Logopedist role in childhood cerebral palsy – case report

ABSTRACT:
This is a case study of a 5 years old girl, born as a premature child (27 weeks of pregnancy - extreme prematurity) with a very low birth weight and diagnosed with childhood cerebral palsy and drug-resistant epilepsy. In physical examination was shown: increased thoracic kyphosis, lopsidedness, contracted thoracic muscles, sciatica shin, hip-lumbar muscles, lower limbs in internal rotation. Girl should not be self-service and requires the help of a second person. She is circulatory efficient. She has also limb paresis, sensory disorders, perception, speech development disorders and eye damage.The girl has undergone logopedic therapy. Initially, the adaptation to the new environment and speech therapist was very hard.
The observations, help in various situations and various games have made the therapy successful. During the therapy, it was noticed that the girl was sitting incorrectly. The parents also reported problems with acceptance of louder sounds and music (confirmed by the therapist and psychologist), which, however, improves during the therapy. Body motor skills in the described patient is very weak. Since the beginning of the therapy, a full logopedic examination was conducted and specialist tests were performed, which were evaluated on the basis of established scales. The girl is also under physiotherapist's care. The evaluation of the course of therapy shows the progress, and on the basis of the results of examinations and observations, a plan of further therapy has been prepared

ADRIAN POP - IN SEARCH OF A LONG LOST SMILE
An anniversary, especially in the case of a generation colleague, the renowned composer Adrian Pop, generates the remembrance of a long series of musical events that have become notorious, with works included in the national and international concert repertoire. The avoidance of nostalgic memories made that a stage miniature be chosen for the present paper, an incursion of the author into the naive world of childhood reading. The result was a modern, glamorous musical transposition of a sequence with Max and Moritz, the playful heroes created by the famous humorist and cartoonist Wilhelm Busch in the middle of the 19th century. The musical act Onkel Fritz by Adrian Pop was first performed in Cluj in 2016, on October 22, on the occasion of his previous anniversary, within the Cluj Musical Autumn Festival. The graceful performers of the act, presented under the title “Anniversaries at the Festival, Adrian Pop compositional portrait”, were soprano Mihaela Maxim, in the role of Max and pianist Eva Butean, in the role of Moritz. How did composer Adrian Pop manage to musically enliven a 150 years old humorous story? The author of the following text will try to answer this question.
Rezumat. ADRIAN POP - ÎN CĂUTAREA SURÂSULUI PIERDUT. O aniversare, mai ales în cazul unui coleg de generație, reputatul compozitor Adrian Pop, generează rememorarea unui lung șir de evenimente muzicale devenite notorii, cu lucrări intrate în repertoriul intern și internațional de concerte. Evitarea unor memorii nostalgice au făcut ca sorții acestei comunicări să cadă asupra unei miniaturi scenice, o incursiune a autorului în lumea ingenuă a lecturilor din copilărie. A rezultat o transpunere muzicală modernă, plină de farmec, a unei secvențe cu Max și Moritz, eroii poznași creați de celebrul umorist și caricaturist Wilhelm Busch la mijlocul secolului XIX. Prima audiție clujeană a scenetei muzicale Onkel Fritz de Adrian Pop a avut loc în 2016, la 22 octombrie, cu ocazia precedentei sale aniversări, în cadrul Toamnei Muzicale Clujene. Interpreții plini de har ai scenetei prezentate sub genericul „Aniversări în Festival, portret componistic Adrian Pop” au fost soprana Mihaela Maxim, în rolul lui Max și pianista Eva Butean, în rolul lui Moritz. Oare cum a reușit compozitorul Adrian Pop să însuflețească muzical o poveste umoristică de 150 de ani? La această întrebare va încerca să răspundă autoarea acestor rânduri.

