Abdominal obesity vs general obesity for identifying arterial stiffness, subclinical atherosclerosis and wave reflection in healthy, diabetics and hypertensive
[SP]Antecedentes
Nuestro objetivo fue analizar la relación entre la obesidad abdominal y la obesidad general, con la aterosclerosis subclínica, la rigidez arterial y la reflexión de la onda en sujetos sanos, diabéticos e hipertensos.
Métodos
Se realizó un estudio descriptivo transversal en 305 individuos (diabéticos 32,8%, hipertensos 37,0% e individuos sanos 30,2%). Mediciones: índice de masa corporal (IMC), circunferencia de cintura (CC), porcentaje de grasa corporal (PCC) y relación cintura/talla (RCT). La rigidez arterial se evaluó según la velocidad de la onda de pulso (VOP), el grosor íntima-media de la arteria carótida común (GIM-C), el índice de aumento (central y periférico), el índice tobillo-brazo (ITB) y la presión de pulso central y periférica.
Resultados
La CC y el RCT mostraron una correlación positiva con la VOP y el GIM-C en los grupos estudiados. Después de ajustar por edad, sexo, proteína C reactiva de alta sensibilidad, glucosa sérica y presencia de diabetes, hipertensión, tabaquismo, dislipidemia, fármacos antidiabéticos, fármacos hipolipemiantes y placas ateroscleróticas, se observó que por cada aumento de 0,1 puntos en el WHtR y por cada aumento de cm en el CC, la VOP aumentó 0,041 y 0,029 m/seg, y el GIM-C aumentó 0,001 mm y 0,001 mm, respectivamente.
Conclusiones
Las medidas de obesidad abdominal (WHtR y CC) se correlacionan mejor que el IMC y el PBF con la rigidez arterial evaluada por la VOP y con la aterosclerosis subclínica evaluada por el GIM-C, independientemente de la presencia de diabetes o hipertensión.

Análisis de las medidas de adiposidad, de glucemia y componentes del síndrome metabólico con la rigidez arterial en sujetos con riesgo cardiovascular intermedio. Estudio Mark.
[ES]Los objetivo de mi tesis doctoral son:
1.Analizar si la asociación entre el CAVI y parámetros de estructura y función vascular en adultos es independiente del RCV y del tratamiento farmacológico utilizado.
2. Analizar la relación existente entre el síndrome metabólico y sus componentes con la rigidez arterial, medida con la VOP brazo tobillo y con el CAVI, en adultos de raza caucásica con RCV intermedio y analizar las diferencias por género.
3. Analizar la relación existente entre la glucemia plasmática en ayunas, la glucemia postprandial y la HbA1c con la rigidez arterial, medida con la VOP brazo tobillo y con el CAVI, en adultos de raza caucásica con RCV intermedio y analizar esta relación según el status de hiperglucemia.
4. Analizar la relación de diferentes medidas de adiposidad con la rigidez arterial, medida con la VOP brazo tobillo y con el CAVI, en adultos de raza caucásica con RCV intermedio y determinar qué medida tiene mayor poder explicativo de la rigidez arterial.
Para dar respuesta a los mismo se ha realizado un estudio trasversal con los sujetos incluidos en el estudio MARK. Cada uno de los objetivos ha sido pulicado en un manuscrito en revistas de 1 y 2 cuartil. Los manuscritos publicados son:

Las conclusiones principales son:
1ª La asociación positiva del CAVI con los parámetros de estructura y función vascular es independiente del RCV y tratamiento farmacológico realizado. La capacidad del CAVI para predecir arterioesclerosis carotidea es similar a la de la VOP carótida femoral y a la de la VOP brazo tobillo en adultos de raza caucásica.
2ª El síndrome metabólico y la mayor parte de sus componentes se asociaron con la VOP brazo tobillo y con el CAVI, (excepto el colesterol HDL con la VOP brazo tobillo y con el CAVI y la circunferencia de la cintura con la VOP brazo tobillo). Sin embargo, esta asociación es diferente por género. Los datos sugieren una mayor asociación de los valores del CAVI y de la VOP brazo tobillo con los componentes del SM en hombres que en mujeres. También indican una mayor rigidez arterial cuando los componentes que se asocian para definir el SM fueron la presión arterial, la glucemia en ayunas y la circunferencia de la cintura elevada.
