    Contrast-enhanced ultrasound (CEUS) represents one of the most interesting and promising examples of imaging application in clinical practice. Ultrasound contrast media are defined as “blood pool” agents because they remain confined into blood vessels without reaching the excretory tract. They are classified into first and second generation depending on the gas in microbubbles. When the ultrasound beam hits microbubbles in the blood flow, their compressible gas cores oscillate in response to the high-frequency sonic energy field. The microbubbles reflect a unique echo that stands in stark contrast to the surrounding tissue due to the orders of magnitude mismatch between microbubble and tissue echogenicity. Both contrast generations are characterised by an excellent safety profile, with rare hepatic and central nervous system side effects and absent nephrotoxicity thanks to their predominant pulmonary clearance. The clinical utility of CEUS, which is widely recognised in the study of liver diseases, is also becoming relevant to the pathology of other organs, such as genitourinary system, gastrointestinal tract, peripheral vascular system, spleen and lymph nodes, and breast, lung, and endocrine and exocrine glands. Possible applications of CEUS in nephrology include renal ischemia, differential diagnosis of cystic and solid lesions, follow-up of ablative therapies, kidney trauma, kidney transplant, inflammatory diseases, renal artery stenosis, and vesicoureteral reflux

    RBP4: A Culprit for Insulin Resistance in End Stage Renal Disease That Can Be Cleared by Hemodiafiltration

    Introduction. Retinol Binding Protein 4 (RBP4) is mainly excreted by the kidney and plays a pivotal role in insulin resistance (IR). In our study, we evaluated the association between RBP4 and IR in hemodialysis subjects (HD). We also assessed how circulating RBP4 could be influenced by kidney transplant or different dialytic techniques.Methods. RBP4 serum levels were evaluated in HD (n=16) and matched healthy controls (C;n=16). RBP4 and glucose transporter type 4 (GLUT4) mRNA expressions were also determined in adipose tissue. Circulating RBP4 was evaluated after kidney transplant (n=7) and in hemodialysis patients (n=10) enrolled in a cross-over study treated with standard bicarbonate dialysis (BD) or hemodiafiltration (HDF).Results. HOMA index (P&lt;0.05) and serum RBP4 (P&lt;0.005) were higher in HD compared to C. RBP4 levels positively correlated with fasting serum glucose (P&lt;0.05). RBP4 mRNA was lower in HD compared to C (P&lt;0.05) and positively correlated with kidney function (P&lt;0.05) and GLUT4 mRNA (P&lt;0.001). Transplant or HDF reduced circulating RBP4 (P&lt;0.01andP&lt;0.05, resp.). Our results demonstrate that IR is associated with high circulating RBP4 and that suppressed RBP4 adipose tissue expression is accompanied by reduced GLUT4 expression in HD. Renal transplantation or HDF are effective in lowering serum RBP4 levels.</jats:p

    BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1RAs) are highly effective in improving glycaemic control either as monotherapy or in combination with other hypoglycaemic drugs, and have low incidence of side effects, such as hypoglycaemia, nausea and weight gain, thus increasing patients' adherence to therapy. METHODS: In this review we report the most recent studies demonstrating the beneficial effects of GLP-1RAs on renal outcomes, and also discuss the direct and indirect mechanisms through which they confer kidney protection. Finally, we discuss the metabolic and anti-inflammatory effects of GLP-1RAs in diabetic patients with COVID-19 disease. RESULTS: GLP-1RAs have a nephroprotective action, which is expressed through both indirect (improvement of blood pressure and glycaemic control, weight loss) and direct (restoration of normal intrarenal haemodynamics, prevention of ischaemic and oxidative damage) effects. They have shown also metabolic and anti-inflammation beneficial effects in patients with COVID-19 disease. CONCLUSIONS: GLP-1RAs prevent albuminuria and slow the decline of renal function towards end stage renal disease in patients with diabetic kidney disease. They might be an opportunity to break the typical inflammation processes of COVID-19 disease