Geothermal utilization of a bored pile wall in the Lainzer Tunnel – Numerical modeling, comparison with measurements and fundamental influences on the structural design
Die vorliegende Diplomarbeit beschäftigt sich einerseits mit der Untersuchung des thermischen Verhaltens der langjährig geothermisch genutzten Bohrpfahlwand des Lainzer Tunnels im Abschnitt LT24 Hadersdorf-Weidlingau und andererseits mit dem Einfluss einer geothermischen Nutzung von Bohrpfählen auf deren statisch-konstruktive Bemessung. Beim Bau des Lainzer Tunnels Anfang der 2000er Jahre wurde im Abschnitt LT24 eine Bohrpfahlwand mit Absorberrohren geothermisch aktiviert. Dieser Prozess und das erste Jahr der Anlage in Betrieb wurden von Markiewicz im Rahmen seiner Dissertation wissenschaftlich begleitet. Somit gibt es seit dem Jahr 2004 Messungen des Temperaturverhaltens dieser Bohrpfahlwand. Dieser Datensatz ist ein Alleinstellungsmerkmal. Ziel dieser Arbeit ist es, eine numerische Vergleichsmodellierung für die Heizperiode 2005/06 mit dem Finite-Elemente-Programm COMSOL Multiphysics anzustellen und damit die Messwerte abzubilden. Dabei soll die Modellierung durch bekannte Parameter und Messwerte, wie der Außenlufttemperatur und der Temperatur im Pfahl an verschiedenen Stellen validiert werden. Zur Kalibrierung dienen Parameter, die bekannt, allerdings mit gewissen Unsicherheiten behaftet sind, sowie Parameter, die erst durch die Modellierung angenähert werden. Dazu zählen unter anderem die Anfangstemperatur des Bodens, die Tunnelinnenlufftemperatur, die Strömungsgeschwindigkeit der Absorberflüssigkeit sowie die Vorlauftemperatur. Die Annäherung erfolgt in einem iterativen Prozess. Anschließend wird durch Variation einzelner Variablen gezeigt, welchen Einfluss diese auf das Gesamtsystem sowie den Wärmeertrag haben. Im zweiten Abschnitt wird der Einfluss einer geothermischen Nutzung auf die statisch-konstruktive Bemessung behandelt. Dabei wird zunächst eine Auflistung der notwendigen Nachweise zur Bemessung eines Bohrpfahls gebracht. Anschließend folgen Überlegungen, welche Parameter dieser Nachweise durch eine geothermische Nutzung des Bauteils beeinflusst werden. Am Ende des Kapitels wird aufgezeigt welche Ansätze es zur Berücksichtigung einer geothermischen Nutzung in der Bemessung bereits gibt und wie diese in internationalen Normen und Richtlinien verankert sind. Dies soll die Planung und Ausführung künftiger Anlagen unterstützen.The present diploma thesis deals on the one hand with the investigation of the thermal behavior of the bored pile wall of the Lainzer Tunnel in section LT24 Hadersdorf-Weidlingau, which has been geothermally utilized for many years, and on the other hand with the influence of a geothermal activation of bored piles on their structural design. During the construction of the Lainzer Tunnel in the early 2000s, a bored pile wall in section LT24 was geothermally activated with heat exchanger pipes. This process and the first year of the system in operation were scientifically monitored by Markiewicz as part of his thesis. Thus, there are numerous measurements of the temperature behavior of this bored pile wall from 2004 on. This data set has a unique characteristic. The aim of this work is to perform a comparative numerical model for the heating period 2005/06 with the finite element software COMSOL Multiphysics and to reproduce the measured values. The model is to be validated by known parameters and values, such as the outside air temperature and the temperature in the pile at different locations. Parameters that are known, but subject to certain uncertainties, as well as parameters that are only approximated through modeling are used for calibration. These include, among others, the initial temperature of the soil, the air temperature inside the tunnel, the flow velocity of the heat exchanger fluid, and the inlet temperature. The approximation is done in an iterative process. Subsequently, by varying individual variables, it is shown what influence they have on the overall system as well as the heat output. In the second section, the influence of geothermal activation on the structural design is discussed. First, a list of the necessary verifications for the design of a bored pile is presented. This is followed by considerations which parameters in the design are influenced by geothermal utilization of the structural element. At the end of the chapter, the approaches for the consideration of geothermal use are shown and how they are already anchored in international standards and guidelines. This should support the design and execution of future plants
