Family-clinician communication in delirium care within acute care settings : a scoping review

Adaptation du cadre conceptuel de Seaman et al. (2017) appliquée aux soins aigus.La communication famille-clinicien est essentielle pour favoriser l’engagement des familles en contexte de délirium, soutenant une détection précoce et une gestion centrée sur la personne. Toutefois, cette communication demeure inconsistante et peu structurée, révélant une lacune importante dans la compréhension des besoins des familles en contexte de délirium en soins aigus. Cette revue de la portée visait à cartographier la littérature sur les besoins et objectifs de communication des familles, et à identifier les obstacles et facilitateurs perçus à leur atteinte. Basée sur la méthodologie de Joanna Briggs Institute (2020), une recherche documentaire a été menée dans quatre bases de données: MEDLINE, CINAHL, PsycINFO et ProQuest. Deux évaluateurs indépendants ont extrait les données, un troisième tranchant en cas de conflit. Intégrée à une revue plus large, un sous-ensemble de textes ciblant la communication a été réévalué. Sur 8,627 articles, 158 textes complets ont été évalués, 32 réévalués et 12 inclus. L’analyse descriptive et thématique a été guidée par le cadre de Seaman et al. (2017). Les besoins rapportés s’alignaient avec quatre des cinq objectifs de communication du cadre: soutien émotionnel, information claire et proactive, compréhension du patient comme une Personne, et implication dans les décisions. Les obstacles comprenaient la rotation du personnel, le manque d’information proactive et la détresse émotionnelle. Les facilitateurs incluaient une communication empathique et les plateformes. Les résultats appuient le développement de stratégies de communication adaptées, interdisciplinaires et partenariales, la formation et la mobilisation des connaissances pour mieux guider la communication en contexte de délirium.Family–clinician communication is crucial for fostering family engagement in delirium care, which supports timely detection and patient-centred management. However, communication practices remain inconsistent and poorly structured, highlighting a significant gap in understanding the needs of families. This scoping review aimed to map the literature on the communication needs and goals reported by families in the context of delirium care in acute care settings. A secondary objective was to identify family-reported barriers and facilitators to achieving these goals. Following Joanna Briggs Institute methodology (2020), four databases were searched: MEDLINE, CINAHL, PsycINFO, and ProQuest. Two reviewers independently screened and extracted data, with a third resolving any conflicts. This review re-screened a subset of full texts within a broader scoping review focused on family-clinician. Of 8,627 records, 158 full texts were assessed, 32 re-screened, and 12 included. Data were analyzed descriptively and thematically using the framework of Seaman et al. (2017). Families reported goals aligned with four of the five communication goals proposed in the framework: emotional support, transparent and timely information, understanding the patient as a person, and involvement in decision-making. Reported barriers included clinician turnover, lack of proactive information, and emotional distress; facilitators included empathic communication, structured communication tools, and platforms. Future research and clinical initiatives should prioritize the development of tailored, interdisciplinary strategies and the advancement of patient–partnered approaches, as findings reinforce the central role of family–clinician communication in improving outcomes for patients and their families in delirium care
Nurses’ perceptions of family involvement in delirium assessment, prevention and management in postoperative cardiac surgery patients
Introduction: Le délirium est fréquent chez les adultes qui récupèrent d’une chirurgie cardiaque à l'unité de soins intensifs (USI), et les familles sont de plus en plus sollicitées à participer aux soins entourant cette complication. Cette étude avait pour but de décrire la perception qu'ont les infirmières et les infirmiers de la participation des familles dans les soins entourant l’évaluation, la prévention et la gestion du délirium en période postopératoire de chirurgie cardiaque à l’USI.
Méthode: Un échantillon de convenance composé de 6 infirmières et infirmiers (âge moyen 38 ans) a été recruté dans deux USI de Montréal. Après avoir obtenu leur consentement éclairé par écrit, les participantes et participants ont pris part à une entrevue individuelle semi-structurée. Les analyses descriptives ont été réalisées selon la méthode de Braun et Clarke (2006).
Résultats: Cinq thèmes ont été identifiés : 1) les familles peuvent être impliquées dans les soins entourant le délirium quand les patients ont atteint une certaine stabilité, 2) la participation des familles peut être facilitée par l’entremise du transfert d'informations, 3) la contribution des familles est utile pour la détection du délirium, 4) les familles peuvent prendre des mesures concrètes lorsque le délirium survient, et 5) la participation des familles est influencée par certaines caractéristiques.
Conclusions: Cette étude suggère que les infirmières et les infirmiers perçoivent assez favorablement la participation de la famille dans les soins entourant le délirium en période postopératoire de chirurgie cardiaque à l’USI, selon le niveau de préparation et les caractéristiques de celle-ci.
Mots clés: Chirurgie cardiaque, délirium, perception des infirmières, participation de la famille, unité de soins intensifs.Introduction: Delirium is common in adults recovering from cardiac surgery in the intensive care unit (ICU), and families are increasingly solicited to participate in the care surrounding this complication. The aim of this study was to describe nurses' perceptions of family involvement in care surrounding the assessment, prevention and management of delirium in the postoperative period of cardiac surgery in the ICU.
