V-Y Tensor Fascia Lata for Trochanteric Pressure Sore Treatment

    No full text
There are several alternatives for pressure sore reconstruction, but it is of utmost importance that an appropriate surgical technique be adopted, because although treatment is successful, recurrence of these ulcers is frequent. The purpose of this study was to conduct a precise evaluation of the V-Y tensor fascia lata flap, pointing out its advantages compared with the classical flap and with other possibilities described in the literature. The author employed 20 V-Y tensor fascia lata flaps to correct 20 trochanteric ulcers. The use of the classical tensor fascia lata flap to cover trochanteric ulcers is unquestionable. However, primary closure for flaps wider than 10 cm is difficult, necessitating an eventual skin grafting of the donor site. The V-Y tensor fascia lata allows a covering similar to the classical flap, simplifies primary closure and essentially presents the possibility of numerous reutilizations. </jats:p

Objective To evaluate the biocompatibility of canine fascia lata (FL) in vitro and after FL allograft implantation in dogs with clinical disease. Study design In vitro experiment and small case series. Sample population Six dogs treated with allogenic freeze-dried FL. Methods Fibroblasts were cultured on disks of FL, polypropylene mesh (PM; negative control), and porcine small intestinal submucosa (SIS; positive control). Constructs were compared at 3, 7, and 14 days for water content, DNA amounts, scanning electron microscopy, and histology. Records of dogs treated with FL allografts with follow-up examination were reviewed for signalment, indication for surgery, surgical procedure, and outcomes. All owners were invited to complete a standardized questionnaire for long-term follow-up. Results Water content was greater in FL and SIS than in PM (P = .03). Fascia lata constructs contained more DNA compared with PM constructs at days 7 and 14 (P < .05), whereas SIS constructs did not differ from FL or PM. Fibroblasts appeared spherical and distributed throughout FL constructs, whereas they appeared stellate and remained on the surface of SIS and PM. Fascia lata allografts were implanted in six dogs with surgical conditions. No incisional complications were noted. All dogs had good to excellent long-term outcomes, except one that experienced recurrence of a perineal hernia 2 years after repair. Conclusion In vitro, canine FL allowed attachment and proliferation of fibroblasts throughout layers of the graft. Canine allogenic FL was clinically well tolerated in this small population of dogs. Clinical significance Allogenic FL is biocompatible and can be considered an alternative to SIS for soft tissue augmentation in dogs

Retraining displaced workers : what can developing countries learn from OECD nations?

The governments of most industrial countries provide financial support for adult training programs intended to retrain displaced workers. The author draws lessons from the experience of six industrial countries (Australia, Britain, Canada, Japan, Sweden, and the United States) on how to design and implement such retraining programs in low-income developing nations and middle-income countries. By retraining, the author means both improving job skills and remediating deficiencies in basic education. These are the lessons he emphasizes: Training programs should be independent of the educational system, with its rigid ties to degree requirements and academic schedules; links to employers must be developed and maintained so that trainees have marketable skills on completing the program. Training programs should be designed to minimize trainees'foregone earnings; basic education should be relevant to the jobs the trainees might seek. External providers of education must be made accountable - but with care; the system of accountability should also ensure that the needs of displaced workers most likely to suffer long-term unemployment are met. Not all displaced workers require relatively expansive retraining; some may need only inexpensive job-search assistance services. A permanent, institutionalized training system is preferable to short-term intervention.Labor Standards,Tertiary Education,ICT Policy and Strategies,Health Monitoring&Evaluation,Teaching and Learning

Hawaiianische Käfer der Gattung Blackburnia (Coleoptera, Carabidae, Platynini) : ökologische Spezialisierungen und Implikationen für den Artenschutz

The Hawaiian Islands have arisen in isolation in the middle of the Pacific Ocean due to volcanism unleashed by interaction of mantle-deep thermal plumes with the overlying Pacific Plate (MONTELLI et al. 2004, ABOUCHAMI et al. 2005). This volcano “factory” has produced a consistently present string of islands increasing in age from the current Big Island of Hawaii (500,000 years old) northwestwardly to the island of Kure, estimated to be 28-30 million years old (CARSON & CLAGUE 1995). Successively colonizing and proliferating on these islands since Miocene time (LIEBHERR 2005), are beetles classified in the carabid beetle genus Blackburnia SHARP, 1878 (LIEBHERR & ZIMMERMAN 2000). The 132 known Blackburnia species are arrayed in four successive adelphotaxa. The monotypic subgenus Protocaccus LIEBHERR & ZIMMERMAN, 2000, adelphotaxon to the rest of the radiation, is represented by B. mandibularis LIEBHERR, 2000 of Kauai. The next-diverging clade is classified as subgenus Colpocaccus SHARP, 1903, and is composed of four flightcapable species. The last two adelphotaxa are the nominate subgenus Blackburnia and subgenus Metromenus SHARP, 1884. The former includes 52 species, and is based on a ground-plan ancestor that was capable of dispersal by winged flight. This clade exhibits the greatest anagenetic diversification of all four clades, with various subgroups exhibiting extensive modifications of the external cuticle, including thickened, ridged, and variously shaped pronota and elytra, as well as elongate legs, and extensive specializations of the male and female genitalia (LIEBHERR & ZIMMERMAN 1998, 2000). Taxa exhibiting cuticular modifications are all brachypterous, suggesting that loss of metathoracic wings was a requisite precursor to modification of body armature. All 75 species of subgenus Metromenus are characterized by brachyptery. These also exhibit various body forms however the member taxa are never characterized by the thickened cuticle and associated modifications seen among taxa of sg. Blackburnia. Monophyly of the latter three subgenera was corroborated using molecular sequence data (CRYAN et al. 2001), though basal relationships of the three clades were resolved so that Colpocaccus and Metromenus were construed as adelphotaxa. In this study, morphological characters, and ecological and genetic characteristics of the various clades compared. These comparisons illustrate the coordinated diversification of ecological and genetic traits, and how these are associated with different levels of speciation. These traits are then put in the context of species endangerment, assessed using biotic survey data started in the 19th Century, and continuing during present-day efforts to completely describe and characterize the Blackburnia fauna.

