta, Cumhuriyet and Tan between 1935 and 1937 as a journalist, some of her stories written in the same period and published in various newspapers, and her novels Bu Olan Şeylerin Romanıdır, İstanbul'un Bir Gecesi, Fosforlu Cevriye and Ankara Mahpusu, in which socialist elements are dominant, are analyzed
RECALCITRANT CELILES, CAVIDES, CEVRIYES: ISSUE OF MORALITY IN THE NOVELS OF SUAT DERVIŞ
Bu anlamda, Suat Derviş'in toplum ile münasebetini anlamak ve ortaya çıkan tablonun yazarın kişiliği, kadınlığı ve yazarlığı hakkında ne anlattığını görebilmek için konu hakkında tipik örnekler veren metinlerine odaklanmak gerekirThere are quite a lot of forgotten women in the history of our literature. Suat Derviş, who lived between 1903-1972 and wrote more than thirty novels published serially in various newspapers and magazines, is one of those women. Derviş, whose name was not even known by most of the readers until the 1990s, came to the fore after Oglak and Ithaki Publishing House republished her books. For the readers who were familiar with her, Suat Derviş was famous for Cevriye of Fosforlu Cevriye or Zeynep of Ankara Mahpusu. In fact, Suat Derviş was a woman who took part in publishing business since the 1920's and produced very valuable works. Moreover, Derviş did not only write works reflecting her political views, but also focused on a wide variety of themes ranging from gothic storylines to heartaches. In this sense, there were decent girls, upper-class people and betrayed women, as well as the 'prostitutes', low-class people and betraying women in her books. Such prominent figures in the works of the author showed her opinion about what 'being a woman in a patriarchal society' meant and turned into a material questioning the sense of morality of the society. This questioning is not only made over women. Both male and female characters of Derviş's novels grow and shape in an era and society. In this sense, her texts giving typical examples about the topic should be focused in order to understand Derviş's relationship with the society and to see the general picture of the author's personality, femininity and authorship
THE EFFECT OF GENDER ON DISEASE ACTIVITY, FUNCTIONAL INDEX AND QUALITY OF LIFE IN PATIENTS WITH AXIAL SPA. THE DATA OF TLAR-NETWORK PSA STUDY
POSSIBLE POTENTIAL INTERACTIONS BETWEEN OBESITY, QUALITY OF LIFE, PSYCHOLOGICAL STATUS AND CLINICAL PARAMETERS IN PSORIATIC ARTHRITIS
Clinical characteristics, disease activity, functional status, and quality of life results of patients with psoriatic arthritis using biological and conventional synthetic disease-modifying antirheumatic drugs
Objectives: This study aims to compare the clinical characteristics, disease activity, and quality of life (QoL) of patients with psoriatic arthritis (PsA) who use biological and conventional synthetic disease-modifying antirheumatic drugs (DMARDs) in a nationwide cohort throughout Turkey. Patients and methods: A total of 961 patients (346 males, 615 females; mean age: 46.9+/-12.2 years; range, 18 to 81 years) with PsA according to the classification criteria for PsA were included in the study. The patients demographic and clinical characteristics, physical examination results, Disease Activity Score 28, Disease Activity Index for Psoriatic Arthritis and Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), Psoriasis Area and Severity Index, Bath Ankylosing Spondylitis Functional Index, Bath Ankylosing Spondylitis Metrology Index, Hospital Anxiety and Depression Scale, Health Assessment Questionnaire, Psoriatic Arthritis Quality of Life (PsAQoL), and Short Form-36 scores were all recorded. Results: Of the patients, 23% underwent biological DMARD (bDMARD) monotherapy, 42% underwent conventional synthetic DMARD (csDMARD) monotherapy, 10% underwent a csDMARD combination therapy, and 10% underwent a combination bDMARD and csDMARD treatment. The Visual Analog Scale (VAS pain), patient global assessment, physician global assessment, and BASDAI scores were found to be lower among patients using combination treatment of csDMARD and bDMARD, while the swollen joint count was found to be lower among patients using bDMARD. The PsAQoL score was found to be the lowest among patients not using any medication and the highest among those using bDMARD. Conclusion: In our study, patients with PsA were successfully treated with both csDMARD and bDMARD monotherapy. When the biological treatments used for PsA were compared with csDMARD, it was found that biological treatments had a positive effect on both disease activity and the QoL. Combinations of csDMARDs and bDMARDs were preferred in cases in which the disease activity was still high or increased. Because of the highest efficacy of the combined treatment, we highly suggest increasing the number of patients on combined treatment
Gender-related differences in disease activity and clinical features in patients with peripheral psoriatic arthritis: A multi-center study
Objective: This study sought to compare disease activity, clinical features, and patient-reported outcomes concerning anxiety, depression, fatigue, function, quality of life, and fibromyalgia between female and male patients with peripheral PsA in a Turkish population. Methods: This multi-center Turkish League Against Rheumatism (TLAR) Network study included 1038 patients (678 females, 360 males) diagnosed with peripheral PsA according to the CASPAR criteria. The demographic and clinic parameters of the patients were recorded. Disease activity was evaluated using the scores of DAS28 and cDAPSA. Remission, minimal disease activity (MDA), and very low disease activity (VLDA) were determined. Health Assessment Questionnaire (HAQ), Short-Form-36 (SF-36), Hospital Anxiety and Depression Scale (HAD), fatigue VAS (0-10), and Fibromyalgia Rapid ScreeningTool (FiRST) were used. Disease activity and patient-reported outcomes were compared in male and female patients, and the predictors of MDA for both genders were analyzed. Results: The patients' mean age was 47.6 years (SD: 12) for females and 46.3 years (SD: 12.3) for males. In terms of DAS28 and cDAPSA, female patients had significantly higher disease activity scores, while male patients had significantly higher remission rates (P<0.05). There was a significant difference in the rate of MDA in favor of males (P<0.05), but not in VLDA. The incidences of dactylitis, enthesitis, tenosynovitis, and inflammatory bowel disease were similar in male and female patients, except for spondylitis, which was higher in males (P<0.05). Overall, although there was no significant between-group difference in age and disease duration, female patients had significantly higher BMI and late-onset disease (P<0.05). Female patients had higher HAD, HAQ, and FiRST and lower SF-36 scores than males (P<0.05). In both male and female patients, the disease activity score of cDAPSA was significantly correlated with the scores of FiRST, HAD, VAS-F, and HAQ(P< 0.05). In regression analysis, tender joint count, swollen joint count, PASI, pain VAS, and enthesitis were the MDA predictors in both genders. Conclusion: In patients with peripheral PsA, males are more likely to develop spondylitis while other extraarticular manifestations are similar. Female patients appear to have lower rates of remission and MDA and higher levels of disease activity. Female patients experience a more severe course of PsA, with higher levels of pain and fatigue, lower quality of life, and increased functional limitations. The predictors of MDA, i.e., tender joint count, swollen joint count, PASI, pain VAS, and enthesitis are similar between the two genders. (C) 2021 Societe francaise de rhumatologie.