Methods: A convenience sample of 6 nurses (mean age 38 years) was recruited from two Montreal ICUs. After obtaining written informed consent, participants took part in a semi-structured individual interview. Descriptive analyses were performed using the method of Braun and Clarke (2006).
Results: The analysis revealed five themes directly related to nurses' perceptions of family involvement in care of the patient in delirium, namely: 1) families can be involved in delirium care once patients have achieved hemodynamic stability and neurocognitive orientation, 2) family involvement can be facilitated through information transfer, 3) family input is helpful for delirium detection, 4) families can take concrete action when delirium occurs, and 5) family involvement is influenced by certain characteristics.
Conclusions: This study suggests that nurses perceive family involvement in delirium care quite favorably in the postoperative period of cardiac surgery in the ICU, depending on the level of preparation and characteristics of the family.

Examen du potentiel de l’oxymétrie cérébrale et de l’électroencéphalographie traitée : nouvelle mesure de détection du délirium aux soins intensifs chirurgicaux cardiaques.
Des indicateurs physiologiques de l’activité cérébrale pourraient améliorer la détection du délirium. Ce mémoire a pour but de décrire les valeurs d’oxymétrie cérébrale (rSO2) et d’électroencéphalographie traitée (pEEG) pendant les trois premiers jours d’hospitalisation aux SI à la suite d’une chirurgie cardiaque chez le groupe de patients qui présentent un délirium (D+) et chez ceux qui n’en présentent pas (D-), puis de comparer ces valeurs entre les deux groupes. Les mesures concomitantes du Confusion Assessment Method — Intensive Care unit (CAM-ICU), de la rSO2 et de la pEEG sur les ondes alpha, thêta et delta ont été recueillies au premier (J1), deuxième (J2) et troisième (J3) jour à la suite de la chirurgie. Parmi les 125 patients retenus, 10 (8 %) étaient en délirium (CAM-ICU + au moins une fois entre J1 et J3). De J1 à J3, les valeurs de rSO2 moyennes sont similaires pour les deux groupes (D+ : 62,9 à 61,4 %, D- : 62,1 à 58,7 % ; p > 0,05). En comparaison avec le groupe D- en J2, les patients D+ présentent un ratio plus bas d’ondes alpha (2,7 % versus 5,6 % ; p = 0,007) et un ratio d’ondes delta plus élevé (50,5 % versus 45,2 % ; p = 0,009).Physiological indicators of brain activity could improve the detection of delirium. The purpose of this Master Thesis is to describe the values of cerebral oximetry (rSO2) and processed electroencephalography (pEEG) during the first three days of hospitalization to ICU following cardiac surgery in the group of patients who have delirium (D+) and in those who do not (D-), and then compare these values between the two groups. Concomitant measures of Confusion assessment method — intensive care unit (CAM-ICU), rSO2 and pEEG on alpha, theta and delta waves were collected on the first (D1), second (D2) and third (D3) day following surgery. Of the 125 patients selected, 10 (8%) were in delirium (CAM-ICU + at least once between Day 1 and Day 3). From D1 to D3, the mean rSO2 values are similar for both groups (D+ : 62.9 to 61.4 %, D-: 62.1 to 58.7 %; p > 0.05). Compared with the D- group to D2, the group D+ patients have a lower ratio of alpha waves (2.7 % versus 5.6 % ; p = 0.007) and a higher delta wave ratio (50.5 % versus 45.2 % ; p = 0.009)
Facilitateurs et obstacles d’une formation pour les infirmières et infirmiers sur l’insertion de cathéters intraveineux périphériques par échographie : une étude de cas
L’insertion de cathéters intraveineux périphériques est une procédure courante en milieu hospitalier, mais elle peut s’avérer complexe chez des patients avec des veines dites « difficiles », entraînant retards dans les soins, inconfort et complications. L’échographie guidée représente une alternative pour améliorer le taux de succès des insertions. Cette étude visait à analyser l’implantation d’un programme de formation à l’insertion de cathéters intraveineux périphériques par échographie guidée, destiné aux infirmières et infirmiers d’un établissement hospitalier tertiaire, en identifiant les obstacles et les facilitateurs, ainsi qu’en comparant les perceptions des responsables et des personnes apprenantes. Une étude de cas qualitative a été menée auprès des personnes apprenantes et responsables du programme, avec des données recueillies entre novembre 2023 et novembre 2024 via des entretiens semi-structurés et une analyse documentaire. Le Theoretical Domains Frameweork (TDF) a été mobilisé pour structurer l’interprétation des résultats et identifier des leviers d’actions. L’analyse a relevé quatre obstacles principaux : manque d’occasions de pratiques supervisées, ressources matérielles insuffisantes, difficultés de coordination et de suivi, ainsi qu’une certaine réticence au changement. Deux principaux facilitateurs ont été relevés : la motivation des personnes apprenantes ainsi que la collaboration des patients au processus de formation. Les résultats montrent que l’implantation d’une formation dépend d’un équilibre entre ressources, encadrement, communication et culture organisationnelle favorable à l’innovation. Ces constats peuvent éclairer le développement de futures initiatives en formation clinique. Des études complémentaires, dans d’autres contextes et selon une approche longitudinale, sont recommandées