The value of patient-generated health data from self-management mHealth solutions for healthcare professionals in chronic care
Abstract
The rapid advancement of mobile technologies has created opportunities to enable more active participation in healthcare among patients. Self-management mobile health (mHealth) solutions, such as applications and wearables, can support the management of noncommunicable diseases (NCDs) through the creation of patient-generated health data (PGHD). However, despite the increasing number of mHealth solutions being developed, their utilisation in clinical workflows remains limited. Healthcare professionals (HCPs) play a crucial role in ensuring the sustained engagement of patients with these solutions. However, the extent to which they perceive these solutions and the PGHD they produce as beneficial additions to clinical practice remains underexplored.
This dissertation examines how PGHD from self-management mHealth solutions can be leveraged by HCPs to enhance the care process for NCDs. It investigates how mHealth solutions can generate value beyond individual use to foster a collaborative relationship between patients and HCPs and support HCPs in specific clinical tasks. A multimethod research approach, including a systematic literature review, interviews, surveys, and focus groups, was employed to explore HCPs' perspectives on the perceived value of PGHD in clinical care and the optimal ways to present PGHD to support clinical decision-making.
The findings indicate that HCPs selectively engage with PGHD, prioritising data that provide actionable insights and facilitate informed decision-making. Additionally, the visual representation of PGHD plays a critical role in its clinical utility, emphasising the need for clear, intuitive, and contextually relevant visualisations. This dissertation contributes to both theoretical and practical domains, expanding knowledge on the role of PGHD in mHealth solutions in the care of NCDs, such as multiple sclerosis, which was used as the case study. It aims to provide evidence-based recommendations for developers, healthcare organisations, and policymakers to optimise the design and development of self-management mHealth solutions to target better integration into healthcare workflows. By addressing key barriers to adoption and emphasising the importance of stakeholder collaboration, this research underscores the potential of PGHD-driven mHealth solutions to enhance the efficiency and effectiveness of NCDs' management. Mobiiliteknologioiden nopea kehitys luo uusia mahdollisuuksia potilaiden aktiivisemmalle osallistumiselle terveydenhuoltoon. Itsehoidon mobiiliterveysratkaisut, kuten sovellukset ja puettavat laitteet, voivat tukea ei-tarttuvien sairauksien (NCD) hallintaa potilaan tuottaman terveystiedon (PGHD) avulla. Vaikka mobiiliterveysratkaisujen määrä kasvaa jatkuvasti, niiden käyttö kliinisissä työnkuluissa on edelleen rajallista. Terveydenhuollon ammattilaiset (HCP) ovat keskeisessä asemassa varmistamassa potilaiden jatkuvaa sitoutumista näihin ratkaisuihin. Kuitenkin se, miten HCP:t kokevat nämä ratkaisut ja niistä saatavan PGHD:n kliinistä käytäntöä tukevina, on edelleen vähän tutkittu.
Tämä väitöskirja tarkastelee, kuinka itsehoidon mobiiliterveysratkaisujen tuottamaa PGHD:tä voidaan hyödyntää HCP:iden työssä NCD-hoitoprosessin tehostamiseksi. Se tutkii, kuinka mobiiliterveysratkaisut voivat tuottaa arvoa yksilöllisen käytön ohella edistämällä yhteistyötä potilaiden ja HCP:iden välillä sekä tukemalla HCP:itä tietyissä kliinisissä tehtävissä. Tutkimuksessa käytettiin monimenetelmällistä lähestymistapaa, joka sisälsi systemaattisen kirjallisuuskatsauksen, haastatteluja, kyselyitä ja fokusryhmiä. Näiden menetelmien avulla kartoitettiin HCP:iden näkemyksiä PGHD:n koetusta arvosta kliinisessä hoidossa.
Tulokset osoittavat, että HCP:t suhtautuvat PGHD:hen valikoivasti ja priorisoivat tietoa, joka tarjoaa toiminnallisia oivalluksia. Lisäksi PGHD:n visuaalinen esitystapa on kriittinen sen kliiniselle hyödylle, mikä korostaa selkeiden, intuitiivisten ja asiayhteyteen sopivien visualisointien tarvetta. Tämä väitöskirja edistää teoreettista ja käytännöllistä ymmärrystä PGHD:n roolista mobiiliterveysratkaisuissa NCD-hoidossa, erityisesti multippeliskleroosin (MS) kontekstissa, joka toimi tapaustutkimuksen kohteena. Tutkimuksen tavoitteena on tarjota näyttöön perustuvia suosituksia mobiiliterveysratkaisujen kehittäjille, terveydenhuollon organisaatioille ja päättäjille, jotta itsehoidon mobiiliterveysratkaisujen suunnittelua ja kehittämistä voidaan optimoida paremman integroinnin saavuttamiseksi terveydenhuollon työnkulkuihin.

