Aims and objectives: To identify the factors that influence decisions made by health professionals when assessing the pain of native English speaking and children whose English is an additional language. Background: Pain assessment in children is often poorly executed following acute injury. Whilst a range of pain assessment tools have been developed, little guidance is provided for assessing pain in children with English as an additional language. Design: Factorial survey design. Methods: Twenty minor injuries unit nurses and 20 children's nursing students participated in an electronic survey to make judgements on 12 scenarios describing a child attending a minor injuries unit following an incident, accompanied by a parent. Respondents had to decide the most important form of pain assessment, and whether they would ask a parent or an interpreter to assess the pain of the child. An open-ended question asked about the difficulties found in making a judgement. Results: Observation of the child's behaviour was the most common pain assessment reported. The visual analogue scale was significantly associated with children with proficient English. Respondents were significantly more likely to involve parents in the assessment if they could speak English well compared to parents with poor English skills. Moreover, nursing students were significantly more likely than registered nurses to call for support from an interpreter. Thematic analysis identified three themes related to difficulties with pain assessment: contrasting approaches, differing perceptions of pain and overcoming challenges. Conclusions: The reduced ability to communicate between child, parent and healthcare professional highlights the need to identify forms of assessment based on individual cases. Relevance to clinical practice: The number of children with English as an additional language has seen a marked rise over the last decade. In situations where communication ability is reduced, assessment of pain should be tailored to meet the needs of the child. This may require timely access to interpreter services

The impact of language on the expression and assessment of pain in children

    No full text
This paper focuses on the importance of language in the expression of pain. Variation in definitions of pain is presented, together with a review of the evidence examining the impact language may have on the way pain is expressed linguistically. The implications for conducting research with children who speak different languages are explored. Strategies such as using non-linguistic methods of communication, additional time required for conducting interviews and the inclusion of research team members from the same ethnic or linguistic background are presented

The effects of acute hyperglycemia on oxidative stress and nitric oxide bioavailability in platelets

    No full text
Objective: Platelet activation and thrombus formation tendency increase in diabetes mellitus. Nitric oxide (NO) synthesized by platelets plays important role in platelet functions and limits the recruitment of new platelets to the platelet-rich thrombus. However, NO may also be deleterious by elevating oxidative/nitrosative stress in cells. The aim of the present study was to examine effects of acute high glucose in both resting and collagen-stimulated platelets on platelet aggregation, oxidative stress and NO bioavailability. In addition, we investigated the role of superoxide production on NO bioavailability using quercetin as scavengers of superoxide anion. Methods: Washed platelets were incubated with 5mM D-glucose (physiological concentration, n:7) or 25mM D-glucose (pathophysiological concentration, n:7) or quercetin (10 mu M) plus 25 mM D-glucose (n:7) for 1 h. Superoxide production, lipid peroxidation (LPO), NO, nitrotyrosine (NT) levels and advanced glycation end products (AGE) in platelets were measured after incubation. Platelet aggregation was also investigated. Results: Incubation of resting and collagen-activated platelets with high glucose resulted in significant elevations in LPO, NT and AGE levels when compared to 5mM glucose concentration (p<0.05). High glucose significantly increased platelet aggregation, superoxide formation and NO levels in collagen-activated platelets (p<0.05). Quercetin markedly increased production and bioavailability of NO and suppressed oxidative stress in high glucose incubated platelets (p<0.05). Conclusion: High glucose increases oxidative stress and reduces bioavailability of NO in resting and especially collagen activated platelets. Hyperglycemia mediated superoxide production could be taken part on these effects. These findings would be suggested that there might be beneficial effects of quercetin in the prevention of platelet-related complications in diabetes mellitus

Cultural responses to pain in primary school age children: a mixed methods study

    No full text
Pain-measurement tools are often criticized for not addressing the influence of culture and ethnicity on pain. This study examined how children who speak English as a primary or additional language discuss pain. Two methods were used in six focus group interviews with 34 children aged 4–7 years: (i) use of drawings from the Pediatric Pain Inventory to capture the language used by children to describe pain; and (ii) observation of the children’s placing of pain drawings on red/amber/green paper to denote perceived severity of pain. The findings demonstrated that children with English as an additional language used less elaborate language when talking about pain, but tended to talk about the pictures prior to deciding where they should be placed. For these children, there was a positive significant relationship between language, age, and length of stay in the UK. The children’s placement of pain drawings varied according to language background, sex, and age. The findings emphasize the need for sufficient time to assess pain adequately in children who do not speak English as a first language

Örneğin Türkiye'de on beşten fazla evcil koyun ırkı bulunmaktadır. Azize Kırıkkale University Graduate School of Natural and Applied Sciences Department of Biology, M. Sc. Thesis Supervisor : Assoc. Prof. Dr. Şükran ÇAKİR September 2003, 84 pages The Anatolia has a rich animal gene resources. For example, more than 15 domestic sheep strains have been found in Turkey. This potantial needs national attention. Because the strains with high economical value can be improvement from these strains in future. Iri order to be successful in this tissue there is need enough knowledge of genetic construction of these strains and this is only possible with genetic resources based on DNA analysis in farm animals. In this study Randomly Amplified Polymorphic DNA (RAPD) which of the rnethod based on PCR markers Were used to determine the genetic diversity in three sheep that Sakiz, Karayaka and Bafra from Turkish sheep strains. This technique gives an opportunity to amplify several discrete loci in the genome by producing species or strain specific DNA profile. Several 111researchers have used RAPD assay for identification of markers linked to economically important traits, some of them were related to sheep traits. In this study 18 RAPD primers chosen in a previous study and 2 primer chosen arbitrary were screened for overall 160 individuals. 6 of these primers yielded 67 polymorphic markers. Polymorphism (P) revealed varried between 22.39 % in Karayaka and 53.73 % in Bafra. Nei's genetic distance was found between 0.0432 and 0.4709. The diversity within the populations (Hs) was 0.13. The magnitude of differentiation among the populations (Gst) was 0.58 suggesting that 58 % of the genetic diversity.
