Going Beyond Counting First Authors in Author Co-citation Analysis
Full text linkThe present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
RUOLO DELL’EMBOLIZZAZIONE ARTERIOSA NELLA GESTIONE DEI TRAUMI EPATICI
No full textTipo di studio: pazienti traumatizzati con lesioni epatiche trattati mediante embolizzazione, senza resezione chirurgica.
Scopo: valutare il ruolo dell’embolizzazione arteriosa nella gestione dei pazienti con trauma epatico sanguinante, come alternativa all’intervento chirurgico open di resezione epatica.
Materiali e metodi: la nostra casistica si compone di 18 pazienti con trauma epatico sanguinante giunti al Pronto Soccorso in un periodo di 4 anni, dal 2007 al 2011, e trattati con embolizzazione arteriosa. Tutti i pazienti inseriti nello studio presentavano alla TC total body eseguita secondo il protocollo di screening per politrauma una lesione epatica di II, III, IV o V grado in base alla classificazione della Società Americana per la Chirurgia dei Traumi e sono stati trattati o con la sola embolizzazione arteriosa tramite Spongel e/o spirali RM compatibili e/o particelle, o con packing chirurgico compressivo e successiva embolizzazione arteriosa. Solo in un caso l’embolizzazione non è stata sufficiente a risolvere il quadro e, a distanza di alcuni giorni, persistendo l’anemizzazione da sanguinamento epatico, si è resa necessaria l’esecuzione di un’epatectomia destra.
Risultati: in tutti i pazienti trattati, escluso uno, abbiamo ottenuto un ottimo controllo finale con arresto di tutti i foci di sanguinamento attivo e risalita dei valori pressori.
Conclusioni: l’embolizzazione arteriosa ha permesso in 17 casi su 18 l’arresto del sanguinamento attivo a carico del parenchima epatico, con conseguente buona stabilizzazione emodinamica, diminuzione della necessità di trasfusioni di sangue e infine mancato ricorso ad interventi chirurgici resettivi e demolitivi sul parenchima epatico
The role of arterial embolization in blunt splenic injury.
No full textPURPOSE: The aim of study was to evaluate the results of our experience with transarterial embolization based on a modified algorithm in patients with splenic injury.
MATERIALS AND METHODS: We collected data of patients admitted to our hospital from January 2006 to August 2008 for blunt splenic injury. During this period, 46 patients were admitted for splenic trauma, of whom 17 were treated surgically, 15 conservatively and 14 with percutaneous embolisation (13 men, mean age 44.8, mean injury severity score 18.5, six with grade IV and eight with contrast blush). Patients in shock were referred for laparotomy and splenectomy, whereas those who were haemodynamically stable or responsive to fluid resuscitation were further evaluated with computed tomography (CT). In the presence of imaging evidence of splenic injury ranging from grade I to grade III (n=15) a conservative approach was adopted, whereas haemodynamically unstable patients with grade V injury (n=17) were treated with splenectomy. Embolisation was performed in 14 patients with grade IV injury or in the event of contrast extravasation, regardless of injury grade. In patients with diffuse organ damage, we embolised the main splenic artery, whereas in the case of localised injury, embolisation was selective.
RESULTS: Proximal embolization was required in eight cases and distal coil embolization in six. In 13 cases, we placed magnetic-resonance-compatible coils 4-6 mm in diameter; only one patient was treated with gel-foam injection. Immediate technical success was achieved in all cases. In 13/14 patients (92.9%), no periprocedural complications were observed, whereas the remaining patient underwent splenectomy within 24 h due to recurrent bleeding.
CONCLUSIONS: On the basis of our algorithm, it is possible to reach a quick decision on the most appropriate treatment for patients presenting with blunt abdominal trauma, and splenic artery embolization seems to offer a reliable option in those with high-grade splenic injury or active bleeding
Variations on the Author
Full text link“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
