Current status and trends in the diagnostics of AML and MDS.
No full textDiagnostics of acute myeloid leukemia (AML) and myelodysplastic syndromes (MDS) have recently been experiencing extensive modifications regarding the incorporation of next-generation sequencing (NGS) strategies into established diagnostic algorithms, classification and risk stratification systems, and minimal residual disease (MRD) detection. Considering the increasing arsenal of targeted therapies (e.g. FLT3 or IDH1/IDH2 inhibitors) for AML, timely and comprehensive molecular mutation screening has arrived in daily practice. Next-generation flow strategies allow for immunophenotypic minimal residual disease (MRD) monitoring with very high sensitivity. At the same time, standard diagnostic tools such as cytomorphology or conventional cytogenetics remain cornerstones for the diagnostic workup of myeloid malignancies. Herein, we summarize the most recent advances and new trends for the diagnostics of AML and MDS, discuss the difficulties, which accompany the integration of these new methods and their results into daily routine, and aim to define the role hemato-oncologists may play in this new diagnostic era

The Author of the Alexander Romance
No full textThis paper, which is based on a portion of the introduction of the author’s edition of Il Romanzo di Alessandro (Mondadori: Fondazione Valla 2007), surveys the generic components of the Alexander Romance in an attempt to arrive at a definition of the work. The argument builds on Merkelbach’s categorisation of elements and uses Fusillo’s insight into the novel as an ‘encyclopaedic genre’ to propose that ‘historical novel’ is not, as Hägg contended, a misnomer for the work. The main components I discuss are: ‘life’; praxeis; chreiai; Cynic elements, including choliambic poetry and utopian perspectives; and the Egyptian aspects of the narrative. A concluding jeu d’esprit offers a characterisation of the putative author, his antecedents and his process of composition.Richard Stoneman was for 25 years editor for classics at Croom Helm and then Routledge. In 1997 he was appointed an Honorary Fellow in the department of classics, University of Exeter. After retiring from publishing in 2006 he has been pursuing his researches on the Alexander legends and teaching a course on the subject at Exeter. His Penguin translation of the Alexander Romance was published in 1991, and a volume of translated Legends of Alexander the Great appeared from Everyman in 1994. Also in 1994 he co-edited Greek Fiction with John Morgan. His edition of the Greek recensions of the Alexander Romance was published (volume I) by the Fondazione Valla in 2007 – volumes II and III will follow over the next few years – and his Alexander the Great: A Life in Legend appeared from Yale University Press in spring 2008. He is the author of a number of other books on Greek history and travel, and is writing a book on oracles

Genetic alterations crossing the borders of distinct hematopoetic lineages and solid tumors: Diagnostic challenges in the era of high-throughput sequencing in hemato-oncology.
No full textOwing to the introduction of next-generation sequencing (NGS) new challenges for diagnostic algorithms and the interpretation of the results for therapeutic decision making in hemato-oncology have arisen. Recurrent somatic mutations crossing the borders between different hematological entities and solid neoplasms have been detected. In analogy to mutant TP53, the same mutation type may occur in myeloid, B- or T-lymphatic malignancies or solid neoplasms. At the same time, a certain mutation can show different prognostic outcomes in different entities and co-existence of certain mutations may change the prognostic relevance. These insights may spark the investigation of targeted therapies with the same substances across different disease entities. This review article summarizes mutations that can emerge in different hematologic and solid malignancies and summarizes other obstacles in the era of modern molecular diagnostics, such as the phenomenon of "clonal hematopoiesis of indeterminate potential" being difficult to interpret in the individual patient
