I, Quaglio G, Miró JM, Paré C, Azqueta M, Marco F, Mestres CA, Moreno A, Pomar JL, Mezzelani JL, Sanz G. Characteristics of aortic periannular abscesses studied with transesophageal echocardiography. A report from the ICE-ME database. The study of periannular complications in infective endocarditis. Analysis of clinical, microbiological, echocardiographic, therapeutic and prognostic aspects of patients with non-ruptured periannular abscesses and with aorto-cavitary fistulous tract formation in IE"Infective endocarditis (IE) is a devastating disease associated with high death rates without appropriate treatment. The prognosis of IE has changed dramatically during the last three decades due to the use of potent antibiotics and the generalization of surgical therapies of valve repair. Infective endocarditis is characterized by the infection of the endothelium of cardiac valves caused by virulent organisms, and the elevated morbidity and mortality is produced by the destruction of valvular tissue and adjacent structures. In the modern era, one-year mortality of native valve IE is in the range of 20% - 30%, and 10-year survival is in the range of 70%-80%. Spread of infection in IE from valvular structures to the surrounding perivalvular tissue results in periannular complications. Periannular complications, including periannular abscess formation and aorto-cavitary fistulous (ACF) tract formation have not been adequately studied, and may place the patient at increased risk of adverse outcomes including heart failure (HF) and death. Aorto-cavitary fistulous tract formation has only been described occasionally. The main objectives of the present investigation were to investigate the clinical characteristics and the prognosis of patients with non-ruptured periannular abscesses and with aorto-cavitary fistulous tract formation in IE and to determine the prognostic impact of fistulization in patients with periannular lesions in the setting of IE.Several manuscripts including a large number of patients with periannular lesions have been published describing the clinical characteristics and prognostic factors of mortality of patients with periannular lesions in IE.Aorto-cavitary fistula is an uncommon complication of aortic IE associated with extensive aortic root destruction and presence of periannular abscesses. Transesophageal echocardiography is the diagnostic tool of choice for the detection of aorto-cavitary fistulae in both native and prosthetic valves. Despite aggressive surgical treatment, rates of heart failure and mortality are high. Prosthetic IE, urgent surgery and the development of heart failure were associated with an increased risk of death in patients with endocarditis complicated by fistulous tract formation. ACF formation in the course of IE is associated with higher rates of aortic annulus destruction and heart failure compared to non-ruptured abscess. Surgery and mortality rates for both complications were very high; however, despite the higher complications, ACF in the current era of high rates of surgical therapy was not an independent risk factor for mortality.Periannular abscess formation occurs frequently in aortic valve IE, is associated with staphylococcal etiology, and has a high morbidity and mortality. In the current era of TEE imaging and high use of surgical treatment periannular abscess formation in aortic valve IE is not an independent risk factor for mortality. Finally, S. aureus infection constitutes an independent prognostic factor for mortality in patients who develop an abscess

Dark side of comet 67P/Churyumov-Gerasimenko in Aug.-Oct. 2014 MIRO/Rosetta continuum observations of polar night in the southern regions

No full text
International audienceThe high obliquity (similar to 50 degrees) of comet 67P/Churyumov-Gerasimenko (67P) is responsible for a long-lasting winter polar night in the southern regions of the nucleus. We report observations made with the submillimeter and millimeter continuum channels of the Microwave Instrument onboard the Rosetta Orbiter (MIRO) of the thermal emission from these regions during the period August-October 2014. Before these observations, the southern polar regions had been in darkness for approximately five years. Subsurface temperatures in the range 25 50 K are measured. Thermal model calculations of the nucleus near-surface temperatures carried out over the orbit of 67P, coupled with radiative transfer calculations of the MIRO channels brightness temperatures, suggest that these regions have a thermal inertia within the range 10-60 Jm(-2) K-1 s(-0.5). Such low thermal inertia values are consistent with a highly porous, loose, regolith-like surface. These values are similar to those derived elsewhere on the nucleus. A large difference in the brightness temperatures measured by the two MIRO continuum channels is tentatively attributed to dielectric properties that differ significantly from the sunlit side, within the first few tens of centimeters. This is suggestive of the presence of ice(s) within the MIRO depths of investigation in the southern polar regions. These regions started to receive sunlight in May of 2015, and refinements of the shape model in these regions, as well as continuing MIRO observations of 67P, will allow refining these results and reveal the thermal properties and potential ice content of the southern regions in more detail

