    Dental implants have been used clinically in a routine manner to restore completely edentulous mandibles. A recent systematic review of the literature conducted by Bryant1 describes the 5-year cumulative survival rates of mandibular fixed and removable prostheses between 83% and 100%, with corresponding levels of crestal bone loss up to 1.1 mm the first year and 0.4 mm per year thereafter. The author included in his review studies using the classical two-stage surgical approach, whereby the implant is initially covered underneath the mucosa and kept unloaded for 4–6 months.2 However, over the past decade changes in dental implant design and surface configuration combined with an improved understanding of the biological and biomechanical aspects have improved the clinical outcome of implant treatments.3 These advancements have led to the one-stage surgical procedures in conjunction with earlier loading, especially in the completely edentulous mandible

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    Objectives: The purpose of this case report is to illustrate the consequences of dental surgi- cal extractions in patients who are receiving or have received IV bisphosphonate. Materials and methods: A 52-year old man suf- fering from multiple myeloma was treated with Zometa for one year. At the end of the therapy he underwent some surgical extractions that caused two areas of mandibular osteonecrosis after a few months. The authors obtained the patient’s informed consent to report his medical data and pictures. Results: The two necrotic areas were treated in different ways and the treatments are still in progress. After 5 months, a slight improvement has been observed in the exposed bone areas. Conclusion: In the management of the osteo- necrosis of the jaw it is important to assess the cost-benefit ratio, trying to improve health expectations and perspectives in a patient who is already affected by a systemic disease

    Abstract Aim: The aim of this paper was to show how it was possible to achieve full-arch dental implant immediate loading rehabilitation in the mandible for patients being treated with biphosphonates. Materials and methods: Nine adult patients (8 women and 1 man) aged 45 to 68 years presented for rehabilitation of the edentulous mandible. All patients were non-smokers with non-contributery medical histories. None of the patients had a history of chemotherapy or radiation therapy. All patients had been demonstrated osteoporotic disease (t-scan < 2,5) and were being treated with bisphosphonates per os (Residronate and Alendronate) for less than 3 years. All patients had residual teeth that were periodontally compromised. All patients were rehabilitated in mandible with a fixed full-arch prosthesis supported by six Way Syntegra ® implants. Results: No subjective complaints were reported throughout the follow-up period. After 2 years of loading, all implants were examined and found to be clinically stable without signs of infection. During the same period, no soft tissue swellings or osteonecrosis lesions were noted. The clinical survival rate is 100% in this interval. Conclusion: Rehabilitation of the edentulous mandible in these nine patients being treated with oral bisphosphonates and restored with an immediate occlusally loaded full-arch prosthesis supported by six Way Syntegra ® implants proofed to be a viable alternative treatment to the classical delayed placement and load protocols

    Relationship between volatie sulphur compounds in vivo and periodontal disease Aim of the work. The purpose of this study was to evaluate the relationship between volatile sulphur compounds (VSC) and gingival health status, and to monitor the changes in VSC in early dental plaqueinduced gingivitis. Materials and methods. Using an experimental gingivitis model, ten subjects between 21 and 35 years old, with a healthy gingival status, refrained from brushing and flossing one randomly selected half of the mandibular arch for two weeks. At baseline and during the six subsequent appointments, gingival inflammation (GI), bleeding on probing (BOP), and sulfide levels (SUL) were measured using the Gingival Index and the Diamond Probe/Perio 2000 System. The Spearman correlation was used to compare the relationships between SUL, GI, and BOP on the brushing (B) and non-brushing (NB) sides. Data on the NB side revealed a stronger correlation than on the B side. Wilcoxon rank sum was used to evaluate the differences between mean SUL, GI, and BOP scores on the B and NB sides over time. Results and conclusions. Results indicate that SUL were the first periodontal parameter to show a significant difference between sides. SUL were significantly higher on the NB side at 4 of the 6 data collection intervals; therefore, SUL may be associated with the initiation and progression of early plaque-induced gingivitis

    Scleroderma and CREST syndrome: a case report in dentistry

    CREST syndrome is part of the heterogeneous scleroderma group of autoimmune diseases that cause thickening, hardening and tightening of the connective tissue in different parts of the body, and it may lead to complex disorders. CREST syndrome is characterized by the coexistence of calcinosis, Raynaud's phenomenon, esophageal hypomotility, sclerodactily and telangectasia. A 72-year-old caucasian woman is referred to the S. Gerardo Hospital of Monza, with a chief complaint of oral pain and difficulties in deglutition and eating, associated with denture instability and difficulties to fit it. She had been previously diagnosed with Raynaud's phenomenon, and afterwards with CREST syndrome. Extra-oral examination underlined taut, thickened and rigid skin, pallid-red irregular maculae all over the face, telangiectasias and acrocyanosis. Intra-oral examination showed no alteration of the mucosa, but we can observe tongue rigidity and some speckled red alternating with white spots on the hard palate and in the vestibule. We undermitted the patient the dental treatment of Sjogren's syndrome. The management of the Sjogren's syndrome is symptomatic and empirical, and involves the use of saliva secretion stimulators, salivary substitutes and coadjuvants. Dental treatment and prophylaxis are important to prevent the consequences of xerostomia, such as rampant caries, based on the administration of topical fluoride in toothpastes and rinses, and supplemented by fluoride gels and varnishes. Instruction and reinforcement of oral hygiene, along with frequent dental assessment and management by the dentist are essential measures to preserve the oral health of those affected with CREST syndrome in progression to SS, complicated with Sjogren's syndrome

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