3ª La glucemia basal, glucemia postprandial y HbA1c muestran asociación con el CAVI y con la VOP brazo tobillo, independiente de otros factores de confusión en adultos de raza caucásica con RCV intermedio. En el análisis por estatus de hiperglucemia, la asociación sólo se mantiene en los sujetos con diabetes mellitus tipo 2.
4ª Las medidas de adiposidad analizadas muestran una asociación negativa con las medidas de rigidez arterial. El porcentaje de variación del CAVI explicado por su relación con las diferentes medidas de adiposidad oscila entre el 5,8% con el CUN-BAE y el 3,7% con el BRI. En el caso de VOP brazo tobillo oscila entre 0,7% con el CUN-BAE e IMC y 0,1% con la CCA. Estos resultados sugieren que las medidas de adiposidad general y porcentaje de grasa corporal explican mejor la variabilidad del CAVI en comparación con las medidas de adiposidad abdominal y visceral
Detective fiction in Cuban society and culture.
PhDThe object of this thesis is to reach towards an understanding of Cuban society through a
study of its detective fiction and more particularly contemporary Cuban society through
the novels of the author and critic, Leonardo Padura Fuentes.
The method has been to trace the development of Cuban detective writing and to
read Padura Fuentes in the light of the work of twentieth century Western European
literary critics and philosophers including Raymond Williams, Antonio Gramsci, Terry
Eagleton, Roland Barthes, Jean Paul Sartre, Michel Foucault, Jean François Lyotard and
Jean Baudrillard in order to gain a better understanding of the social and historical
context from which this genre emerged.
By concentrating on the literary texts, I have explored readings which lead out into
an analysis of the broader philosophical, political and historical issues raised by the
Cuban revolution. Since it deals primarily with modes of deviance and notions of legality
and justice within the context of the modern state, detective fiction is particularly well
suited to this type of investigation. The intention is to show how this is as valid in the
Cuban context as it is in advanced capitalist societies where such research has already
been carried out with some success.
The thesis comprises an introduction, ten chapters and a conclusion. The chapters
are divided into three sections. Chapters 1 to 3 attempt a broad theoretical, historical and
socio-political analysis of the cultural reality within which the Cuban revolutionary
detective genre emerged. Chapters 4 to 6 analyse the Cuban detective narrative from its
inception in the early part of the twentieth century until the emergence of Leonardo
Padura Fuentes as the foremost exponent of the genre in Cuba after 1991. Chapters 7-
10 concentrate upon the work of Leonardo Padura Fuentes, offering a reading of his
detective tetralogy informed by the preceding discussion.
The contribution made by the thesis to knowledge of the subject is to build upon the
work of Seymour Menton and Amelia S. Simpson on the development of the Cuban
detective novel and to provide analyses of the pre-Revolutionary Cuban detective
narrative and the work of Leonardo Padura Fuentes for the first time in the English
language. The thesis concludes that the study of this popular genre in Cuba is of crucial
importance to the scholar who wishes to reach as full an understanding of the social
dynamics within that society as possible. In particular, it proves that Cuban detective
fiction provides a useful barometer of social change which records the shifts in the Cuban
Zeitgeist that have taken place over the past century
Efectos de un programa de entrenamiento de resistencia en core en oficinistas entre 25 y 30 años

Esta investigación demuestra el uso del entrenamiento funcional con el propósito de desarrollar la resistencia en la zona media muscular en adultos de 25 a 30 años de edad; para esto se tomó una muestra de catorce personas, tres hombres y once mujeres, todos usuarios del mismo centro de entrenamiento, y diferentes profesiones donde se demandan posiciones y posturas durante un tiempo prolongado. Se aplicó un plan de entrenamiento de cuatro semanas y se utilizaron ejercicios propioceptivos y unilaterales, ambos enfocados a la higiene postural mediante una estructura de entrenamiento gradual en su intensidad, volumen y descanso, buscando que haya un desarrollo de la resistencia muscular sobre la zona media, y un efecto inmediato sobre el balance muscular y la prevención de lesiones.New training trends are an effective way to develop the middle muscular area of the human body, currently known as Core. This research demonstrates the use of functional training with the purpose of developing resistance in the middle muscular area in adults aged 25 to 30 years; This requires a sample of people, three men and once women, all users of the same training center, and different professions where positions and positions are demanded for a long time. A four-week training plan was applied and proprioceptive and unilateral exercises were used, both focused on postural hygiene through a gradual training structure in its intensity, volume and rest, looking for a development of muscular endurance on the middle zone , and an immediate effect on muscle balance and injury prevention.