    The intention of this work is to share with the academic community the advances and results of a work of investigation that is based on a theoretical frame proposed by the author, who allows him to consolidate a methodology to describe the Corporate Culture of an organization. For it, he, has fone up to the date application in approximately 28 companies in Colombia and 1 in Mexico with students of pregraduate and postgraduates students of the Faculty High Studies of Administration and Business (FAEN) of the University of the Rosario and in activities of Consultancy.To illustrate the methodology, object of this article, it he refers in his application in the company of the industrial sector, demonstrating the relevancy of the theoretical frame, the validity of his methodology, as well as reliability of the technologies of qualitative and quantitative charater used, joined the classification, organization, tabulation and analysis in the information that they present for the intention of the author. El propósito de este trabajo es compartir con la comunidad académica los avances y resultados de un trabajo de investigación, que se fundamenta en la descripción de la cultura, en el marco teórico propuesto por el autor, y la consolidación de una metodología para describir la cultura de una organización. Para ello, se han realizado aplicaciones en aproximadamente 28 compañías en Colombia y una en México, con estudiantes de pregrado y posgrado de la Facultad de Altos Estudios de Administración y Negocios (FAEN) de la Universidad de la Rosario, y en actividades de consultoría. Para ilustrar la metodología objeto de este artículo se presenta su aplicaci6n en una compañía del sector industrial, demostrando la importancia del marco teórico, la validez de su metodología, así como la confiabilidad de las técnicas de carácter cualitativo y cuantitativo, utilizadas mediante la clasificación, organización, tabulación y el análisis de la información que presentan para los propósitos del autor

    La patologia renale cronica, indipendentemente dall’eziologia, provoca un sovvertimento strutturale del parenchima con progressivo sviluppo di fibrosi. Il grado di fibrosi renale sembrerebbe correlare con un rischio aumentato di progressione verso l’uremia terminale; pertanto, il monitoraggio nel tempo della fibrosi renale può svolgere un ruolo importante nel follow-up delle patologie renali sia focali che diffuse, e nella valutazione della risposta ai trattamenti. La biopsia renale è l’unica metodica in grado di fornire informazioni oggettive e confrontabili sull’estensione della fibrosi rispetto al parenchima sano ma, data la sua invasività, non è indicata per il follow-up dei pazienti con malattia renale cronica. L’elastosonografia rappresenta una metodica ultrasonografica innovativa e non invasiva che consente la misura dell’elasticità tissutale attraverso la trasmissione di onde meccaniche e la misurazione della loro velocità di propagazione. Sebbene numerosi autori abbiano dimostrato l’utilità delle tecniche elastosonografiche per la quantificazione della fibrosi epatica, pochi sono gli studi sistematici sulle applicazioni dell’elastosonografia nelle patologie renali. Inoltre, fattori quali la profondità dei reni nativi rispetto al piano cutaneo, l’elevata anisotropia del tessuto renale e la possibilità di esaminare solamente una piccola regione di interesse limitano ad oggi la diffusione della metodica nella pratica clinica. Obiettivo di questa review è quello di riesaminare i principi fisici dell’elastosonografia e di passare in rassegna le ultime evidenze circa le possibili applicazioni del modulo ARFI (acoustic radiation force impulse) nello studio delle patologie renali.Chronic kidney disease is characterized by increasingly amplified fibrotic processes regardless of etiology. The severity of renal fibrosis seems to correlate with an increased risk of end-stage renal disease; therefore, monitoring of renal fibrosis over time may play an important role in the follow-up of both focal and diffuse renal diseases and in evaluating the response to treatments. Renal biopsy is the only method capable of providing objective and comparable information on the extent of fibrosis, but it is not suitable for outpatient monitoring of chronic kidney disease due to its invasiveness. Elastosonography is an innovative and non-invasive ultrasound method that allows the measurement of tissue elasticity through the transmission of mechanical waves and the measurement of their propagation speed. Although some authors have demonstrated the usefulness of elastosonographic techniques for the quantification of liver fibrosis, few studies have investigated the applications of elastosonography in renal pathology. Furthermore, the depth of native kidneys, the high anisotropy of the renal tissue, and the possibility of examining only a small region of interest currently limit its spread in clinical practice. The aim of this review is to examine the physical principles of elastosonography and to review the latest evidence about the possible applications of the ARFI (acoustic radiation force impulse) technique in the study of kidney diseases