Representação da Informação em Arquivos Deslocados: uma metassíntese de literatura qualitativa
Esta comunicação visa realizar uma síntese qualitativa de literatura relativa à representação da informação em arquivos deslocados. O conceito de “arquivo deslocado” é
polissémico e integra diversos termos na literatura arquivística em distintas línguas (u.g., arquivo removido, expatriado, exilado, migrado, confiscado, contencioso arquivístico, etc.). Desta forma, o objetivo deste estudo consiste em realizar uma prospeção de literatura que informe sobre como a comunidade científica se debruçou sobre a representação da informação em arquivos deslocados. Pretende-se responder à seguinte questão de investigação: “Como as entidades arquivísticas têm representado os arquivos deslocados de acordo com a literatura científica produzida entre 1954 e 2019 no domínio da Ciência da Informação?”. Metodologicamente, esta comunicação encontra-se configurada numa metassíntese orientada à construção de teoria, constituindo-se num tipo de estudo de tipo não reativo, de base documental e exploratória, focalizada em artigos e capítulos em livros publicados em inglês entre 1954 a 2019. A recolha de textos é suportada pela estratégia SPICE, aplicada na pesquisa em bases de dados (WoS e EBSCO). Adotamos a análise de conteúdo de acordo com os pressupostos de Charmaz (2006) e de Finfgeld-Connett (2018). Dos 443 registos, que a pesquisa nos devolveu, incluímos 155 textos que responderam aos propósitos do estudo. Predominam estudos de caso, explícitos ou não, simples ou sobrepostos com outros métodos (análises téorico--conceptuais, métodos biográficos, retórica coletiva), análises históricas, análises teórico-conceptuais, métodos etnográficos, auto-etnográficos e investigação naturalística. Os métodos menos frequentes são a análise de conteúdo ou temática, a análise documental, o estudo longitudinal e o relatório de investigação. Os principais fatores incoativos de remoção de arquivos dos seus contextos originários, que emergiram desta metassíntese, derivam de fenómenos ocorridos em contexto de conflito armado, de descolonização, de sucessão de Estados e do tráfico ilícito de bens culturais. Além disto, emergiram novos casos de arquivos constituídos na diáspora, casos de conflito de custódia em contexto subnacional e a dispersão de arquivos na web/nuvem. Esta diversidade também se manifesta em conjuntos documentais não só com valor histórico e/ou administrativo para as comunidades desapossadas, mas também nas suas múltiplas materialidades (analógico/digital, audiovisual, textual, iconográfico, etc.). Foram constituídos três temas que emergiram da análise de conteúdo em torno do tema suprarreferido: (1) “anarquivismo como (não) representação”: analisámos processos de representação anarquívica praticadas por algumas entidades custodiantes, através da manipulação do conteúdo por motivos políticos e ideológicos no sentido de obscurecer a proveniência ou de descaracterizar a ordem original (u. g. “higienização do passado colonial”, “troféus de guerra”, eufemização); (2) “arquivo do arquivo” (como “meta-arquivo” e “arqui-arquivo”): exploramos como os “arquivos dos arquivos” representam os conjuntos documentais removidos dos seus contextos originários, como forma de empoderar as entidades custodiantes através de mecanismos adotados para o controlo, gestão e determinação de políticas de acesso; e (3) “cânone arquivístico”: verificamos como as representações de arquivos deslocados em instrumentos de acesso visam legitimar cânones arquivísticos institucionais, com ênfase em torno de representações em ambiente digital. Electrochemical Bioelectronics in Drug Delivery: Effect of the Initial Gas Volume
Implantable bioelectronic devices with drug delivery capabilities have emerged as suitable candidates for biomedical applications focusing on localized drug delivery. These classes of miniaturized bioelectronics offer wireless operation and refillable designs that can be used for repeated animal behavioral studies without restricting their motion. The pumping mechanisms of these bioelectronic devices features soft materials, microfluidics, and electrochemical subsystems that can be scaled from behavioral studies in small animals to delivery of life-saving medication in humans. Here, we study the refillable aspect of these bioelectronic systems using an analytic model for the drug delivery time established from the ideal gas law when an initial gas volume is present in the device electrolyte reservoirs. The effect of the initial gas volume in delaying the drug delivery time is captured via a non-dimensional parameter identified as the normalized initial gas volume. An analytical solution is derived from the perturbation method, which agrees well with the numerical solution. These results have relevance in the reusability aspect of these bioelectronic systems since modifying the amount of initial gas in the device reservoirs for different experiments affects the total delivery time and can serve as a tunable parameter to ensure timely and successful delivery of the drug in the target region

Maria da Conceição Soares Machado : identificação e organização do arquivo pessoal
O presente relatório de estágio foi elaborado no âmbito do Mestrado em Ciências da
Documentação e Informação da Faculdade de Letras da Universidade de Lisboa, tendo como
principal objetivo a identificação, a organização, a descrição e a valorização do arquivo pessoal
de Maria da Conceição Soares Machado, antiga aluna da Faculdade de Letras da Universidade
de Lisboa e professora de Grego e Latim no ensino secundário português ao longo do século
XX. O estágio decorreu entre outubro e dezembro de 2024, no Arquivo Histórico da Biblioteca
da mesma Faculdade, sob a orientação do Professor Doutor Carlos Guardado da Silva e do Dr.