Elder abuse within the family environment in the Azores Islands
ABSTRACT Objectives: to dimension abuse against vulnerable adults within the family and community environment in the Azores Islands, identify risk factors for abuse and describe the profile of an abused elder. Method: descriptive cross-sectional study. Random sampling. The instruments used were: clinical histories of the users, Mini-Mental State Examination, Index of Independence in Basic Activities of Daily Living, Family APGAR Scale, Elder Abuse Suspicion Index and Social Work Assessment Form. Descriptive statistical analysis was used for qualitative and quantitative variables and multiple logistic regression was used to identify factors associated with elder mistreatment. Results: abuse suspicion was identified in 24.5% of elderly participants. Psychological abuse was the most common type of abuse and sons were the main abusers. Conclusion: being a woman and belonging to a dysfunctional family is associated with an increased risk of becoming a victim of abuse; the high level of domestic violence against the elderly in the Azores Islands is in line with the rest of Portugal
CLN5 in heterozygosis may protect against the development of tumors in a VHL patient
© The Author(s).Von Hippel-Lindau syndrome (VHL) is a rare disease of dominant inheritance that increases susceptibility to tumor development, with a complete penetrance at the age of 60. In this report, we present the unprecedented case of a VHL carrier who remains healthy at 72. Under the course of this study, it was discovered that this patient carries a mutation for a second rare disease, Neuronal Ceroid Lipofuscinosis (NCL or CNL). We hypothesize that the CLN mutation she carries offers a protective effect, preventing tumor development in the cells potentially suffering a VHL second hit mutation. To test this hypothesis, we ran a series of molecular experiments and confirmed that cell viability of primary endothelial cells decreases upon CLN5 silencing. Infections caused by KPC-producing Klebsiella pneumoniae (KPC-KP) represent a persistent public health challenge. This prospective study examines ten-year trends, clinical features, and genomic epidemiology of hospital-onset (HOI) and non-hospital-onset (non-HOI, including healthcare-associated [HcAI] and community-acquired [CA]) KPC-KP infections following a 2012 outbreak. We evaluated the impacts of a 2014 antimicrobial stewardship program (ASP) and COVID-19-related infection prevention and control (IPC) measures, with emphasis on hospital-to-community dissemination.[Methods]
We analysed a prospective, longitudinal cohort of patients (2012–2022) in a tertiary referral hospital. Interrupted time series and ARIMA models assessed ASP and IPC impacts on incidence density (ID). Cross-correlation analysis explored temporal associations between HOI and non-HOI trends. Whole-genome sequencing and PERMANOVA evaluated the genomic structure of ST512/KPC-3 isolates. Multivariable regression analysed the association between infection type and clinical outcomes.[Results]
Among 467 patients, 33.2% had non-HOI (ID 0.53/1,000 admissions/month) and 66.8% HOI (ID 0.30, p = 0.39). Urinary tract infections predominated in non-HOI (52.9%), while bloodstream and respiratory infections were more common in HOI. Incidence density of HOI and non-HOI infections declined significantly following ASP implementation, with a 4-month lag suggesting sequential transmission dynamics. These reductions were maintained during the pandemic. Genomic data confirmed ST512/KPC-3 dominance and hospital-to-community spread, with temporal factors—rather than acquisition type—explaining genetic variation. Adjusted analyses showed similar 30-day mortality and treatment responses across HOI and non-HOI.[Conclusions]
ASP and COVID-19 IPC measures contributed to maintaining low KPC-KP incidence. Genomic evidence underscores the role of temporal dynamics and clonal expansion in ST512/KPC-3 dissemination.
