Free labeling of facial expressions of emotions in children surviving the terrorist attack in Beslan's school.
No full textBackground: The identification of facial emotional expressions is a crucial component of social and emotional development. Although recent studies indicate that terrorism induced trauma has a negative impact on child psychological functioning, no previous studies have ever investigated the effects of terrorism on children’s face emotion processing. This study reports pilot information on free labeling of facial expressions of emotions in a group of severely traumatized children surviving the terrorist attack in Beslan 3 months after their school siege. These children were compared with a group of non exposed children matched by age and gender.
Method: Participants were 66 children with a mean age of 9.85 years (SD=1.33) who were either exposed or not exposed to the terrorist attack in Beslan, Russia. All children surviving the terrorist attack reported symptoms of post traumatic stress, whereas non-exposed children did not. Children were tested on free labeling of 10 facial expression stimuli representing happiness, sadness, anger, disgust, fear and surprise. All responses were judged by two raters on valence and specific emotion category. Uninterpretable or nonsensical responses were labeled as “non responses”.
Results: Children of both groups equally recognized both the positive valence and the correct specific category of happiness. However, significant differences were found in responses to negative emotions stimuli. When considering the valence (positive versus negative) of the emotion expressed by the face stimuli, terrorism exposed children more frequently attributed a negative valence to anger, whereas non exposed children more frequently attributed a negative valence to fear. When considering the correct specific category of the face stimuli, exposed children produced a significantly lower proportion of fear specific labels when needed, suggesting that they were less accurate in recognizing fear compared to non exposed children. The qualitative analysis of the “non responses” category revealed the presence of other labels that significantly differed between the two groups. For example, terrorism exposed children frequently mentioned suspect (e.g., “she does not trust someone”) and offence, whereas not exposed children often described face stimuli as thoughtful (e.g., “she is thinking what to do next”) or concentrated.
Conclusions: These results suggest that terrorism and PTSD may impact on children’s perception of facial expression of negative emotions. Findings are discussed in terms of their implications for child development; clinical suggestions are given for working with children surviving terrorism
Diagnosis and workup of 522 consecutive patients with neuroendocrine neoplasms in Switzerland.

Neuroendocrine neoplasms (NENs) are difficult to diagnose. We used SwissNET data to characterise NEN patients followed in the two academic centres of western Switzerland (WS), and to compare them with patients followed in eastern Switzerland (ES) as well as with international guidelines.
METHOD
SwissNET is a prospective database covering data from 522 consecutive patients (285 men, 237 women) from WS (n = 99) and ES (n = 423).
RESULTS
Mean ± SD age at diagnosis was 59.0 ± 15.7 years. Overall, 76/522 experienced a functional syndrome, with a median interval of 1.0 (IQR: 1.0-3.0) year between symptoms onset and diagnosis. A total of 51/522 of these tumours were incidental. The primary tumour site was the small intestine (29%), pancreas (21%), appendix (18%) and lung (11%) in both regions combined. In all, 513 functional imaging studies were obtained (139 in WS, 374 in ES). Of these, 381 were 111In-pentetreotide scintigraphies and 20 were 68Ga-DOTATOC PET. First line therapy was surgery in 87% of patients, medical therapy (biotherapy or chemotherapy) in 9% and irradiation in 3% for both regions together.
CONCLUSION
Swiss NEN patients appear similar to what has been described in the literature. Imaging by somatostatin receptor scintigraphy (SRS) is widely used in both regions of Switzerland. In good accordance with published guidelines, data on first line therapy demonstrate the crucial role of surgery. The low incidence of biotherapy suggests that long-acting somatostatin analogues are not yet widely used for their anti-proliferative effects. The SwissNET initiative should help improve compliance with ENETS guidelines in the workup and care of NEN patients

Pautamo-nos numa oficina desen- volvida com alunos com deficiência visual e videntes, na qual a pretensão foi desenvolver o ensino da arte, tornando acessível à linguagem contemporânea e realizar uma análise do desenvolvimento escolar desses alunos. Como resultado percebemos a eficácia dos Objetos Pedagógicos

Identifying flares in rheumatoid arthritis: Reliability and construct validation of the OMERACT RA Flare Core Domain Set
No full textObjective: To evaluate the reliability of concurrent flare identification using 3 methods (patient, rheumatologist and Disease Activity Score (DAS)28 criteria), and construct validity of candidate items representing the Outcome Measures in Rheumatology Clinical Trials (OMERACT) RA Flare Core Domain Set. Methods: Candidate flare questions and legacy measures were administered at consecutive visits to Canadian Early Arthritis Cohort (CATCH) patients between November 2011 and November 2014. The American College of Rheumatology (ACR) core set indicators were recorded. Concordance to identify flares was assessed using the agreement coefficient. Construct validity of flare questions was examined: convergent (Spearman's r); discriminant (mean differences between flaring/non-flaring patients); and consequential (proportions with prior treatment reductions and intended therapeutic change postflare). Results: The 849 patients were 75% female, 81% white, 42% were in remission/low disease activity (R/LDA), and 16-32% were flaring at the second visit. Agreement of flare status was low-strong (κ's 0.17-0.88) and inversely related to RA disease activity level. Flare domains correlated highly (r's≥0.70) with each other, patient global (r's≥0.66) and corresponding measures (r's 0.49-0.92); and moderately highly with MD and patient-reported joint counts (r's 0.29-0.62). When MD/patients agreed the patient was flaring, mean flare domain between-group differences were 2.1-3.0; 36% had treatment reductions prior to flare, with escalation planned in 61%. Conclusions: Flares are common in rheumatoid arthritis (RA) and are often preceded by treatment reductions. Patient/MD/DAS agreement of flare status is highest in patients worsening from R/LDA. OMERACT RA flare questions can discriminate between patients with/without flare and have strong evidence of construct and consequential validity. Ongoing work will identify optimal scoring and cut points to identify RA flares
