    Does a closed reduction improve aesthetical and functional outcome after nasal fracture?

    Background and purposes: How closed reduction (CR) to repair a nasal fracture affects the patient's quality of life (QoL) has not been investigated. Here, we assessed QoL before and after CR using disease-specific questionnaires and compared the QoL scores of patients with nasal fractures with normative scores from a reference cohort. Methods: This was a prospective study of 96 patients with nasal fractures undergoing CR. Patients were interviewed about aesthetic, functional, and QoL issues before and after surgery using the Functional Rhinoplasty Outcome Inventory (FROI-17) and the Rhinoplasty Outcome Evaluation (ROE). Photographs of the nasal area were taken before and after surgery and reviewed. Data were compared with those from a reference cohort (n = 1000). Results: Most fractures were type I (80.6%) and most were caused by sport-related accidents (36.5%). The ROE scores increased from 67.3 preoperatively to 73.4 postoperatively (p = 0.001). The FROI-17 also improved, indicating the overall effect of the nose on QoL (p = 0.002). Compared with the reference cohort, patients felt more affected by nasal symptoms before surgery (- 9.37, p = 0.02) than by more general aspects. ROE scores returned to normative values after surgery (p < 0.001). The postoperative cohort had lower scores for the FROI-17 item overall effects of the nose on QoL than the reference cohort did, although the nasal symptom score remained higher in patients than in reference controls. Conclusions: This study showed that CR can improve the aesthetical but not the functional outcome of the nose

    Biomechanical methods for the assessment of fracture repair

    The progress of fracture healing is directly related to an increasing stiffness and strength of the healing fracture. Similarly the weight bearing capacity of a bone directly relates to the mechanical stability of the fracture. Therefore, assessing the progress of fracture repair can be based on the measurement of the mechanical stability of the healing fracture. However, fracture stability is difficult to assess directly due to various obstacles of which shielding of the mechanical properties by the fracture fixation construct is the most relevant one. Several assessment methods have been proposed to overcome these obstacles and to obtain some sort of mechanical surrogate describing the stability of the fracture. The most direct method is the measurement of the flexibility of a fracture under a given external load, which comprises the challenge of accurately measuring the deformation of the bone. Alternative approaches include the measurement of load share between implant and bone by internal or by external sensors. A direct 3 dimensional measurement of bone displacement is provided by radiostereometric analysis which can assess fracture migration and can detect fracture movement under load. More indirect mechanical methods induce cyclic perturbations within the bone and measure the response as a function of healing time. At lower frequencies the perturbations are induced in the form of vibration and at higher frequencies in the form of ultrasonic waves. Both methods provide surrogates for the mechanical properties at the fracture site. Although biomechanical properties of a healing fracture provide a direct and clinically relevant measure for fracture healing, their application will in the near future be limited to clinical studies or research settings.P.     A avaliação da educação é um assunto recorrente em todas as Instituições de Ensino Superior do Brasil. A partir da criação do SINAES – Sistema Nacional de Avaliação da Educação Superior, o MEC – Ministério da Educação e Cultura estabeleceu dez dimensões que devem ser avaliadas na educação superior, entre elas, a da percepção do aluno acerca da qualidade do ensino. Este artigo busca elucidar sobre: A Satisfação do Cliente em Instituições de Ensino Superior, a partir do estudo nas Comissões Próprias de Avaliação (CPAs) em quatro universidades privadas da região Sul do Brasil. A pesquisa visa compreender até que ponto os instrumentos de avaliação aplicados em IES contemplam a complexidade da relação aluno (educando) e universidade (educador)? E quais os seus reflexos na formulação de estratégias organizacionais? Constituem-se como fontes de dados os relatórios elaborados pelas CPAs e as entrevistas realizadas com os coordenadores do setor em cada IES. A avaliação institucional ainda não está arraigada à cultura das Instituições. A baixa adesão a pesquisa e a dificuldade de mensurar adequadamente a satisfação do aluno são alguns dos problemas do processo. Buscar atender os anseios do aluno, satisfazendo suas expectativas, pode ser o prenúncio para a melhoria da qualidade de ensino da universidade