\ua9 2022, The Author(s), under exclusive licence to Springer Science+Business Media, LLC, part of Springer Nature.Studies showed that JM-20, a benzodiazepine-dihydropyridine hybrid molecule, protects against rotenone and 6-hydroxydopamine neurotoxicity. However, its protective effects against cytotoxicity induced by endogenous neurotoxins involved in Parkinson’s disease (PD) pathogenesis have never been investigated. In this study, we evaluated the ability of JM-20 to inhibit alpha-synuclein (aSyn) aggregation. We also evaluated the interactions of JM-20 with aSyn by molecular docking and molecular dynamics and assessed the protective effect of JM-20 against aminochrome cytotoxicity. We demonstrated that JM-20 induced the formation of heterogeneous amyloid fibrils, which were innocuous to primary cultures of mesencephalic cells. Moreover, JM-20 reduced the average size of aSyn positive inclusions in H4 cells transfected with SynT wild-type and synphilin-1-V5, but not in HEK cells transfected with synphilin-1-GFP. In silico studies showed the interaction between JM-20 and the aSyn-binding site. Additionally, we showed that JM-20 protects SH-SY5Y cells against aminochrome cytotoxicity. These results reinforce the potential of JM-20 as a neuroprotective compound for PD and suggest aSyn as a molecular target for JM-20

The fate of active left-side infective endocarditis with operative indication in absence of valve surgery

Objectives Although the benefits of surgery in infective endocarditis (IE) are clear, an unneglectable proportion of patients do not undergo surgery despite clear operative indication. Outcomes of these patients are poorly reported. With this study, we aim to analyze patient profiles, indication for surgery, decision-making, and outcomes of patients not undergoing surgery despite contemporary surgical indications. Materials and Methods Retrospective review of single institution database of patients with IE was done to identify patients that, although a clear surgical indication did not receive surgery. We aimed to review the most contemporary practice from June 2014 to December 2018. Only patients who were physically evaluated at our facility were included. Follow up was 100% complete. Kaplan-Meier methods were used to estimate survival and freedom from a composite outcome of death, stroke, and heart failure. Results Of the 174 patients with surgical indication during the review period, 46 (27%) did not undergo surgery. The reasons for not pursuing surgery were varied and usually multiple, with severe brain injury and end stage liver disease between the most common. The 30-day mortality was 63%, and the estimated survival at 6 months, 1 year, and 2 years was respectively 22%, 15%, and 10%. Conclusion The mortality of this cohort of patients is extremely high. A multidisciplinary evaluation is of paramount importance in the decision-making process with shared responsibility for denial of operative options. In a perspective of correct healthcare resources allocation an early palliative care consult may need to be considered in some of those patients

Uterine transplantation: a promising surrogate to surrogacy? Grynberg M1, Ayoubi JM, Bulletti C, Frydman R, Fanchin R. Author information Abstract Infertility due to the inability of the uterus to carry a pregnancy ranks among the most unresolved issues in reproductive medicine. It affects millions of women worldwide who have congenital or acquired uterine affections, often requiring hysterectomy, and potentially represents a considerable fraction of the general infertile population. Patients suffering from severe uterine infertility are currently compelled to go through gestational surrogacy or adoption; both approaches, unfortunately, deprive them of the maternal experience of pregnancy and birth. Uterine transplantation represents an outstanding, yet complex, perspective to alleviating definitive uterine infertility. In the past decades, a number of scientific experiments conducted both in animals and women, focusing on uterine transplantation, have led to promising results. Collectively, these findings undoubtedly constitute a sound basis to clinically apply uterine transplantation in the near future. This paper is, however, an overview not only of the extent and limitations of accumulated scientific knowledge on uterine transplantation, but also its ethical implications, in an effort to define the actual place of such an approach among the therapeutic arsenal for alleviating infertility. © 2011 New York Academy of Sciences

Translation and interpretation: Translation redundancy reconsidered

No full text
Interpretation is an integral part of the process of translating. This article raises the question of whether interpretation fo a literary work by a translator should be guided by extratextual factors or not. The discussion is illustrated with examples taken from David Hawkes' translation of a Chinese classic, A Dream of Red Mansions. As the work of a scholar-translator, Hawkes' version is richly supplemented with disclosures concerning the characters and explanations of the cultural environment embodied in the novel. In many cases, however, this translation procedure is redundant and explanatory, enlightening the readers but at the same time robbing them of the pleasure of literary interpretation and cultural exploration. By means of this illustration of translation redundancy, the author points out that there is difference between a scholar who helps the interpretation fo a work and a translator who presents a work close to its original version. It is particularly important to pay attention tot his difference in literary translations in a cross-cultural situation involving two enormously different cultures.Language & LinguisticsA&HCI0ARTICLE1,SI115-12
