The political economy of work security and flexibility : Italy in comparative perspective
The recent economic crisis has shown us the dark side of deregulating the labor market: rising unemployment, limited access to social security and, due to low wages, depleted savings to rely upon in bad times. An emphasis on flexibility has led to inequality and insecurity, Fabio Berton, Matteo Richiardi, and Stefano Sacchi reveal in this compelling study of Italy’s embrace of nonstandard work contracts. Analyzing outcomes at the individual level, the authors contrast Italy with Germany, Spain, and Japan as they demonstrate the social and economic fallout of pursuing flexibility merely as a tool to reduce costs

Efficacy of intravitreal dexamethasone implant for prostaglandin-induced refractory pseudophakic cystoid macular edema: case report and review of the literature
Background: Macular edema is a known complication even after uneventful cataract surgery. The chronic use of prostaglandin analogs is a risk factor for the development of pseudophakic cystoid macular edema (CME). Nonsteroidal anti-inflammatory drugs (NSAIDs) are considered first-line therapy but refractory postsurgical CME represents a therapeutic challenge, as there is not an evidence-based treatment. Objective: To report the use of a single implant of intravitreal dexamethasone for tafluprost-associated pseudophakic CME refractory to NSAIDs and to sub-Tenon's corticosteroid injections. Case report: A 64-year-old female with ocular hypertension treated with tafluprost experienced decreased vision (visual acuity 20/60) and metamorphopsia 2 months after uneventful cataract extraction. Spectral domain optical coherence tomography (SD-OCT) revealed CME. After 1 month of topical and oral NSAIDs, CME was still evident on SD-OCT (visual acuity 20/50). Two sub-Tenon's betamethasone injections were performed at a 2-week interval. As CME was still present, 2 months after the diagnosis of CME (visual acuity 20/40), the patient underwent a single dexamethasone intravitreal implant. One month later, macular appearance was normal, and visual acuity increased to 20/30. This result was maintained throughout the 6 months of follow-up. Conclusion: In this report, a single implant of intravitreal dexamethasone successfully treated pseudophakic CME associated with the use of prostaglandin analogs unresponsive to NSAIDs and sub-Tenon's betamethasone. The results of this report need to be corroborated by powered, prospective, randomized trials. The need for repeated treatments as well as the retreatment interval in patients requiring more than a single injection are issues still needing further investigations
Endonasal Dacryocystorhinostomy in a Pediatric Patient With Ectrodactyly-ectodermal Dysplasia After Failure of External Dacryocystorhinostomy and as First Procedure
A 9-year-old patient affected by ectrodactyly-ectodermal dysplasia was referred to our clinic complaining of epiphora in both eyes. She had previous external dacryocystorhinostomy in her left eye. Ophthalmic examination revealed absence of the superior tear ducts and discharge in both eyes. Lacrimal irrigation through the inferior canaliculi revealed nasolacrimal duct obstruction on both sides. A bilateral endo-dacryocystorhinostomy was performed as a revision procedure in the left eye and as first procedure in the right eye. A monocanalicular silicone stent through the inferior tear duct was introduced on both sides at the end of the surgery. Follow up was 8 and 5 months for the left and the right eye, respectively. Epiphora was absent, and lacrimal irrigation showed a patent osteum at last follow-up. This case suggests that endo-dacryocystorhinostomy seems to be a safe and effective procedure, both as revision and as first procedure, even in children with ectrodactyly-ectodermal dysplasia
Localization of the bacterial symbiont Candidatus Midichloria mitochondrii within the hard tick Ixodes ricinus by whole-mount FISH staining.
Here, we present an investigation on the spatial distribution of the bacterial symbiont Candidatus Midichloria mitochondrii within Ixodes ricinus, by whole mount fluorescence in situ hybridization (FISH). M. mitochondrii is a peculiar, recently discovered bacterium that resides in the mitochondria of female ticks. We applied a rapid and specific FISH protocol with oligonucleotide probes targeted on the 16S rRNA of M. mitochondrii, 12S rRNA of tick mitochondria, and a probe revealing active mitochondria. In this report that represents the first application of whole mount FISH on ticks, we observed strong, specific fluorescence signals in all the examined life stages, as the optimized protocol allowed us to overcome the autofluorescence interference of the cuticle. Cellular localization and quantification of the symbionts were also assessed with electron microscopy and specific real-time PCR, respectively

Infortuni a rischio biologico nel personale sanitario: analisi epidemiologica descrittiva nel decennio 1994-2003
The study evaluates the body fluid exposure among health care workers (HCV) at IRCCS Policlinico San Matteo of Pavia from the 1994 through 2003 years. Our data showed that all HCV fro less than 5 years of employment, nurses, midwifes and surgeons are the most exposed occupational groups. Some variables were compared with a previous study performed from 1983 to 1994 at the same institution. the study documented 1 case of seroconversion to hepatitis C virus among a physician after percutaneous exposure. In order to reduce the body fluid exposure among the HCV, the authors underline the importance of administrative and educational interventions carried out in collaboration with the Infection Control Committee and Occupational Medicine Service
