    Haglund Excision and Suture Bridge Repair

    Background: Haglund deformity and insertional Achilles tendinopathy involve a degenerative process demonstrating disorganized collagen and mucoid degenerative material causing pain and swelling. Indications: Operative management through a distally based midline incision should be explored if symptoms are refractory to nonoperative management (activity and shoewear modifications, heel lifts, physical therapy, and heel sleeves). Direct lateral approach with small longitudinal split and removal of lateral prominence “pump bump” is performed when a patient has pain isolated solely to the prominence. We feel that endoscopic removal does not allow for appropriate evaluative of degenerative tissue. Techniques: Through a distally based incision, we dissect to the level of the paratenon and make one smooth cut through paratenon and tendon. We then excise the mucoid, degenerative tissue. Removal of Haglund deformity and exostosis is carefully accomplished with a saw. We then utilize a rongeur to smooth Haglund deformity and any remaining medial/lateral calcaneal prominences. We then complete a dual-row speed bridge with 4 suture anchors (horizontal mattress) and a modified Mason-Allen technique. A Thompson test is then repeated and the longitudinal split is repaired. Postoperatively, we have patients weight bear as tolerated in a boot for with suture removal and transition to shoewear at the 2-week mark. We begin range of motion and activity once the wound has healed. We expect return to sport beginning at 3 months with full return by 6 months. Results: The current body of literature shows significant patient satisfaction and minimal risk of complication. Some authors have shown significant improvement in American Orthopaedic Foot and Ankle Score (AOFAS) scores, with only 1 patient requiring a flexor hallucis longus (FHL) transfer and no complications. Further studies identify early weight-bearing in Achilles suture bridge technique to have some promise with improvement in visual analog scale (VAS)/AOFAS scores, though there is risk of wound breakdown. Discussion/Conclusion: An Achilles suture bridge technique with early weight-bearing is a viable option for patients with Haglund deformity, and insertional Achilles tendinopathy who have failed conservative management.     Weightbearing Protocols and Outcomes in Open Surgical Management of Haglund Syndrome: A Large Retrospective Analysis

    Background: Following insertional Achilles tendinopathy debridement and Haglund prominence resection for Haglund syndrome, patients undergo varying degrees of weightbearing limitation (weightbearing as tolerated [WBAT], partial weightbearing [PWB], touchdown weightbearing [TDWB], and nonweightbearing [NWB]). Given the scarcity of large-scale literature on the topic, the purpose of this study is to evaluate the impact of postoperative weightbearing protocols on outcomes after open surgical management of Haglund syndrome. Methods: This was a retrospective cohort study of patients who underwent open surgical management for Haglund syndrome between January 2015 and December 2023 at a single academic institution by fellowship-trained foot and ankle surgeons. Patients were excluded if they underwent concurrent operative management of additional foot pathologies. Patient demographics, comorbidities, surgical techniques, and postoperative weightbearing protocols were recorded. Complications (Achilles tendon rupture, wound breakdown/infection, persistent pain, plantarflexion weakness) and revision rates were compared across weightbearing protocols. Statistical analysis was conducted using R with significance set at P  <.05. Results: Three-hundred eighty-seven patients were included (mean age 55.2 years, 66.1% female) with a mean follow-up of 10.1 (range: 0-86.3) months. The most common regimen was NWB (n = 268; 69.3%) followed by TDWB (n = 56; 14.5%), WBAT (n = 54; 14.0%), and PWB (n = 9; 2.3%). There were no significant differences in complications between the weightbearing protocols ( P  = .48354). Complications included persistent pain (n = 40; 10.3%), weakness (n = 6; 1.6%), wound breakdown/infection (n = 33; 8.5%), and rupture (n = 1; 0.3%)]. Revision surgery occurred in 1.8% (n = 7). Conclusion: This large cohort study found no significant association between postoperative weightbearing protocols and outcomes following open surgical treatment for Haglund syndrome at a mean follow-up of 10.1 months. This study provides evidence that surgeons can individualize appropriate weightbearing protocols based on patient needs and preferences when treating Haglund syndrome with Achilles debridement and Haglund resection.     We conjecture two combinatorial interpretations for the symmetric function ∆eken, where ∆f is an eigenoperator for the modified Macdonald polynomials defined by Bergeron, Garsia, Haiman, and Tesler. Both interpretations can be seen as generalizations of the Shuffle Conjecture, a statement originally conjectured by Haglund, Haiman, Remmel, Loehr, and Ulyanov and recently proved by Carlsson and Mellit. We show how previous work of the second and third authors on Tesler matrices and ordered set partitions can be used to verify several cases of our conjectures. Furthermore, we use a reciprocity identity and LLT polynomials to prove another case. Finally, we show how our conjectures inspire 4-variable generalizations of the Catalan numbers, extending work of Garsia, Haiman, and the first author