António Jorge Ramalho.
O fundo documental em questão, composto por três caixas de documentos em estado
desorganizado, foi doado pela filha da titular e acolhido institucionalmente com o intuito de
garantir a sua preservação e acessibilidade. A intervenção arquivística implicou o
levantamento, análise, estruturação e descrição do acervo, partindo da construção de uma
macroestrutura e, consequentemente, de quadros de classificação que respeitassem a
organicidade dos documentos e refletissem as diferentes facetas da vida da titular, tais como:
aluna, docente, autora, gestora patrimonial, católica, associada e figura familiar.
Metodologicamente, o trabalho adotou uma abordagem qualitativa, combinando o
método biográfico com o método de investigação documental, adequados à natureza do arquivo
pessoal e à necessidade de reconstrução do contexto de produção da informação. Para a
descrição arquivística foi utilizada a Norma Geral Internacional de Descrição Arquivística
(ISAD(G)), com o objetivo de garantir coerência, normalização e facilidade de acesso à
informação.
Paralelamente à organização técnica do acervo, foi realizado um estudo biográfico
aprofundado sobre Maria da Conceição Soares Machado, complementado com visitas a
instituições de ensino onde lecionou, bem como com a análise da sua biblioteca pessoal. Este
estudo permitiu contextualizar o conteúdo do arquivo e destacar a relevância da sua trajetória
intelectual e pedagógica no panorama educativo português.
Os resultados do estágio traduzem-se não só na recuperação e na valorização de um
arquivo pessoal de significativo interesse histórico, académico e cultural, como também na
aplicação prática de conhecimentos arquivísticos, com vista à preservação da memória individual e institucional. O relatório pretende, assim, constituir-se como um contributo para a
reflexão sobre os desafios da organização de arquivos pessoais e para a promoção de boas
práticas no campo da Arquivística.This internship report was prepared within the scope of the Master’s Degree in Ciências
da Documentação e Informação at the Faculdade de Letras da Universidade de Lisboa. Its main
objective was the identification, organization, description, and valorization of the personal
archive of Maria da Conceição Soares Machado, a former student of the Faculdade de Letras
and a teacher of Greek and Latin in Portuguese secondary education throughout the 20th
century.
The internship took place between October and December 2024, at the Arquivo
Histórico da Biblioteca da Faculdade de Letras da Universidade de Letras, under the
supervision of Professor Dr. Carlos Guardado da Silva and Dr. António Jorge Ramalho.
The archival collection in question, consisting of three boxes of untreated documents,
was donated by the holder’s daughter and institutionally accepted with the aim of ensuring its
preservation and accessibility. The archival intervention involved the survey, analysis,
structuring, and description of the collection, starting with the construction of a macrostructure
and, consequently, classification frameworks that respected the organic nature of the
documents and reflected the different facets of the holder’s life, such as: student, teacher,
author, patrimonial manager, Catholic, member of associations, and family figure.
Methodologically, the work adopted a qualitative approach, combining the biographical
method with the documentary research method, both appropriate to the nature of a personal
archive and to the need to reconstruct the context in which the information was produced. For
the archival description, the General International Standard Archival Description (ISAD(G))

In parallel with the technical organization of the collection, an in-depth biographical
study on Maria da Conceição Soares Machado was carried out, complemented by visits to
educational institutions where she taught, as well as an analysis of her personal library. This
study allowed for the contextualization of the archive’s content and highlighted the relevance
of her intellectual and pedagogical journey within the Portuguese educational landscape.
The results of the internship are reflected not only in the recovery and valorization of a
personal archive of significant historical, academic, and cultural interest, but also in the
practical application of archival knowledge, aimed at preserving both individual and
institutional memory. The report thus aims to contribute to the reflection on the challenges of
organizing personal archives and to the promotion of best practices in the fields of Archival
Studies
Public Thresholds: Experimenting with Public Value Creation through Spatial Interventions in Public Buildings
In current times, which are dominated by uncertainty and change, the limits of public and private realms are in a continuous definition. As a condensation of the public sphere, public buildings have turned into thresholds, into activepublic sphere agents that can motivate behaviour and, as a result, produce public values. Public buildings designed for values should be created as unfinished processes instead of objects, leaving room for socio-spatial change and value dynamics. This research will use design-driven methodologiesto show how spatial interventions in existing public buildings can incite public values. Connecting buildings and theories will highlight knowledge gaps leading to working hypothesis tested in experimental spatial interventions. Consequently, and as a case study, the experimentation phase will explore the public role of makerspaces within libraries. A co-creation process, conceptual design and prototyping of spatial solutions will highlight findings on the effects of design decisions and interventions in the public sphere of the future spaces of literacy.History, Form & AestheticsTheory, Territories & Transition
