No full textAbnormalities of enteric nerves, interstitial cells of Cajal (ICC) and smooth muscle are often associated with severe gastrointestinal motility disorders. In this context, full-thickness biopsy of the gut may provide important diagnostic and prognostic clues as well as some possible therapeutic implications. Nonetheless, the unavoidable risk to further worsen prognosis evoked by laparotomy, and the unclear yield of histopathological analysis has hampered full-thickness gut sampling in patients with severe dysmotility. However, recent advances in minimally invasive surgery have refuelled enthusiasm in gastrointestinal neuromuscular pathology. In this issue of Neurogastroenterology and Motility, Miller et al. provide novel and exciting evidence that the appendix might be used as a surrogate tissue to analyse changes to enteric nerves, ICC and smooth muscle cells in patients with diabetic gastroenteropathy. The objective of this short review was to place this very important work in the context of current understanding of enteric neuromuscular dysfunction
Acute colonic pseudo-obstruction
No full textBackground: Acute colonic pseudo-obstruction is characterized by clinical and radiological evidence of acute large bowel obstruction in the absence of a mechanical cause. The condition usually affects elderly people with underlying co-morbidities, and early recognition and appropriate management are essential to reduce the occurrence of life-threatening complications. Methods: A part-systematic review was conducted. This was based on key publications focusing on advances in management. Results and conclusions: Although acute colonic dilatation has been suggested to result from a functional imbalance in autonomic nerve supply, there is little direct evidence for this. Other aetiologies derived from the evolving field of neurogastroenterology remain underexplored. The rationale of treatment is to achieve prompt and effective colonic decompression. Initial management includes supportive interventions that may be followed by pharmacological therapy. Controlled clinical trials have shown that the acetylcholinesterase inhibitor neostigmine is an effective treatment with initial response rates of 60-90 per cent; other drugs for use in this area are in evolution. Colonoscopic decompression is successful in approximately 80 per cent of patients, with other minimally invasive strategies continuing to be developed. Recourse against Judgments in the Netherlands
No full textIn the 1980s, the Netherlands witnessed the start of a comprehensive programme of reform in the fields of judicial organisation and procedure. The reform programme consists of three stages, of which the first has now been completed. This has, amongst other things, resulted in the consolidation of several administrative procedures which were formerly adjudicated by different judicial bodies, in the Arrondissementsrechtbank. Consequently, the Arrondissementsrechtbank has obtained extensive administrative jurisdiction together with its jurisdiction in civil and criminal cases. As part of the reforms in the second stage, a bill has been submitted to Parliament which contains changes in the court structure. It is proposed to abolish the Kantongerecht and to reallocate its jurisdiction to the Arrondissementsrechtbank, transforming the latter court into a general first-instance court. In addition, the bill contains changes in the Code of Civil Procedure. In the present report the author discusses the current situation January 1998

In a series of papers, Brock and Hommes, propose to model economic and financial markets as adaptive belief systems. Asset price fluctuations in adaptive belief systems are characterized by phases of close-to-the-fundamental-price fluctuations, phases of optimism where most agents follow an upward price trend, and phases of pessimism with small or large market crashes. In this paper will be discussed the EMH benchmark and forecasting rules of fundamentals and trend extrapolators. No full textI admit that I have read only two pages of this book (154-5). It seems a non-stop barrage of one-liners. The author does indeed, as the cover illustration suggests, take the bull by the horns! I picked the book up because its frontispiece of Aesop's fox and crow depicts the book's insight: The fox admires the cheese, not the raven (154). My two pages had lots of good zingers in them, like He who believes in promises made at elections has long ears, and may try to eat thistles. I suspect there are many more fable-related proverbs along the way in the book, but for now I will let someone else find them. The role of humoral autoimmunity in gastrointestinal neuromuscular diseases.
No full textDysfunction of the gastrointestinal neuromuscular apparatus (including interstitial cells of Cajal) is presumed to underlie a heterogeneous group of disorders collectively termed gastrointestinal neuromuscular diseases (GINMDs). There is increasing experimental and clinical evidence that some GINMDs are immune-mediated, with cell-mediated dysfunction relatively well studied. Humoral (antibody)-mediated autoimmunity is associated with several well-established acquired neuromuscular diseases and is now implicated in an increasing number of less well-characterised disorders, particularly of the central nervous system. The role of autoimmunity in GINMDs has been less studied. Whilst most work has focused on the presence of antibodies directed to nuclear antigens, particularly in the context of secondary disorders such as paraneoplastic intestinal pseudo-obstruction, the possibility that 'functional' anti-neuronal antibodies directed to membrane-bound ion channels may cause disease (channelopathy) is now also being realised. The evidence for humoral autoimmunity as an etiologic factor in primary (idiopathic) and secondary GINMDs is systematically presented using the original paradigms previously applied to established autoimmune neuromuscular disorders. The presence of anti-enteric neuronal antibodies, although repeatedly demonstrated, still requires the identification of specific neuronal autoantigens and validated evidence of pathogenicity