    Type C parking functions and a zeta map

    International audienceWe introduce type CC parking functions, encoded as vertically labelled lattice paths and endowed with a statistic dinv'. We define a bijection from type CC parking functions to regions of the Shi arrangement of type CC, encoded as diagonally labelled ballot paths and endowed with a natural statistic area'. This bijection is a natural analogue of the zeta map of Haglund and Loehr and maps dinv' to area'.     Nanoscale dolmen structure exhibiting a tunable Fano resonance

Haglund Jr.A Fano resonance is an asymmetric, resonant scattering phenomenon which occurs in a multitude of fields, such as atomic physics, nuclear physics, nonlinear optics, and nanophotonics. The Fano resonance is a many-particle excitation arising from a single-particle excitation, and occurs due to the interference of a narrow discrete resonance overlapping with a spectrally broad resonance. Multi-element nanoparticles are explored as a means to realize this resonance type, which has a characteristic, steep dispersion useful in sensors, among other devices. A gold dolmen nanostructure consisting of a bright, radiative, dipolar mode coupled with a dark, quadrupolar mode is investigated, which produces the plasmonic analogue of electromagnetically induced transparency through the interference of the bright and dark modes. Additionally, the plasmonic resonance of a metallic nanostructure is extremely sensitive to its local dielectric environment. Vanadium dioxide exhibits a large change in its dielectric function during its metal to insulator phase transition. The combination of the gold dolmen nanostructure with vanadium dioxide produces a tunable Fano resonance. The dimensions of the structure are optimized such that the resonance is near symmetric in shape and located in the near infrared to allow for spectroscopic measurement. The author is unable to obtain experimental results of this hybrid structure, but previous work, combined with simulation data, suggest the proposed structure will exhibit the expected result.United States. IFRS regulations contribute to the understanding of accounting regarding write-downs and the incentives companies must influence accounting. Empirical basis: The essay studies companies listed on the Stockholm Stock Exchange's Small Cap, Mid Cap and Large Cap between the years 2010-2023. Results and conclusion: The study finds no significant relationship between write-downs and CEO changes. The conclusion is thus that Swedish listed companies do not carry out big baths when changing the managing director. However, the tests have shown significant relationships between the various write-downs and total assets, return on assets and leverage.

    Pathways to Social Transformation: Delhi and the Human Right to Housing

    abstract: The objective of this study was to better understand promising pathways to realizing human rights norms in the context of rapidly developing cities, and the role that the courts play in this process. Scholars have already started to ask these larger questions of social transformation; however, there continues to be a need for further research since the answers are vast and context-dependent. In order to contribute to these larger conversations, this project examined a key social right in Delhi \u2014 the right to housing. This study relied on interviews with key actors in Delhi's housing sector as well as a review of housing rights cases in the Delhi High Court in order to understand what mechanisms various actors utilize in the context of Delhi to realize the human right to housing on the ground. These two types of data were compared and contrasted to past research on human rights scholarship, law and social literature, and studies on urbanization. Two frameworks from these bodies of knowledge, the MAPs framework developed by Haglund and Aggarwal (2011) and the triangular framework created by Gauri and Brinks (2008), were utilized in particular to analyze interview and court data. Overall, this study found that the courts in India are advocates for housing rights, but that their advocacy is often limited, cautious, and influenced by a pattern of bias against populations without legal title to land. This study also found that communities and their allies are often more successful in realizing the right to housing when they combine litigation with other non-legal social change mechanisms. Consequently, it appears that the role of the courts in realizing ESR in Delhi is both complicated and limited, which means that pathways toward ESR realization are more promising when they incorporate non-legal mechanisms alongside court action
