Ein digitaler Klassifikator zur Unterscheidung des desmoplastischen vom spindelzelligen Melanom anhand der Zellkernrundheit im histologischen Bild
Die Unterscheidung von spindelzelligem Melanom (SM) und desmoplastischem Melanom (DM) ist klinisch aufgrund der Unterschiede in der Metastasierungstendenz und Prognose wichtig. Diese Unterscheidung ist allerdings nicht immer problemlos. Die Routinedurchsicht einiger Fälle zeigte interessante unterschiede in der Zellkernform zwischen SM und DM.
Primäres Ziel dieser Studie war es herauszufinden, ob die Unterschiede in der Form der Zellkerne als diagnostischer Klassifikator zur Unterscheidung von SM und DM dienen können. Die Form der Zellkerne wurde dazu mittels einer ImageJ-basierten Schwellenwertextraktion gewonnen und analysiert. Die Be-stimmung der Rundheit zeigte sich hierbei den anderen Messwerten als Überle-gen.
Die retrospektive Analyse einer bestehenden Patientenkohorte (SM n=9, DM n=9) wurde benutzt, um die Trennschärfe des Algorithmus zu bestimmen.
Repräsentative Bildausschnitte (= ROIs; insgesamt n=108; 6 Bilder pro Fall) wur-den aus gescannten, H&E-gefärbten, histologischen Schnitten ausgewählt und die Zellkern-Rundheit unter Einsatz von Open-Source-Programmen (Plugins für ImageJ) computergestützt extrahiert und quantifiziert.
Die extrahierten, quantitativen Form-Messwerte wurden durch Mann-Whitney-Test, Kruskal-Wallis-Test und einfache Varianzanalysen verglichen; p-Werte <0,05 wurden als signifikant erachtet.
Die Einteilung der Rundheitswerte in vier Formkategorien (spindelig, elongiert, oval und rund) offenbarte signifikante Unterschiede in der spindelförmigen und der runden Kategorie. Erstaunlicherweise enthält DM mehr spindelförmige Zell-kerne als SM (p=0,011) und SM enthält mehr runde Zellkerne als DM (p=0,026). Die Beurteilung der diagnostischen Trennschärfe erfolgte an einer kombinierten Form-Klassifikation der runden und spindelförmigen Zellkernanteile und zeigte eine Genauigkeit von 88,9% und einen Youden-Index von 0,77.
SM und DM unterscheiden sich signifikant in ihrer Zellkernmorphologie. Diese Studie zeigt, dass die Quantifizierung der Zellkernrundheit als zusätzliches diag-nostisches Werkzeug zur Unterscheidung von SM und DM genutzt werden kann. Die Unterscheidung von SM und DM ist aufgrund erheblicher Unterschiede in Metastasierungstendenz und Prognose klinisch wichtig. Basierend auf einer his-tologischen Beobachtung konnte ich hier den quantifizierbaren Nachweis er-bringen, dass SM mehr runde und DM mehr spindelförmige Zellkerne enthält. Die angewandten Quantifizierungswerkzeuge sind überwiegend frei verfügbar und erlauben eine diagnostische Unterscheidung der beiden Melanom-Subtypen mit einer belastbaren Trennschärfe

Deep Next-Generation Sequencing Enables Mutational Profiling of Classical Hodgkin Lymphoma
Effective treatments are available for classical Hodgkin Lymphoma (cHL); however, for many patients the success of initial treatment is often followed by a high rate of treatment-related conditions including infertility and secondary malignancies. Due to the rarity of the malignant Hodgkin and Reed/Sternberg (HRS) cells in cHL, molecular genotyping has not been successfully implemented. The inability to obtain the mutational profile currently precludes genotyping of cHL which is informative for clinical management in numerous other cancers. The genetic pathways implicated in the pathogenesis and oncogenic activity of cHL have been described and leveraged for the selection of first-line therapeutics. However, a comprehensive evaluation of relevant mutations in the SOCS1 gene remain elusive despite its role in cHL being verified in several studies. To assess the feasibility of next-generation sequencing (NGS) for genotyping in cHL, an NGS panel employing Anchored Multiplex PCR (AMP) was designed to target and sequence three relevant genes: B2M, JAK2, and SOCS1. The primary endpoint was suitable detection of low-lying mutations in the rare HRS cells which could be adequately detected by a clinical-grade laboratory assay. Secondary endpoints were the prevalence of mutations and establishing the limit of detection (LOD). Sufficient average sequencing depth for SOCS1 was achieved in normal feasibility samples at 236 total reads. In cHL cases sequencing coverage over genes B2M, JAK2 