    Peran Modified Hematological Scoring System dan Indeks Platelet pada Sepsis Neonatal

    Latar belakang Diagnosis dini sepsis neonatal masih menjadi suatu tantangan berat. Kultur darah yang merupakan uji baku emas memerlukan waktu yang cukup lama serta sering dijumpai positif dan negatif semu. Penggunaan modified hematological scoring system and indeks platelet (trombositopenia, peningkatan mean platelet volume dan platelet distribution width), diharapkan dapat membantu dalam diagnosis sepsis neonatal. Metode Penelitian ini merupakan studi analitik observasional dengan desain potong lintang yang dilaksanakan selama 9 bulan di Unit Perinatologi RSUP H. Adam Malik Medan. Sebanyak 95 neonatus diambil secara consecutive sampling dengan sangkaan sepsis yang termasuk dalam kriteria inklusi dan eksklusi dilakukan penilaian nilai diagnostik skor mHSS, indeks platelet, dan kultur darah. Hasil Proporsi kultur positif pada penelitian ini sebesar 29.4%. Skor mHSS≥3 memiliki sensitivitas 82.1%, spesifisitas 83.6%, PPV (positive predictive value)= 67.6%, NPV (negative predictive value) = 83.6%, PLR (positive likelihood ratio) = 5.01, NLR (negative likelihood ratio) = 0.21, AUC 0.834. Kombinasi indeks platelet memiliki sensitivitas 82.1%, spesifisitas 86.6%, PLR = 6.21, NLR = 0.20, AUC 0.816 dan kombinasi skor mHSS≥3 + indeks platelet meningkatkan nilai diagnostik dengan sensitivitas 92.9%, spesifisitas 83.6%, PLR=5.66, NLR=0.08, AUC 0.917. Kesimpulan Pada penelitian ini, nilai diagnostik skor mHSS ≥ 3 dan indeks platelet memiliki nilai sensitivitas, spesifitas, PPV, NPV, PLR, NLR dan AUC yang sangat baik.     Critical illness is collection of conditions and disorders that have impact on the occurrence of organ dysfunction, prolonged morbidity and mortality. Score of mortality objectively calculates the severity of disease and predicts risk of mortality according to the clinical condition of patient. Today, no standardized scoring system that used in PICU for initial assessment of patients in Indonesia yet. Objective: Comparing mortality’s predictor ability between PRISM IV and PELOD-2 score as predictors of mortality in critically ill children at Haji Adam Malik General Hospital Medan. Subject and Methods: Prospective cohort study was conducted in PICU of Haji Adam Malik General Hospital from September 2020 to March 2021. Characteristics of the sample were obtained through the medical record of patients treated in PICU. Assessment of PRISM IV and PELOD-2 score was carried out on the first and third days of patient being admitted in PICU. Results: Of the 35 children who were treated, 15 (42.9%) children were treated with respiratory problems. There were 18 (51.4%) children died. It was found that there was no significant difference mean score of PRISM IV and Pelod-2 score on first and third days of examination (p>0.05). It was also found that PRISM IV and Pelod-2 score could not predict mortality of children with critical illness on the first (AUC 49.7%; 57.2%) and third days (AUC 65 .4%;69.3%,). Conclusion: PRISM IV on the first and third day has lower accuracy than PELOD 2 in predicting mortality in critically ill children at Haji Adam Malik General Hospital Medan.Penyakit kritis merupakan sekumpulan kondisi dan kelainan yang memberikan dampak pada terjadinya disfungsi organ, morbiditas yang lama dan kematian. Skoring mortalitas secara objektif menghitung keparahan penyakit dan memprediksi resiko mortalitas sesuai kondisi klinis pasien. Sampai saat ini belum ada sistem skoring yang digunakan di PICU secara baku untuk penilaian awal pasien di Indonesia Tujuan : Mengetahui perbandingan skor PRISM IV dan PELOD-2 sebagai prediktor mortalitas pada anak sakit kritis di Rumah Sakit Haji Adam Malik Medan. Metode: Penelitian ini merupakan penelitian kohort prospektif yang dilakukan di ruang rawat PICU RSUP H. Adam Malik pada September 2020 sampai Maret 2021. Karakteristik sampel didapatkan melalui data rekam medis pasien yang dirawat di ruang PICU. Penilaian skor PRISM IV dan PELOD-2 dilakukan pada hari pertama dan ketiga pasien dirawat di ruang PICU. Hasil Penelitian: Dari 35 anak yang dirawat dengan penyakit kritis di PICU RSUP H.Adam Malik, sebanyak 15 (42,9%) anak dirawat dengan indikasi masalah respirasi. Rerata lama rawat anak adalah 10 hari dan sebanyak 18 (51,4%) anak dengan luaran meninggal. Dari penelitian didapat tidak ada perbedaan rerata yang signifikan skor PRISM IV dan skor Pelod-2 pada pemeriksaan hari pertama dan ketiga (p>0,05). Dari penelitian ini juga didapatkan bahwa skor PRISM IV dan Pelod-2 tidak dapat memprediksi mortalitas anak dengan penyakit kritis pada hari pertama (AUC 49,7%, p = 0,974; AUC 57,2%, p = 0,468) dan ketiga (AUC 65,4%, p = 0,121; AUC 69,3%, p = 0,051). Kesimpulan: Skor PRISM IV pada hari pertama dan ketiga memiliki akurasi yang lebih rendah dibandingkan PELOD-2 dalam memprediksi mortalitas pada anak sakit kritis di Rumah Sakit Haji Adam Malik Medan.81 halamanTesis Magiste

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
