    Clinical Characteristics and Distribution of Pediatric Fractures at a Tertiary Hospital in Northern France: A 20-Year-Distance Comparative Analysis (1999–2019)

    Background and objectives: The epidemiology and distribution of pediatric fractures change over time and are influenced by a multitude of factors including geography, climate, and population characteristics. The aims of our work were to study the distribution of traumatic pediatric orthopedic injuries admitted to the Lille University Hospital (LUH) Pediatric Emergency Department in 1999 and in 2019 and to analyze the epidemiological differences 20 years apart. Materials and methods: This was a retrospective, comparative, monocentric, and epidemiological study involving all children between 0 and 15 years and 3 months of age who consulted the pediatric emergencies of LUH from 1 January 1999 to 31 December 1999 and from 1 January 2019 to 31 December 2019. On admission, the following data were collected: sex, age at the time of injury, month and time of the day the trauma occurred (4:00 a.m to 11:59 a.m, 12:00 p.m. to 19:59 p.m, and 20:00 p.m to 3:59 a.m.), mechanism of injury, laterality (right or left), anatomical location, type of injury, and whether the fracture was closed or open. The type of treatment (orthopedic or surgical) was collected from the medical records. Results: A total of 939 children were included in 1999 compared with 781 in 2019 (21% decrease); the average age of children with fractures was significantly higher in 1999 (8.81 years) than in 2019 (7.19 years). This difference was explained by the majority involvement of older children (10–15 years) in 1999 (43% of fractures in 1999 versus 25% of fractures in 2019). Conversely, small children (1–5 years) had significantly more fractures in 2019 (36%) than in 1999 (24%). Conclusions: Overall, the types and sites of fractures did not change over the studied time despite a change in the population and mechanism of injury. This suggested that the reflexes of breaking a fall still tended to implicate and damage the same bone segments. Finally, the proportion of fractures managed surgically versus orthopedically has not evolved since 1999. Exploring this is a possible area of further research that would complement our study

    Comparison between anterior cuneiform osteotomy and Dunn procedure in the surgical management of unstable severe slipped femoral epiphysis: a case-control study of 41 patients

    International audienceBackgroundThere is little consensus on the best treatment algorithm for unstable severe slipped capital femoral epiphysis (SCFE). Subcapital osteotomy, which is one of the surgical options, is performed either anteriorly (anterior cuneiform osteotomy, CO) or laterally with trochanteric osteotomy (Dunn procedure, DP). The CO is technically easier and decreases operating time. Moreover, because the DP was the standard in our department before it was replaced by the CO, we had a series of consecutive patients. Therefore, we did a retrospective case-control study in unstable, severe SCFEs treated by CO versus DP, which is to our knowledge the first one aiming to compare: (1) postoperative complications and in particular avascular necrosis, (2) functional outcome, (3) radiologic findings.HypothesisCO is less or just as likely to cause avascular necrosis and has the same clinical and radiologic findings as DP.MethodsA total of 41 patients (24 girls, i.e. 58.5%) were included between 2005 and 2018: 23 in the CO group and 18 in the DP group. The median age was 12.9 years (range, 11.5–14.9) and the median slip angle 70̊ (range, 62.5̊–80̊) with a median follow-up of 3 years (range, 2–4). Preoperative, intraoperative, and postoperative clinical and radiologic parameters (Southwick and alpha angles, and femoral head-neck offset) were analyzed, and all complications were documented.ResultsTwo (8.7%) cases of avascular necrosis were reported in the CO group and 6 (33.3%) in the DP group (p = .11), with an overall rate of avascular necrosis of 19.5% (8/41). Five out of the 41 patients (12.2%) underwent a total hip arthroplasty: 1/23 (4.3%) in the CO group and 4/18 (22.2%) in the DP group (p = .16). Two (9.5%) patients in the CO group and 7 (38.9%) in the DP group developed postoperative limping before any arthroplasty was performed (p = .055). The alpha angle at follow-up (54 ± 6.1̊ vs. 59.1 ± 7.2̊; p = .027), Oxford hip score at follow-up (17/60 [range, 14–20] vs. 23.5 [range, 19–27]) (p = .021), operating time (132 min [range, 103–166] vs. 199.5 min [range, 142–215]) (p = .011) and intraoperative bleeding (250 mL [range, 100–350] vs. 300 mL [range, 197–450]) (p = .088) were more favorable in the CO group than in the DP group.ConclusionsThe CO has similar results to DP in the surgical management of unstable severe SCFE

    RÉSUMÉ • Introduction : Les scolioses infantiles et juvéniles représentent, parmi les déformations rachidiennes, un défi pour les chirurgiens orthopédistes pédiatriques. Si le traitement par corsets plâtrés est actuellement considéré comme la méthode de référence dans les scolioses infantiles (SI) idiopathiques, son efficacité dans les scolioses juvéniles (SJ) et congénitales (SC) est beaucoup plus discutée, et moins étudiée. Le but de cet article est de faire l’état des lieux via une revue de la littérature des résultats du traitement par corsets plâtrés chez les patients avec une SI, SJ ou SC en abordant les complications décrites au sein de ces études. Méthodes: Nous avons effectué la recherche au sein des bases de données PUBMED Medline et EMBASE allant de Janvier 1955 à Janvier 2021, avec les termes de recherche suivants : • congenital coliosis/scoliose congénitale • early onset scoliosis/scoliose à début précoce • serial casting/corsets successifs • EDF casting/corset EDF • Risser frame/cadre de Risser • Cotrel frame/cadre de Cotrel • et children/enfants ; les limites suivantes ont été utilisées : • langue anglaise • langue française et • tous les enfants de 0 à 10 ans. Les résultats de chaque article ont été compilés au sein d’un tableau comparatif détaillant pour chacun, la date de publication, les auteurs, le type de scoliose étudié (SI et SJ, idiopathique ou syndromique, et SC), l’âge moyen des patients au début du traitement, le taux de correction (angle de Cobb) et le délai entre la fin du traitement et l’éventuelle prise en charge chirurgicale. Les complications retrouvées dans ces études ont également été répertoriées, comptabilisées et classées. Résultats: Un total de 32 articles a été analysé. La réalisation de corsets plâtrés successifs est une alternative efficace à la chirurgie chez les patients atteints de scoliose à début précoce. Sa réalisation est systématiquement associée à une diminution de l’angle de Cobb moyen chez les patients atteints de SI idiopathique ou non. Son efficacité dans les SJ et SC est plus discutée mais elle permet de retarder la prise en charge chirurgicale au prix de peu de complications, majoritairement réversibles et peu sévères. Conclusion: Aucune étude au sein de ce travail ne fait état de contre-indication ou de complications sévères impliquées par la réalisation de corsets plâtrés dans les scolioses à début précoce. Le traitement par corsets plâtrés successifs permet de retarder le traitement chirurgical dont le taux de complications est plus élevé. En règle générale, l’efficacité du traitement par corset est d’autant plus importante que l’enfant est jeune (< 2 ans), que le traitement est mis en place tôt et sur de faibles courbures initiales

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