pour approfondir la pérennisation de l’échographie guidée en pratique infirmière.Peripheral intravenous (IV) catheter insertion is a common procedure in hospital settings but can be challenging in patients with so-called “difficult” veins, leading to delays in care, discomfort, and complications. Ultrasound guidance represents an alternative to improve insertion success rates. This study aimed to analyze the implementation of an ultrasound-guided IV catheter training program for nurses in a tertiary hospital, identifying barriers and facilitators and comparing the perceptions of program coordinators and participants. A qualitative case study was conducted with participants and program coordinators. Data were collected between November 2023 and November 2024 through semi-structured interviews and document analysis. The Theoretical Domains Framework was used to structure the interpretation of results and identify means of action to support implementation. The analysis identified four main barriers: lack of supervised practice opportunities, insufficient material resources, coordination and follow-up difficulties, and some resistance to change. Two key facilitators were highlighted: participant motivation and patient collaboration during the training process. Results indicate that the implementation of a training program depends on a balance between available resources, accessible supervision, clear communication, and an organizational culture supportive of innovation. These findings can inform the development of future clinical training initiatives. Further studies in different contexts and using a longitudinal approach are recommended to deepen understanding of the conditions supporting sustainable integration of ultrasound-guided IV catheter insertion in nursing practice
Le délirium postopératoire en chirurgie cardiaque chez l’adulte en lien avec la pulsatilité des vélocités Doppler de la veine fémorale
Les troubles cognitifs postopératoires sont fréquents après une chirurgie cardiaque, variant entre 26 % à 70 %. Bien que diverses hypothèses expliquent leur pathogenèse, la congestion veineuse représente une hypothèse prometteuse. Des études ont associé le délirium à des signes échographiques de congestion veineuse tels que la pulsatilité du Doppler de la veine porte et rénale. Détecter la congestion veineuse pourrait potentiellement prédire le trouble cognitif, facilitant ainsi sa gestion. Une technique prometteuse, mais peu explorée, est l’évaluation de la congestion veineuse par le Doppler de la veine fémorale.
Les objectifs de ce mémoire sont : (1) synthétiser les évidences sur la relation entre le Doppler de la veine fémorale et la congestion veineuse chez les adultes ; et (2) caractériser l’existence d’une association entre les trouble cognitifs et le Doppler pulsatile de la veine fémorale après une chirurgie cardiaque. Afin d’atteindre ces objectifs, nous avons effectué une revue rapide de littérature portant sur la corrélation entre le Doppler de la veine fémorale et la congestion veineuse. De plus, nous avons complété une étude (NCT05038267) dont l’objectif secondaire était de déterminer la corrélation entre un Doppler anormal de la veine fémorale et le délirium.
Les résultats ont montré une association entre la congestion veineuse et Doppler pulsatile de la veine fémorale, ainsi qu’entre les troubles cognitifs et le Doppler anormal de la veine fémorale. Toutefois, des études approfondies sont nécessaires.
En conclusion, un lien entre le Doppler de la veine fémorale, la congestion veineuse, et les troubles cognitifs après une chirurgie cardiaque a été démontré. Ces observations ouvrent la voie à des recherches approfondies sur ce lien et à de potentielles applications cliniques et thérapeutiques.Postoperative cognitive disorders are common after cardiac surgery, ranging from 26% to 70%. While various hypotheses explain their pathogenesis, venous congestion emerges as a promising hypothesis. Studies have linked delirium to ultrasound signs of venous congestion, such as pulsatility in the Doppler of the portal and renal veins. Detecting venous congestion could potentially predict cognitive disorders, thus facilitating their management. A promising yet underexplored technique is the assessment of venous congestion using the Doppler of the femoral vein.
The objectives of this thesis are: (1) to synthesize evidence on the relationship between FV Doppler characteristics and venous congestion in adults; and (2) to characterize the existence of an association between cognitive disorders and pulsatility in femoral vein Doppler after cardiac surgery. To achieve these objectives, we conducted a rapid literature review focusing on the correlation between femoral vein Doppler and venous congestion. Additionally, we completed a study (NCT05038267) with the secondary objective of determining the correlation between abnormal femoral vein Doppler and delirium.
The results showed an association between venous congestion and pulsatility in the FV Doppler, as well as between cognitive disorders and abnormal FV Doppler. However, in-depth studies are needed.
In conclusion a connection has been demonstrated between femoral vein Doppler, venous congestion, and cognitive disorders after cardiac surgery. These findings open the way for in-depth research on this link and potential clinical and therapeutic applications
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases.