and SOCS1 was achieved at an average of 503 reads, 462 reads, and 191 reads respectively, which demonstrated the technical performance of the method. Diagnostic performance was satisfactory with mutations successfully detected in each of the three targeted genes for all sequenced cHL cases, with an established LOD of 5% allelic fraction establishing the analytical performance. Despite the technical barriers imposed by the rarity of tumor cells and the necessity to test DNA extracted from tissue biopsies with variable quality, the application of this method can reliably detect somatic mutations with a sufficient level of high quality sequencing coverage across the coding regions of B2M, JAK2, and SOCS1. The demonstrated levels of technical, diagnostic, and analytical performance are adequate to move this approach forward towards implementation of a clinical-grade laboratory assay.Biotechnolog
Potentielle klinische Implikationen von BRAF Mutationen in histiozytären Proliferationen
BRAF-V600E-Mutationen sind für eine wachsende Zahl von Tumoren und Leukämien von therapeutischer Relevanz, welche bei histiozytären Proliferationen, ebenso wie die Mutationsfrequenz, nicht abschließend geklärt ist. Zusammenfassend werden hier sowohl eine retrospektive Studie mit Genotypisierung von histiozytären Erkrankungen und entsprechender statistischer Auswertung sowie eine Metaanlyse der Literatur präsentiert. Die Genotypisierung der 69 histiozytären Läsionen zeigte, dass 23 der 48 Langerhans-Zell-Histiozytosen BRAF V600E mutiert waren (48%), während alle Non- Langerhans-Zell-Läsionen (einschließlich des Dendriten-Zell-Sarkoms, des juvenilen Xanthogranuloms, der Rosai-Dorfman-Erkrankung und des Granularzelltumors) einen BRAF Wildtyp zeigten. Die Metaanalyse von 29 Publikationen zeigte bei einer Anzahl von 653 Fällen eine Gesamtmutationsfrequenz von 48,5% und ist auf dieser Grundlage als valide definiert einzuordnen. Der BRAF-Mutationsstatus kann auf dem Boden klinischer Parameter nicht vorhergesagt werden, ebenso wenig ist das Outcome der Patienten vorhersagbar. Trotzdem hat das Vorkommen einer BRAF Mutation einen therapeutischen Wert, da im Falle eines Versagens der etablierten Therapieprotokolle bei aggressiven und therapierefraktären, systemischen, BRAF-mutierten Erkrankungen eine Behandlung mit einem BRAF Inhibitor eine Therapieoption darstellen kann. Abschließend beschränken sich BRAF-Mutationen in histiozytären Tumoren auf Läsionen des Langerhans-Zell-Typs und können weder über Lokalisation noch Therapieansprechen oder Outcome Informationen bereitstellen. Obwohl für die meisten LCH-Patienten effiziente Therapien zur Verfügung stehen, können Patienten mit BRAF-Mutationen von der Therapie mit dem BRAF-Inhibitor Vemurafenib profitieren (gesamte Zusammenfassung nach Bubolz et al., 2014
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. SOCS1 mutation subtypes predict divergent outcomes in diffuse large B-Cell lymphoma (DLBCL) patients.
Suppressor of cytokine signaling 1 (SOCS1) is frequently mutated in primary mediastinal and diffuse large B-cell lymphomas (DLBCL). Currently, the prognostic relevance of these mutations in DLBCL is unknown. To evaluate the value of the SOCS1 mutation status as a prognostic biomarker in DLBCL patients, we performed full-length SOCS1 sequencing in tumors of 154 comprehensively characterized DLBCL patients. We identified 90 SOCS1 mutations in 16% of lymphomas. With respect to molecular consequences of mutations, we defined two distinct subtypes: those with truncating (major) and those with non-truncating mutations (minor), respectively. The SOCS1 mutated subgroup or the minor/major subtypes cannot be predicted on clinical grounds; however, assignment of four established gene-expression profile-based classifiers revealed significant associations of SOCS1 major cases with germinal center and specific pathway activation pattern signatures. Above all, SOCS1 major cases have an excellent overall survival, even better than the GCB-like subgroup. SOCS1 minor cases had a dismal survival, even worse than the ABC gene signature group. The SOCS1 mutation subsets retained prognostic significance in uni- and multivariate analyses. Together our data indicate that assessment of the SOCS1 mutation status is a single gene prognostic biomarker in DLBCL