29ı 30; Markham 1988: 57 (list); Kensley and Schotte 1989: 112 (Table 2); Brasil-Lima 1998: 639 (list); Camp 1998: 133 (list); An et al. 2015; 47ı 48 (mention)ı 61 (key); Baeza et al. 2018: 1 – 11 ı figs. 1 – 4. Material examined Brazil: mature female (3.85 mm TL)ı Praia do Pachecoı Cearáı 15/ V / 2003ı Colls. Victor Torresı Daniel Malledo & Daniel Laovoı infesting right branchial chamber of ovigerous female Alpheus packardii Kingsleyı 1880 (5.4 mm CL) (UFRGS 6629). Remarks Based on the redescription by Markham (1985) and key of An et al. (2015) ı the present specimens are closest in morphology to P. lata (Nierstrasz and Brender à Brandisı 1929) (Figure 1 (aıb)). Within Parabopyrellaı P. lata is considered to be part of the ‘ C3 ’ group which has a deeply medially incised pleotelson and longı divergent posterolateral lobes (An et al. 2015). According to Markham (1988) ı P. lata has a known range in the northwest Atlantic (Florida from Cape Canaveral to Homosassa Springsı including Bahamas) and the Caribbean Sea (between St. Thoması Virgin Islands and Sabaı Lesser Antilles) where it has been reported parasitising Alpheus normanni Kingsleyı 1878 and unidentified alpheids (Markham 1985; Boyko 2006; Baeza et al. 2018). The range of hosts for P. lata has expanded to include the shrimp Lysmata boggessi Rhyne and Linı 2006 (Boyko 2006; Baeza et al. 2018). In additionı Lemos de Castro (1965) recorded the species in Brazil (Rio de Janeiro and São Paulo) infesting Alpheus sp. and the gebiid Upogebia a ffi nis (Sayı 1818). This last record was considered questionable by Markham (1985) and Boyko (2006) due to it being the only record of a Bopyrinae isopod parasitising a species outside of the infraorder Caridea. In the present studyı a female specimen found parasitising a new hostı Alpheus packardii Kingsleyı 1880 ı has been preliminarily assigned to P. lata. It is also the first record of the species for the state of Cearáı Brazil. The present material differs from Markham ’ s (1985) Florida specimens in the following: the posterior article of oostegite 1 is more elongate and broadly rounded (Figure 1 (hıi)) (versus more acute in the Florida specimens) and the inner lobes of the barbula are oriented laterally (Figure 1 (d)) (versus inner lobes of the barbula oriented medially in the Florida specimens). Boyko (2006) analysed one bopyrid female (9.6 mm TL) parasitising the left branchial chamber of a female L. boggessi (14.7 mm CL) and identified it as P. lata. The author also pointed out the differences between P. lata and P. mortenseni (Nierstrasz and Brender à Brandisı 1929). The specimen was characterised by the broad shape of the anterior body in dorsal viewı the irregular and asymmetrical development of the dorsolateral bosses and coxal plate on the first pereomereı the pleotelson medially indentedı the posterior article of oostegite 1 broadly rounded and weakly curved and the maxilliped palp as an elongated spur with few setae (Boyko 2006). Neverthelessı the pleotelson with acute projections (Figure 1 (aıb)) (versus quadrangular projections in Boyko ’ s specimen) and the presence of ten of setae in the maxilliped palp (Figure 1 (eıf)) (versus four in Boyko ’ s specimen) distinguish the present material from that identified by Boyko (2006). The absence of illustrations for other appendages make more accurate comparisons between these two specimens difficultı but these morphological differences along with the hippolytid host raise the possibility that the material analysed by Boyko (2006) could be a different species than reported by Markham (1985) and that of our study. Howeverı based on the fact that only one female was collected from Brazil and lack of a male specimen for comparisonı there is not enough data to show that the specimen analysed here constitutes a new species. Additional material is needed to elucidate this question.Published as part of Horch, Felipe Bezerra Ribeiro ı Amanda Porciuncula & Williams, Jason D., 2019, New occurrences and host records for two species of parasitic isopods (Isopodaı Cymothoidaı Bopyridae) associated with caridean shrimps (Decapodaı Caridea) from Brazil, pp. 2437-2447 in Journal of Natural History 53 (39) on page 2438, DOI: 10.1080/00222933.2019.1704589, http://zenodo.org/record/366637

Ausgehend von diesem Spannungsverhältnis zwischen Theorie und Praxis erwägt er, wie das Interesse an einem konzentrierten Grundbuchverfahren, die Richtigkeitsgewähr des Grundbuchs und die formalistischen Zwänge des Liegenschaftsrechts de lege ferenda schonender austariert werden könnten.»Forms of Decision in Land Register Procedural Law de lege lata and ferenda«: Stefan Bohlsen examines the forms of decision in land register procedural law on an application for registration that is not immediately enforceable, structures their areas of application and sheds light on their interrelationship with real erstate and enforcement law. The author comes to the conclusion that the current legal situation is not sufficiently taken into account in the practical work of the land registry. He considers, how the interest in a concentrated land register procedure, the guarantee of correctness of the land register and the formalistic constraints of real estate law could be balanced more gently de lege ferenda
